Wang's commentary: This clause serves as a cautionary instruction against the reckless use of diaphoretic (sweating) methods for treating warm diseases. Mengyin's remark is concise and to the point. Generally, if warm diseases are mistakenly treated with diaphoresis, even if it does not result in a fatal outcome, recovery will certainly be difficult. While people still hesitate to use Mahuang Tang (Ephedra Decoction) or Guizhi Tang (Cinnamon Twig Decoction), the most misleading formula is Tao Jie'an's Chai Ge Jie Ji Tang (Bupleurum and Kudzu Decoction to Resolve the Muscles).
The *Yangming Maijie Pian* (Chapter on Explaining the Yangming Pulse) states: When the Foot-Yangming meridian is diseased, the patient dislikes seeing people and fire; hearing the sound of wood causes them to startle in fear, but the sound of bells and drums does not disturb them. Why does hearing the sound of wood cause fear? Qibo answered: The Yangming meridian is the stomach meridian. The stomach belongs to Earth in the Five Elements. Therefore, hearing the sound of wood causes fear because Earth fears Wood (Wood overacts on Earth). The Yellow Emperor asked: Why does he dislike fire? Qibo answered: The Yangming meridian governs the flesh, and its vessels are abundant in blood and qi. When pathogenic factors invade, heat is generated; when heat is severe, the patient dislikes fire. The Yellow Emperor asked: Why does he dislike seeing people? Qibo answered: When Yangming qi rebels upward, it leads to panting and mental restlessness (ao nao); due to this mental restlessness, the patient dislikes seeing people.
Zhang Xugu stated: Earth is constrained by pathogenic factors and further fears the controlling influence of Wood, hence the startle upon hearing the sound of wood. The sound of bells and drums belongs to Metal, and since Earth does not fear Metal, it remains undisturbed. Due to severe heat, the patient dislikes fire; this corresponds to Zhang Zhongjing's statement: "dislikes cold no longer, but instead dislikes heat." When pathogens bind and qi rebels upward, panting and mental restlessness occur. "Wan" refers to mental restlessness (ao nao), hence the dislike for seeing people.
The Yellow Emperor asked: Some patients die from panting, while others survive despite panting. What is the reason? Qibo answered: If the rebellion of qi connects to the Zang organs, death ensues; if it connects to the meridians, survival is possible.
Zhang Xugu stated: When pathogens bind in the Fu organs, qi becomes obstructed leading to panting, and cannot flow through the meridians to reach the four limbs, resulting in cold extremities (jue ni). This is because the four limbs derive their qi from the Spleen and Stomach. If pathogens enter inwardly and connect to the Zang organs, death occurs. If they move outwardly and connect to the meridians, survival occurs.
The Yellow Emperor asked: When the disease is severe, the patient discards clothes and runs about, climbs high places and sings, or goes without food for several days, climbs over walls and roofs. These places are beyond what they could normally do, yet the sick person can do so. What is the reason? Qi
