Dàquán states: Postpartum chùláo (Puerperal Asthenia) occurs when, shortly after delivery, the blood and qi are deficient, and diet has not yet recovered. Before a full month has passed, if the blood and qi are depleted and emaciated due to improper nursing care (There are two theories regarding chùláo; both originate from the puerperium, but one involves external pathogens while the other arises from emotional disturbances. Some cases involve internal injury from diet and diarrhea, or residual blood stasis; these must be distinguished), wind-cold pathogens invade. Wind-cold clashes with blood and qi, failing to warm the skin and muscles, causing deficiency, exhaustion, and fatigue. The patient may lie down or rise intermittently, appear haggard in complexion, and fail to digest food. Wind-cold pathogenic factors affect both the exterior and the Lungs; the Lungs suffer slight cold, leading to cough, dry mouth, dizziness, and pain in all joints. Ying and Wei (Nutritive and Defensive Qi) are invaded by wind pathogens, which flow into the Zang-Fu organs, causing frequent attacks within moments, occasional night sweats, alternating chills and fever resembling malaria, oppression and irritability in the back and shoulders, inability to lift the limbs, heaviness in bed. These are the signs of chùláo.
It is also said: Women who experience difficult labor, exhausting their sinews and strength, and suffering from worry, labor, and overthinking, become deficient, emaciated, short of breath, with alternating chills and fever like malaria, headache, spontaneous sweating, limb fatigue, cough with rebellious phlegm, and cramping/stabbing pain in the abdomen. This is named chùláo.
Dr. Xue says: Regarding the aforementioned symptoms, the primary treatment should be to nourish and support healthy qi. Use Liujunzi Tang (Six Gentlemen Decoction) added with Danggui (Angelicae Sinensis Radix). If Spleen and Lung qi deficiency causes cough and dry mouth, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Maimendong (Ophiopogonis Radix) and Wuweizi (Schisandrae Fructus). If middle qi deficiency causes dry mouth and dizziness, use Buzhong Yiqi Tang added with Manjingzi (Vittatenii Fructus). If Liver channel blood deficiency causes limb pain, use Siwu Tang (Four Substances Decoction) added with Renshen (Ginseng Radix) and Baizhu (Atractylodis Macrocephalae Rhizoma). If Liver and Kidney deficiency causes spontaneous and night sweats with alternating chills and fever, use Liuwei Wan (Six-Ingredient Pill with Rehmannia) added with Wuweizi. If Spleen deficiency and weak blood cause abdominal pain and irregular menstruation, use Bazhen Tang (Eight Treasures Decoction) with doubled Baizhu. If Spleen deficiency and dry blood cause skin itching, use JiaWei Xiaoyao San (Modified Free and Easy Wanderer Powder). Generally, this condition often results from Spleen and Stomach weakness and reduced diet, leading to fatigue in all meridians; thus, tonifying the Spleen and Stomach is key. Once diet enters, essence and qi are transformed, and the Zang-Fu organs have reliance upon them, so the disease heals itself. Refer to the treatise on fevers due to deficiency and damage for further guidance.
Dr. Wang treated a woman whose delivery was less than a month prior. Due to anger, her blood and qi flowed out like water, stopping only after three days. She then developed chùláo, with weak limbs, sweating during sleep, tidal fever in the late afternoon, dry mouth, and burning sensations in the five centers (palms, soles, and chest). Physicians used Chaihu (Bupleuri Radix), Huangqin (Scutellariae Radix), Bohe (Menthae Haplocalycis Herba), etc., but her heat intensified. Her pulse was wiry, large, and powerless. This is chùláo. He prescribed Siwu Tang (1 liang) added with Huhuanglian (Coptidis Rhizoma variant/Anemarrhenae Rhizoma? Note: Text says Hu Huang Lian, likely Coptis or similar bitter cold herb, but context suggests clearing deficiency heat), Qinjiao (Gentianae Macrophyllae Radix), and Qinghao (Artemisiae Annuae Herba) each 0.5 qian. After several doses, the fever subsided and body cooled. Later, she took one dose of Huanglian Bazhen Wan (Coptis Eight Treasures Pill) and stabilized. (Note: A wiry, large, powerless pulse resembles Ge Mai (Leather Pulse), which indicates deficiency. Huhuanglian is not typically indicated for pure deficiency without heat signs; if it were effective here, the pulse would likely not be powerless. Please investigate carefully.)
Baifuran San (White Poria Powder): Treats postpartum chùláo with pain in head, eyes, and limbs, and alternating chills and fever like malaria.
Baifuran (Poria) (1 liang), Danggui (Angelicae Sinensis Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Shudihuang (Rehmanniae Radix Preparata), Baishao (Paeoniae Radix Alba, stir-fried), Huangqi (Astragali Radix), Renshen (Ginseng Radix), Guixin (Cinnamomi Cortex) (each 0.5 liang).
First, boil water (3 zhan) with one pig kidney, 3 slices of ginger, and 3 jujubes until reduced to 2 zhan. Remove these three items. Add 0.5 liang of the herbal powder, boil to 1 zhan, strain, and take warm in three divided doses before meals. (This is a variation of Shiquan Dabu Tang (Ten Significant Tonification Decoction), lacking Baizhu and Gancao, but using Baifuran as the sovereign herb and pig kidney as the assistant to harmonize Heart and Kidney, resolving chills and fever.)
Huangqi Jianzhong San (Astragalus Establishing the Middle Powder): Treats various postpartum deficiencies, fever, or aversion to cold with abdominal pain.
Huangqi (Astragali Radix, stir-fried), Rougui (Cinnamomi Cortex) (each 1 liang), Baishao (Paeoniae Radix Alba, stir-fried, 2 liang), Gancao (Licorice Root, stir-fried, 7 qian).
Chop. Take 5 qian per dose, decoct with jujubes and ginger in water, twice or thrice daily. For severe deficiency, add Fuzi (Aconiti Lateralis Radix Preparata). ("Establishing the Middle" refers to strengthening the Earth element. Baishao and Gancao resemble the transformation of Jia-Ji into Earth. Rougui treats wiry, deficient, cold pulses with abdominal pain. Huangqi supplements exterior qi to assist deficient middle qi.)
Danggui Jianzhong San (Angelica Establishing the Middle Powder): Treats postpartum labor injury, deficiency and insufficiency, abdominal pain, shortness of breath, lower abdominal tension, pain radiating to the lower back, spontaneous sweating, loss of appetite. From delivery until the end of the first month, take three times daily to strengthen the body. (This formula specifically treats conditions arising from blood and qi deficiency and cold. Adding Yitang (Maltose) is an excellent method for establishing the middle.)
Danggui (Angelicae Sinensis Radix) (4 liang), Baishao (Paeoniae Radix Alba) (6 liang), Guixin (Cinnamomi Cortex) (3 liang), Huangqi (Astragali Radix) (1.5 liang).
Chop. Take 4 qian per dose, add ginger and jujubes, decoct in water, add a piece of Yitang (Maltose), boil again, and take warm. If there is metrorrhagia or epistaxis, add Ejiao (Asini Corii Colla) and Dihuang (Rehmanniae Radix).
JiaWei Foshou San (Modified Buddha's Hand Powder): Treats postpartum blood deficiency, fatigue-induced night sweats, lethargy and weakness, cough with phlegm.
Danggui (Angelicae Sinensis Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Huangqi (Astragali Radix, honey-fried) (each 1 liang), Chaihu (Bupleuri Radix), Qianhu (Peucedani Radix) (each 1.5 qian).
Coarsely crush. Take 5 qian per dose, decoct in 1 da zhan of water with 3 cun pieces of peach and willow branches, 1 jujube, 1 wumei (Mume Fructus), and 1 slice of ginger. (Using Chaihu, Qianhu, and Chuanxiong for night sweats seems inappropriate; using Huangqi for cough with phlegm also seems difficult. However, as the sovereign herbs, while Chaihu and Qianhu are used sparingly, the disparity in dosage is significant. Ultimately, postpartum conditions prioritize blood and qi, hence their use. Suitable only for those without exogenous pathogens.) If there is phlegm, do not use Wumei.
Chaihu Siwu Tang (Bupleurum Four Substances Decoction): Treats prolonged postpartum deficiency and asthenia with a floating and large pulse.
See the section on alternating chills and fever.
Sanhe San (Three Harmonies Powder): Treats prolonged postpartum deficiency and asthenia where acupuncture and medication have failed. (This combines Siwu Tang and Sijunzi Tang, removing Baizhu and adding Huangqi, merged with Xiao Chaihu Tang. Integrating the intent of these three formulas reveals the true principle.)
See the section on fever.
Yunqi Shudihuang San (Yunqi Prepared Rehmannia Powder): Treats postpartum chùláo caused by physical weakness and unconsolidated qi, relapse due to overexertion, resulting in painful limbs, intermittent chills and fever, and loss of appetite.
Shudihuang (Rehmanniae Radix Preparata), Renshen (Ginseng Radix), Baishao (Paeoniae Radix Alba), Baifuran (Poria), Baizhu (Atractylodis Macrocephalae Rhizoma), Xuduan (Dipsaci Radix) (each 1 liang), Huangqi (Astragali Radix), Danggui (Angelicae Sinensis Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Wuweizi (Schisandrae Fructus), Guixin (Cinnamomi Cortex) (each 7.5 qian).
Coarsely crush. Take 4 qian per dose, decoct with 3 slices of ginger and 1 jujube in water. (Chùláo generally belongs to dual deficiency of blood and qi, so the method follows Shiquan Dabu Tang. Xuduan treats various deficiencies and uterine leakage in women. Wuweizi generates fluids and calms the spirit; adding Maimendong is even better. Removing three herbs and adding three makes it another "Three Harmonies".) Dàquán's formula includes Maimendong (7.5 qian). Another version lacks Guixin, Wuweizi, and Xuduan, but includes Chaihu, Huangqin, and Banxia (each 7.5 qian).
Huangqi Wan (Astragalus Pill): Treats postpartum chùláo with advancing and retreating chills and fever, dizziness and headache, soreness and pain in bone joints, and emaciation and weakness.
Huangqi (Astragali Radix), Biejia (Testudinis Carapax et Plastrum), Danggui (Angelicae Sinensis Radix, stir-fried) (each 1 liang), Chuanxiong (Ligustici Chuanxiong Rhizoma), Baishao (Paeoniae Radix Alba), Guixin (Cinnamomi Cortex), Xuduan (Dipsaci Radix), Niuxi (Achyranthis Bidentatae Radix), Rongrong (Cibotii Rhizoma), Baiziren (Platycladi Semen), Chenxiang (Aquilariae Lignum Resinatum), Zhike (Aurantii Fructus Immaturus) (each 7.5 qian), Wuweizi (Schisandrae Fructus), Shudihuang (Rehmanniae Radix Preparata) (each 0.5 liang).
Grind into fine powder, make pills with refined honey the size of paulownia seeds. Take 40-50 pills per dose after meals with gruel or drink. (Note: This formula relies on Siwu Tang, Huangqi, Xuduan, Guixin, and Wuweizi. Biejia clears bone-joint deficiency heat; Baiziren moistens heart fluids; Niuxi alleviates foot/knee pain; Chenxiang connects heaven and earth; Rongrong is vital; Zhike benefits the chest/diaphragm. Reviewing previous formulas, the main theme is tonifying blood and qi, with variations for warming, clearing, astringing, dispersing, breaking stasis, or moving qi according to accompanying symptoms. Practitioners should not be rigid.)
Shizi Tang (Kidney Stone Soup - referring to Pig Kidney): Treats postpartum emaciation, shortness of breath, alternating chills and fever like malaria, limb pain, and sallow complexion, named chùláo.
Zhu Shizi (Pig Kidneys, 1 pair, remove fat membrane, cut into four), Xiangchi (Fermented Soybean), Cong (Scallion, chopped), Jingmi (Oryza Sativa Glutinosa), Danggui (Angelicae Sinensis Radix), Baishao (Paeoniae Radix Alba) (each 2 liang).
Coarsely crush. Divide into two doses. Each dose uses 3 he of rice, cook to obtain a small bowl, strain, and take in three divided doses. (Shizi refers to pig kidneys, used to assist other soups, restoring their original nature. Observing Cong and Chi suggests external pathogens; substituting Zhimu suggests internal heat; this must be distinguished.) Guangji Fang lacks Baishao but has Renshen. Xu Renze Fang lacks Xiangchi but has 2 liang Ginger, 1 liang Guixin, and only 1 liang Congbai.
Renshen Biejia San (Ginseng and Turtle Shell Powder): Treats chùláo occurring within 100 days postpartum due to bodily deficiency, weak blood and qi, lack of proper care, or injury from labor. Symptoms include emaciation, intermittent rising/lying, indigestion, occasional cough, dizziness and headache, unpredictable onset, night sweats, alternating chills and fever like malaria, stiffness in back/shoulders, heaviness in bed. This formula is highly effective.
Huangqi (Astragali Radix), Biejia (Testudinis Carapax et Plastrum) (each 1 liang), Niuxi (Achyranthis Bidentatae Radix, 7.5 qian), Renshen (Ginseng Radix, preferably used), Baifuran (Poria), Danggui (Angelicae Sinensis Radix), Baishao (Paeoniae Radix Alba), Sangjisheng (Taxilli Caulis), Maimendong (Ophiopogonis Radix, remove heart), Shudihuang (Rehmanniae Radix Preparata), Taoren (Persicae Semen), Guixin (Cinnamomi Cortex), Gancao (Licorice Root) (each 0.5 liang), Xuduan (Dipsaci Radix, 2.5 qian).
Grind into fine powder. For each dose, first take one pig kidney, remove fascia, boil in 2 da zhan of water with 0.5 fen of ginger and 3 jujubes until reduced to 1 zhan. Remove kidney, ginger, and jujubes. Add 2 qian of herbal powder, 3 cun of Congbai (Scallion White), 1 Wumei (Mume Fructus), and 5 spikes of Jingjie (Schizonepetae Spica). Boil to 0.7 portion, strain, and take warm on an empty stomach and before evening meal. Divine effect. (This is Shiquan Dabu Tang minus Chuanxiong and Baizhu, plus Biejia, Niuxi, Sangjisheng, Xuduan, Taoren, and Maidong to treat chùláo, embodying the idea of eliminating the old and generating the new. Sangjisheng treats residual postpartum ailments. Pig kidneys are used because the fetus attaches to the waist, borrowing them for assistance.)
Hu Shi Mudan San (Dr. Hu's Peony Powder): Treats postpartum emaciation, fever with spontaneous sweating, impending chùláo, blood/qi clumping causing irregular menstruation, or chills/fever with emaciation.
Danggui (Angelicae Sinensis Radix), Baishao (Paeoniae Radix Alba), Renshen (Ginseng Radix), Wujapi (Acanthopanacis Cortex - removes wind evil, bad blood, back pain), Digupi (Lycii Cortex) (each 0.5 liang), Mudanpi (Moutan Cortex, 3 qian), Guixin (Cinnamomi Cortex), Moyao (Myrrha) (each 2 qian).
Grind into fine powder. Take 2 qian per dose, decoct in half water and half wine (1 ban zhan each). If non-drinker, use 1 zhan water. Add 1 Kaiyuan Tongbao coin, dip in Sesame Oil, decoct together to 0.7 portion, strain, and take orally. Do not stir while boiling, do not blow while drinking. (Kaiyuan Tongbao coin and sesame oil meanings are unclear/unexplained).
Huangqi Zhu San (Astragalus Decoction Powder): Treats postpartum chùláo with yellow/sallow skin, pale face, aversion to cold with high fever, limb soreness, irritability, and headache.
Biejia (Testudinis Carapax et Plastrum, vinegar-processed), Huangqi (Astragali Radix) (each 1 liang), Niuxi (Achyranthis Bidentatae Radix, 7.5 qian), Danggui (Angelicae Sinensis Radix), Baishao (Paeoniae Radix Alba), Shudihuang (Rehmanniae Radix Preparata), Renshen (Ginseng Radix), Baifuran (Poria), Maimendong (Ophiopogonis Radix, remove heart), Sangjisheng (Taxilli Caulis), Guixin (Cinnamomi Cortex), Gancao (Licorice Root, processed) (each 0.5 liang).
Grind into fine powder. For each dose, take one pig kidney, remove fat membrane, cut open. First boil 1 zhan of water with 0.5 fen ginger and 3 jujubes to 0.7 portion. Remove kidney, ginger, jujubes. Add 5 qian of medicine, boil to 0.4 portion, strain, and take warm on empty stomach and before evening meal. Combine two residues for second decoction. (This formula lacks Taoren and Xuduan compared to Renshen Biejia San, and lacks Congbai, Wumei, Jingjie in decoction method. Know that if there is stasis, break it; if wind, treat wind; if cold, treat cold; if heat, treat heat. There is no fixed rule.)
Zhuyaozi Zhou (Pig Kidney Porridge): Treats postpartum chùláo with fever. (This is "medicine that is not medicine," meaning grain, meat, fruit, and vegetables complete dietary therapy.)
One pig kidney, remove white membrane, cut into willow-leaf slices. Mix with salt and wine. First, cook 1 he of Jingmi (Glutinous Rice) with scallion and pepper to make porridge. Season with salt and vinegar. Place kidney slices at the bottom of the bowl, cover with hot porridge like making soup/congee. Take on an empty stomach.
