Qianjin Loulu Decoction (Thousand Gold Decoction of Rhapontic Rhizome): Treats accumulated heat, erysipelas (acute febrile disease characterized by red, swollen skin), unnamed malignant swellings, with a pulse that is surging, solid, wiry, and slippery, accompanied by fever and irritability.
Herbs: Rhapontici Rhizoma (Loulu, 漏芦), Ephedrae Herba (Mahuang, 麻黄), Cimicifugae Rhizoma (Shengma, 升麻), Paeoniae Radix Rubra (Chishao, 赤芍), Scutellariae Radix (Huangqin, 黄芩), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), Ampelopsis Rhizoma (Bailian, 白蔹), Bletillae Rhizoma (Baiji, 白芨), Aurantii Fructus Immaturus (Zhiqiao, 枳壳) (each 4 liang), Rheum Palmatum Radix et Rhizoma (Dahuang, 大黄) (12 liang).
Preparation and Administration: Cut the herbs into small pieces. For each dose, take 1 liang, add fresh ginger, and decoct in water.
Commentary: This formula belongs to the category of Yangming channel formulas. It is an agent that expels accumulated heat retained in both the exterior and interior.
Fangfeng Tongsheng San (Saposhnikovia Powder for Unblocking the Heroic)
See the Stroke section for the formula details.
Commentary: This is a formula that harmonizes the exterior and interior, as well as Qi and Blood. It treats all wind-toxins and accumulated heat causing sores and swellings, where the pulse is wiry, surging, solid, rapid, or floating-tight, indicating abundant Qi and Blood. Such cases cannot lack this formula. A secret transmission version exists in recent times.
Surgeons modify this formula by adding Ginseng Radix et Rhizoma (Renshen, 人参), Red Poria Cocos (Chifuling, 赤茯苓), Astragali Radix (Huangqi, 黄耆), Atractylodis Rhizoma (Cangzhu, 苍术), and Lonicerae Japonicae Flos (Jinyinhua, 金银花), naming it "Xiaozhong Tuoli San" (Swelling-Reducing Interior-Supporting Powder). Although Ginseng is the chief herb, it states "Ginseng may be omitted," which causes doubt and difficulty in usage. Moreover, clinical modifications must compare the principles of treating the exterior and interior: if exterior syndromes predominate, one should follow this formula, using pungent and sweet properties primarily to disperse; if interior syndromes predominate, one may alter the method, thereby distinguishing between pungent-warm, pungent-neutral, and pungent-cool differences.
Jiyao Neituo Fujian San (Essential Interior-Supporting Double-Decoction Powder): Used to support the interior and strengthen the Stomach.
Herbs: Cortex Lycii (Digupi, 地骨皮), Scutellariae Radix (Huangqin, 黄芩), Poria Cocos (Fuling, 茯苓), Paeoniae Radix Alba (Baishao, 白芍), Ginseng Radix et Rhizoma (Renshen, 人参), Astragali Radix (Huangqi, 黄耆), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Cinnamomi Ramulus (Guizhi, 桂), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), Stephaniae Tetrandrae Radix (Fangji, 防己), Angelicae Sinensis Radix (Danggui, 当归) (each 1 liang), Saposhnikoviae Radix (Fangfeng, 防风).
Preparation and Administration: First, decoct 1 jin of Atractylodis Rhizoma (Cangzhu, 苍术) in 5 sheng of water until reduced to 3 sheng. Remove the Cangzhu residue. Add the aforementioned twelve herbs and continue decocting until reduced to three or four zhan. Strain to obtain the clear juice. Divide into three or four doses and drink throughout the day. Additionally, decoct the Cangzhu residue again as a tea. Remove the dregs, and according to the previous method, decoct the residue of the twelve herbs again. Divide and drink.
Wuxiang Tang (Five Fragrances Decoction): Treats toxic vapors entering the abdomen, providing interior support. Modifications can be made internally if other symptoms arise.
Herbs: Caryophylli Flos (Dingxiang, 丁香), Aquilariae Lignum Resinatum (Muxiang, 木香), Aquilariae Lignum (Chenxiang, 沉香), Boswelliae Resina (Ruxiang, 乳香) (each 1 liang), Moschus (Shexiang, 麝香) (3 qian).
Preparation and Administration: Grind into fine powder. Decoct in water and take on an empty stomach. If vomiting occurs, remove Moschus and add Agastaches Herba (Huoxiang, 藿香) 1 liang. If thirst occurs, add Ginseng Radix et Rhizoma (Renshen, 人参) 1 liang.
Commentary: Zhu Danxi said: Someone asked if the ancients practiced the method of supporting the interior. The reply was: Hejian treated external abscesses where the root base was not deep and the manifestations were on the exterior, with a mostly floating pulse, indicating the disease was in the skin and flesh. Unless Qi is abundant, it will inevitably invade the interior, requiring urgent interior support to save the interior. One should use Fujian San to dampness-dispel and depression-scatter, making the Stomach Qi peaceful. If the condition persists, decoct half a dose and drink. If there is constipation and irritability, take a small amount of Huanglian Tang (Coptis Decoction); if there is slight diarrhea and irritability has subsided, then give half a liang of Fujian San. This allows Ying and Wei to circulate together, preventing pathogenic factors from injuring the interior. However, the secular world often uses Shibu San (Ten Tonics Powder), discussed previously.
Shengji Tuoli Tang (Holy Efficiency Interior-Supporting Decoction): Treats various ding sores (hard, painful nodules) and back carbuncles, after sweating or purging have been used, where toxic vapors attack the Heart causing confusion, vomiting, and pain. Can be taken two or three times. Also known as Neituo San (Interior-Supporting Powder).
Herbs: Boswelliae Resina (Ruxiang, 乳香) (clear quality, 1 liang), Mung Bean Powder (Zhenvdoufen, 真绿豆粉) (4 liang; one prescription uses only 2 liang).
Preparation and Administration: Mix the two ingredients evenly. Take 1 qian or 2 qian per dose, mixed with Gancao Decoction (Licorice Decoction).
Commentary: Zhu Danxi said: *Jingyao* states that Neituo San should be taken ten or more times within one to three days to prevent toxic vapors from attacking the Zang-Fu organs, named Huxin San (Heart-Protecting Powder). Carefully examining, Mung Bean resolves dan-toxin and treats stone-toxin; its sweet flavor enters the Yangming channel, its cold nature tonifies, serving as the Sovereign herb. Ruxiang removes malignant swellings, enters the Shaoyin channel, its warm nature facilitates penetration, serving as the Assistant herb. Gancao’s mild nature resolves toxins of metals, stones, and hundred medicines, serving as the Envoy herb. This formula seems specifically designed for those who develop carbuncles after taking mineral pills (Dan-Shi). For those developing carbuncles not due to mineral pills, it may not be an essential formula. Secondly, combine with Wuxiang Lianqiao Tang (Five Fragrances Forsythia Decoction) and Shegan San (Belamcanda Powder) to scatter knotting Qi and eliminate blood stasis, excelling at driving out pathogens, assisted by five fragrant penetrating substances, alternating with Loulu Tang (Rhapontic Rhizome Decoction), with Dahuang (Rhubarb) as assistant. Although there are tonifying effects from Huangqi (Astragalus), Loulu, and Gancao, can they truly avoid the disaster of expulsion? Moreover, Dahuang enters the Yangming and Taiyang channels, its nature moves without staying, purging excess heat, earning the name "General" due to its swift and fierce action. Carbuncles and sores result from accumulated toxins in the Zang-Fu, formed over days and nights. Now manifesting externally, treat the branch severity first. Assist Qi and strengthen the Stomach to solidify the root, promote meridian circulation and blood movement as assistants, consider meridians and seasons to allow toxic vapors to emerge externally. This aligns with Zhang Zhongjing’s intent of using Mahuang (Ephedra) and Guizhi (Cinnamon Twig) to release the exterior. Early application can resolve internally, which is the true meaning of interior support. For the elderly, deeply ill, fully manifested, or weak constitution, although Mung Bean is tonifying, it may bear the burden inadequately. I once treated a woman nearly seventy years old, robust in form, irritable temperament, fond of wine, who developed a brain carbuncle. After five days, her pulse was urgent and choppy. Urgently, I used Dahuang (Rhubarb), wine-steamed, finely cut, wine-fried into powder, combined with wine-fried Ginseng, added fresh ginger, decocted, and administered 1 qian heavy dose. Two hours later, another dose was given. She slept, sweated on the upper body, and upon waking, the illness had vanished. This is also the meaning of interior support. Another case: a man over fifty, robust form, dark complexion, red swelling on the back and pain under the scapula. His pulse was floating, rapid, surging, and tight. He vomited even when eating. In mid-winter, I gave Mahuang Guizhi Tang (Ephedra and Cinnamon Twig Decoction) plus wine-processed Huangbai (Phellodendron), raw Fuzi (Aconite), Gualouzi (Trichosanthes Seed), Gancaojie (Licorice Joint), Qianghuo (Notopterygium), Qingpi (Tangerine Peel), Renshen (Ginseng), Huangqi (Astragalus), Huangqin (Scutellaria), Banxia (Pinellia), and fresh ginger. Six doses resolved it. Is this not the meaning of interior support? There are many such cases, but I dare not list them all.
Guoshi Dingteng Tuoli San (Guo’s Pain-Calming Interior-Supporting Powder): Treats all sore swellings with unbearable pain. For young, strong individuals with abundant Qi, use laxatives first, then this medicine.
Herbs: Papaveris Pericarpium (Suke, 粟壳 - Poppy Shell) (remove stalk, stir-fry, 3 liang), Angelicae Sinensis Radix (Danggui, 当归), Paeoniae Radix Alba (Baishao, 白芍), Ligusticum Chuanxiong Rhizoma (Chuanxiong, 川芎) (each half liang), Boswelliae Resina (Ruxiang, 乳香), Myrrha (Moyao, 没药), Cinnamomi Ramulus (Guizhi, 桂) (each 3 qian).
Preparation and Administration: Cut into pieces. Take 5 qian per dose, decoct in water.
Ruxiang Zhitong San (Frankincense Pain-Stopping Powder): Treats all sore swellings with unceasing pain.
Herbs: Papaveris Pericarpium (Suke, 粟壳) (6 liang, processed), Angelicae Dahuricae Radix (Baizhi, 白芷) (3 liang), Roasted Glycyrrhizae Radix et Rhizoma (Zhi Gancao, 炙甘草), Citri Reticulatae Pericarpium (Chenpi, 陈皮) (each 2 liang), Boswelliae Resina (Ruxiang, 乳香), Myrrha (Moyao, 没药) (each 1 liang), Caryophylli Flos (Dingxiang, 丁香) (half liang).
Preparation and Administration: Cut into pieces. Take 3 qian per dose, decoct in 1.5 zhan of water.
Commentary: Zhu Danxi said: *Jingyao* Volume 13 discusses: At the initial stage, vent heat and extract toxin; once ulcerated, drain pus and stop pain. Was there no pain at the initial stage of a sore? Is the pain worse after ulceration? Every time a sore appears, it first swells; Qi and Blood stagnate, steaming flesh into pus. Thus, the degree of pain reflects the initial stage. After pus drains, swelling recedes, muscles relax, and pain must gradually decrease. If pain persists, it indicates deficiency, suitable for tonification. If touched by foul qi, suitable for harmonization. If forced by wind-cold, suitable for warming dispersion. Examining these aspects allows selection between the above two formulas.
Dongyuan Huangqi Renshen Tang (Dongyuan Astragalus and Ginseng Decoction): Treats various sores after rupture, with poor appetite, insomnia, deficient heat, and exposure to foul qi.
Herbs: Ginseng Radix et Rhizoma (Renshen, 人参), Ophiopogonis Tuber (Maidong, 麦门冬), Citri Reticulatae Pericarpium (Chenpi, 陈皮), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Atractylodis Rhizoma (Cangzhu, 苍术) (each half qian), Astragali Radix (Huangqi, 黄耆) (1 qian), Phellodendri Cortex (Huangbai, 黄柏) (4 fen, fried), Cimicifugae Rhizoma (Shengma, 升麻) (6 fen), Angelicae Sinensis Radix (Dangguishen, 当归身) (half qian), Stir-fried Medicated Leaven (Chao Qu, 炒曲) (3 fen), Roasted Glycyrrhizae Radix et Rhizoma (Zhi Gancao, 炙甘草) (half qian), Schisandrae Fructus (Wuweizi, 五味子) (9 berries).
Preparation and Administration: Cut into pieces. Prepare as one dose, decoct in water.
Commentary: This formula belongs to the Hand and Foot Taiyin channel examples.
Shengji Zhuye Huangqi Tang (Holy Efficiency Bamboo Leaf and Astragalus Decoction): Universally treats ding sores and various sores with fever and thirst.
Herbs: Lophatheri Herba (Danzhuye, 淡竹叶) (2 liang), Rehmanniae Radix Recens (Shengdihuang, 生地黄) (8 liang), Ophiopogonis Tuber (Maidong, 麦门冬), Astragali Radix (Huangqi, 黄耆), Angelicae Sinensis Radix (Danggui, 当归), Ligusticum Chuanxiong Rhizoma (Chuanxiong, 川芎), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), Scutellariae Radix (Huangqin, 黄芩), Paeoniae Radix (Shaoyao, 芍药), Ginseng Radix et Rhizoma (Renshen, 人参), Pinelliae Tuber (Banxia, 半夏), Gypsum Fibrosum (Shigao, 石膏) (each 2 liang).
Preparation and Administration: Cut into pieces. Take 1 liang per dose, decoct in water.
Commentary: This formula involves Shaoyin, Taiyin, and Foot Yangming channel herbs.
Tuoli Huangqi Tang (Interior-Supporting Astragalus Decoction): Treats various sores with thirst and a deficient pulse.
Herbs: Mian Huangqi (Cotton Astragalus Root) (1 liang), Roasted Glycyrrhizae Radix et Rhizoma (Zhi Gancao, 炙甘草) (1 qian).
Preparation and Administration: Cut into pieces, decoct in water.
Dongyuan Shengyu Tang (Dongyuan Holy Recovery Decoction): Treats various sores with excessive bleeding, leading to mental restlessness and insomnia, indicating loss of Blood.
Herbs: Rehmanniae Radix Praeparata (Shudihuang, 熟地黄), Rehmanniae Radix Recens (Shengdihuang, 生地黄), Ligusticum Chuanxiong Rhizoma (Chuanxiong, 川芎), Ginseng Radix et Rhizoma (Renshen, 人参) (each half qian), Angelicae Sinensis Radix (Dangguishen, 当归身), Astragali Radix (Huangqi, 黄耆) (each 1 qian).
Preparation and Administration: Cut into pieces, decoct in water.
