The pulse of warm-heat disease (Wenbing) and heat disease (Rebing) may sometimes appear floating-tight (Fu Jin). This is due to a secondary contraction of severe, pathogenic cold (Bao Han). The cold pathogen binds the exterior, while the heat pathogen knots within, resulting in a pulse that is tense and urgent on the outside and surging and vigorous on the inside. If one fails to distinguish the pulse form and merely sees a string-like pulse (Xian Mai), calling it tight and treating it recklessly, this is an error. For when a pulse is full and forceful, it often accompanies a string-like quality; how can it be mistaken for a tight pulse and misdiagnosed as a cold syndrome? The pulse of warm-heat diseases mostly resides at the muscle level and is not very superficial. Moreover, the right-hand pulse is often stronger than the left-hand pulse, which is indeed due to the depression of heat within. If the left-hand pulse is strong or floating, it must be due to a secondary contraction of wind-cold; otherwise, it is not a warm-heat disease but rather non-seasonal sudden cold.
Warm-heat diseases may also present with exterior symptoms first, followed by interior symptoms. This is because the depressed heat expands from the interior to the exterior. When heat is depressed in the interstices (Cou Li), if acrid-cool dispersing methods are not used, the pathogen cannot vent outward and will retreat inward, forming a syndrome suitable for purgation. This differs from Taiyang disease (Shanghan), which begins at the exterior. Or perhaps some do not understand this principle, seeking a floating-tight pulse in warm-heat diseases during spring and summer; is this not too negligent?
