[Symptoms of Dysentery due to Internal Injury by Seven Emotions] At the onset, there is initially difficulty in digesting food, followed by loose stools that are not solid, with frequent watery diarrhea. If left untreated for a long time, pus and blood gradually appear in the stool, resembling dysentery caused by external damp-heat (a disease characterized by abdominal pain, tenesmus, and passage of pus and blood in the stool due to external damp-heat pathogens). It starts with watery diarrhea and progresses to pus and blood; however, this condition develops slowly, differing from the sudden onset of external contraction diseases. This is what medical texts refer to as "Spleen Diarrhea Dysentery" (Pi Xie Li), and what the *Huangdi Neijing* (Yellow Emperor's Inner Canon) describes as spleen pathogenic factors transmitting to the kidney, constituting a "Thief Pathogen" (Ze Xie - an excessive controlling pathogen in Five Element theory) syndrome.
[Etiology of Dysentery due to Internal Injury by Seven Emotions] Worry and overthinking injure the Spleen. Once the Yin of the Spleen is damaged, its transportive and transformative functions fail. The daily intake of water and grain cannot be properly transformed and transported, leading to stagnation and accumulation within the intestines and stomach. When Qi reaches this area, it becomes stagnant; when Blood flows through this area, it becomes sluggish. Qi stagnation and Blood stasis, combined with retained water and grain, become firmly entangled, causing obstruction and stagnation in the Spleen system, thereby triggering the syndrome of thief pathogen transmission to the Kidney.
[Pulse Diagnosis of Dysentery due to Internal Injury by Seven Emotions] The pulse must be deficient upon heavy pressure. A large and deficient pulse indicates Qi injury; a thin and deficient pulse indicates Blood deficiency. A slow and deficient pulse suggests a favorable prognosis (life); a wiry and large pulse indicates a poor prognosis (death). If the wiry pulse retains a "Stomach Qi" (gentle and moderate quality), recovery is possible; if the wiry quality dominates without Stomach Qi, death is inevitable.
[Treatment of Dysentery due to Internal Injury by Seven Emotions] Initially, use Zhachu Gao ( Hawthorn and Atractylodes Paste), which combines tonification and resolution, to assist the Spleen in transforming accumulations. Subsequently, use Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder) to tonify the Spleen and consolidate the root. Note: This condition involves astringency/stagnation and is distinctly different from external dysentery; misusing cold and purgative herbs is erroneous. Furthermore, astringents should not be used too early because this syndrome involves deficiency with underlying stagnation; premature astringency will instead aid the pathogenic factor. If Kidney Yang is insufficient and symptoms of coldness are present, use Shenqi Bawei Wan (Kidney Qi Eight-Ingredient Pill). If Kidney Yin is insufficient and symptoms of deficiency heat and dryness are present, use Liuwei Wan (Six-Ingredient Pill) combined with Baizhu Zhachu Gao to treat dysentery caused by Spleen deficiency, excessive eating, and subsequent accumulation.
