In febrile diseases characterized by frequent convulsions, twitching (spasms), and mania, treat the blood vessels using the fourth type of needle (Fengzhen/Sharp Needle), urgently draining the excess pathogenic factors. If this progresses to epilepsy (Dianji) resulting in hair loss, treating the blood from the perspective of the Heart yields no cure; instead, one must resort to Kidney Water, as Water corresponds to the Kidney.
This occurs because heat pathogens damage the Blood. Blood is governed by the Heart and stored in the Liver; therefore, frequent convulsions, twitching, and mania are manifestations of the Heart and Liver meridians. By needling the blood vessels, one urgently drains the excess pathogenic factors. If epilepsy develops, total alopecia inevitably ensues, indicating extreme blood heat. If treatment focusing on blood and the Heart fails, one should still assist Kidney Water to nourish the Liver and control Heart Fire.
Febrile diseases presenting with heavy body, bone pain, deafness, and a preference for keeping eyes closed require treatment at the bone level using the fourth type of needle, combined with the Fifty-Nine Acupoints Needling method. In cases of bone disease leading to anorexia, teeth grinding, and bluish ears, treating the bone from the perspective of the Kidney yields no cure; instead, one must treat the Spleen Earth, as Earth corresponds to the Spleen.
This occurs because heat pathogens damage the Kidney. The Kidney governs the bones; when bones are diseased, the body feels heavy and bones ache. The ear is the orifice of the Kidney, hence deafness. Yang pathogens sinking deeply into Yin cause a preference for closing the eyes. Pathogenic excess in the interior leads to an inability to eat. Teeth are the surplus of bones; when pathogens reside in the bones, numbness and itching in the teeth become unbearable, causing teeth grinding. Bluish ears indicate Wind generated by Kidney Heat. If treatment focusing on bone and the Kidney fails, one should treat the Spleen to overcome it.
Febrile diseases presenting with unawareness of pain location, deafness, inability to voluntarily control limbs, dry mouth, intense Yang heat, yet considerable coldness in the Yin sinews, indicate heat residing in the Marrow. This is a fatal condition that cannot be treated.
Pathogenic heat has penetrated deep into the marrow, so the patient does not feel pain in specific areas, but rather experiences flaccid paralysis of the joints unable to maintain posture. With dry mouth, intense Yang heat, and considerable coldness in the Yin sinews, it is precisely because heat is deeply entrenched in the Marrow, blocking Yang Qi from extending. This is a fatal condition that cannot be cured.
Febrile diseases presenting with headache, temporal region pain where the eyes pull and vessels ache, and frequent epistaxis (nosebleeds), constitute Jue-Heat Disease. Treat using the third type of needle (Maohzhen/Hair-Needle), assessing excess or deficiency to regulate balance, addressing conditions involving cold-heat alternation and hemorrhoids.
The temporal region refers to the pulsating acupoint on the foot-Shaoyang meridian anterior to the ear, also known as Naokong (GB13). When heat pathogens lodge in the Shaoyang meridian, headache occurs, and the meridian acupoints along with the eyes pull, causing vessel pain. Blood heat rising with Qi leads to epistaxis, hence named Jue-Heat Disease, because Yang pathogens reverse upward; excessive Yang at the top causes deficiency at the bottom, leading to Jue (reversal/collapse). Assessing excess or deficiency allows regulation to achieve balance. Pathogens in the Shaoyang meridian inevitably cause alternating chills and fever; if they invade the Jueyin meridian, they follow the anal region to form hemorrhoids.
