Mr. Xue: Swelling began in the feet, followed by abdominal distension and facial edema. This is caused by cold-dampness pathogenic factors invading from below and gradually attacking upwards. The treatment principle is to unblock Yang, transform dampness, and promote urination.

Mr. Luo: Damp-heat from sores/ulcers and Qi stagnation of the Liver intermingle internally. Recently, Wind-Warmth was contracted externally, leading to alternating chills and fever, cough, and sudden generalized edema. The syndrome belongs to "Chuang Gu" (Distension due to Sores/Ulcers).

Second visit: Dampness combined with Heat generates sores; Wind combines with Dampness to cause swelling. Wind enters from outside, hence chills, fever, and cough; Dampness arises from within, hence abdominal distension and shortness of breath. Use Zhang Zhongjing's Maxing Yigan Tang (Ephedra, Apricot Seed, Coix Seed, and Licorice Decoction) with modifications.

Grind into coarse powder, steep in boiling water, and take the infusion.
Third visit: Swelling of the four limbs and face has subsided, but abdominal distension remains unresolved. The Wind-Cold on the exterior has been resolved, but the Damp-Heat inside has not been treated. Now, plan to regulate the middle jiao and resolve dampness.

Mr. Bai: Fire flares upward, Water overflows to the high source (Lung). Metal of the Lung is affected, causing redness and swelling of the face, red lips and nose, and skin erosion. The abdomen, legs, and feet are also swollen; the Sanjiao (Triple Burner) is all affected. Walking causes cough and wheezing; the pathogen is undoubtedly in the Hand-Taiyin Lung Meridian. Use Wu Hegao's Maidong Tang (Ophiopogon Decoction) to drain fire and discharge water.

Commentary by Wang Yuan: This is a variant syndrome of edema. Using light and clear methods to ventilate and transform the Upper Jiao is what is meant by "lightness can remove excess."
Mr. Fan: Damp-Heat exists below, Wind-Warmth is contracted above. Initially, there were chills and fever, followed immediately by generalized edema, cough, and airway obstruction. This resembles Fengshui (Wind-Water) syndrome. Prescribe Yuespi Jia Zhu Tang (Ma Huang Decoction plus Atractylodes Macrocephala).
Mahuang (Ephedra Herb), Tinglizi (Descurainia Seed), Banxia (Pinellia Tuber), Chiling (Red Poria), Jiaobaizhu (Stir-fried Atractylodes Macrocephala), Sangbaipi (Mulberry Root She), Shegan (Belamcanda Rhizome), Tongcao (Tetrapanax Pith), Xingren (Apricot Seed), Dafupi (Areca Peel), Dongguapi (Winter Melon Peel), Jiangpi (Ginger Skin).
Mr. Zhu: Facial swelling is called "Wind"; swelling of the shins and feet is called "Water." Generally, Wind injures the upper part, Dampness injures the lower part. Air pathways become obstructed, the Lung loses its dispersing and descending function, the Spleen loses its transportation and transformation function. Above, there is cough and wheezing; below, there is scanty or difficult urination; in the middle, there is abdominal distension, resulting in edema. This syndrome is named Fengshui (Wind-Water), recorded in the *Jingui Yaolue* (Essential Prescriptions of the Golden Casket). The disease location is in the Lung and Spleen. The method should be to open the upper, soothe the middle, and渗 (percolate/drain) the lower, discharging pathogens separately through the Sanjiao (Triple Burner).

Mr. Fan: Latent Damp-Heat is stored internally in the Taiyin Spleen and Yangming Stomach. Body heat, abdominal distension, facial edema, foot swelling, soreness and pain in both knees, and scanty urine. Plan to unblock meridians to release the exterior, and dry dampness and clear heat to purify the interior.
Qianghuo (Notopterygium Rhizome), Duhuo (Angelica Pubescens Root), Fangfeng (Saposhnikovia Root), Chuanpu (Magnolia Bark), Chenpi (Aged Tangerine Peel), Dafupi (Areca Peel), Yiren (Coix Seed), Chaihu (Bupleurum Root), Qianhu (Peucedani Radix), Zexie (Alisma Rhizome), Chiling (Red Poria).
Commentary by Wang Yuan: Distension caused by Damp-Heat lies in the Taiyin and Yangming Spleen and Stomach. Modify Baidu San (Decoction for Resisting Toxins) to separate and disperse the latent pathogens. Since there is body heat, it is appropriate to add one or two bitter-cold herbs to drain heat. This is the method of "bitter-pungent unblocking and descending, sweet-bland separating and draining."
Ren Yuan said: The *Neijing* states that distension occurs outside the Zang-Fu organs, pushing them aside and expanding the chest and hypochondrium; this is Qi distension. Its root is in the Kidney, its branch is in the Lung; this is Water distension. All Zang-Fu organs can have distension, speaking generally of Qi and Water. Shijia (Stone Fibroma) and Changtan (Intestinal Polyp) are distensions in women caused by blood stasis and Qi stagnation.
Later scholars distinguished between deficiency/excess and cold/heat, and whether the pathology is in Qi or Blood, making the therapeutic methods quite comprehensive, seemingly leaving no need for further discussion. However, I observe that in consumptive diseases, the pathology lies in Essence; in swelling/distension diseases, the pathology lies in Qi. Regardless of whether it is Qi Gu (Qi Distension), Water Gu (Water Distension), or Blood Gu (Blood Distension), the key lies in the Lung organ. Because the Lung governs the regulation of Qi throughout the body and leads the Qi of the five Zang and six Fu organs. Once Lung Qi is damaged, the regulatory mechanism of the whole body fails. Consequently, the Spleen loses healthy transportation, Liver Wood rebels horizontally, causing Qi Gu; the Kidney loses pivot function, Bladder water passages become unfavorable, causing Water Gu; the Liver loses free flow and drainage, Qi stagnates and Blood coagulates, causing Blood Gu. Are these not all diseases where Lung Qi is impaired, failing to transform Water and Blood, leading to a failure of self-transformation? Although the causes are numerous and the pathologies vary, those acquired from external contraction are often acute, excess, and hot; those acquired from internal injury are often chronic, deficiency, and cold. Edema is mostly excess syndrome, coming suddenly; Qi edema is mostly deficiency syndrome, coming slowly; Damp-Heat edema is between excess and deficiency, coming neither suddenly nor slowly, usually presenting other symptoms before distension appears. If edema causes cough, reverse flow of Qi, and wheezing, it is either severe excess or severe deficiency, which must be differentiated. Excess means Lung Qi is congested and does not descend; deficiency means Kidney Qi rushes upward and cannot grasp. Deficiency syndromes indeed require warming and tonifying; excess syndromes must be drained and descended. For example, in excess-type edema, even using Zhouche Wan (Boat and Cart Pill) or Yugong San (Yu Gong Powder) is not considered too harsh, provided they are not overdosed. The Classics say: "When treating with highly toxic drugs, stop when six-tenths of the disease is gone." Alternatively, use a method of alternating between treating excess and deficiency: administer a dose of harsh medicine, follow with conditioning medicines for two or three days, then administer another dose of harsh medicine. This is the most stable approach. I have verified this on several patients. As for isolated abdominal distension (Dan Zhang Fu), it is due to the deterioration of true Qi in the Spleen, Lung, and Kidney, belonging entirely to deficiency. Blood Gu, Changtan, and Shijia, although the pathology is in the Blood aspect, should not be treated solely by targeting Blood; one should guide Qi to unblock Blood. "Qi is the commander of Blood," a clear instruction from the ancients, which must not be ignored.
