Phlegm nodules, scrofula, and tumors, whether large as a fist or small as a chestnut, can be treated by grinding *Arisaema Cum Bile* (Nanxing, 南星) into powder, mixing it with vinegar to form a cake for topical application. Alternatively, moxa cones may be applied on top of the cake for moxibustion, three to five sessions daily. The goal is to induce warmth and dispersion without causing excessive heat that might damage the skin, rendering further moxibustion inconvenient; this method yields excellent results.
To resolve masses and dissipate nodules, crush garlic together with *Cornus Officinalis* (Shizhuyu, 食茱萸) and apply externally.
For phlegm nodules associated with bi-syndrome at the back of the head, calcined *Pumice* (Fushi, 浮石) ground into powder, mixed with *Calomelas* (Qingfen, 轻粉), and blended with oil for topical application is effective. Alternatively, *Stemona Root* (Baibu, 百部) combined with *Ricinus Seed* (Bimaren, 蓖麻仁) can be crushed and applied.
To eliminate phlegm nodules, apply a paste made from boiling down *Prunella Vulgaris* (Xiakucao, 夏枯草); they typically disappear within a few days.
A divine prescription for treating tuberculosis, scrofula, goiter, and tumors consists of: *Sargassum* (Haidai, 海带), *Sargassum Horneri* (Zaozao, 海藻), *Laminaria* (Kunbu, 昆布), *Sepia Shell* (Haipiaoxiao, 海螵蛸), and *Pumice* (Haishi, 海石), each one tael; dried *Viola Yedoensis* (Zibeitiankui, 紫背天葵), two taels; dried *Prunella Vulgaris* (Xiakucao, 夏枯草), two taels; *Forsythia Fruit* (Lianqiao, 连翘) including seeds, two taels; *Fritillaria Bulb* (Beimu, 贝母), one tael; *Platycodon Root* (Jiegeng, 桔梗), one tael; *Trichosanthes Root* (Tianghuafen, 天花粉), one tael; and *Gleditsia Sinensis Thorn* (Zaociaoci, 皂角刺), five qian. All ingredients are finely powdered and formed into pills the size of wu-tzu seeds using refined honey. Take one hundred pills after each meal, washed down with boiling plain water.
Atrophy syndrome (Wei Bing, 痿病) and lung abscess (Fei Yong, 肺痈) are indeed both diseases of the Lung, but tracing their origins reveals differences. Why? Atrophy arises from constrained fire and qi deficiency leading to lung dryness, whereas lung abscess results from fire forcing blood heat and causing lung tissue ulceration. These two conditions differ as clearly as black and white; how could their treatments not differ? The Lung organ is pure and empty, inherently dry and governing Qi. When Metal Qi is clear and肃 (solemn/clear), during every inhalation and exhalation, it permeates the zang-fu organs, meridians, four limbs, and hundred bones, governing all movements and functions. Currently, if fire is constrained and pathogens obstructed, the Lung body becomes dry and fierce, Lung Qi becomes weak and minute, distribution and transportation cease, and the organ itself weakens, failing to fill the vessels. This leads to flaccidity of tendons and bones, giving rise to atrophy disease. Thus, the Classics state: "When the Lung is injured, atrophy easily occurs." Since the Lung leaves are damaged and scorched, respiration becomes difficult. If respiration is impaired, pathogenic fire cannot be vented, leading to constraint, steaming, and薰 (fuming), resulting in coughing and expectoration, with blood leaking and flowing recklessly. There will inevitably be dull pain in the Yunmen and Zhongfu acupoints, a fishy smell in the throat upon coughing, and a rapid yet deficient pulse. Based on these signs, it is termed Lung Atrophy (Fei Wei, 肺痿). Its symptoms resemble consumptive disease (Lao Zhai, 劳瘵). Treatment should prioritize nourishing Qi, assist with clearing Metal (cooling the Lung), and concurrently attack phlegm, which is optimal. As for lung abscess, the etiology is different. It may begin with wind-cold invading the Lung, failing to dissipate immediately, causing Metal Qi to become obstructed, compounded by alcohol consumption, akin to adding oil to fire. Fire forces blood to aggregate, burning and forming an abscess. Or, due to chronic alcoholism and indulgence, Kidney Water is depleted while Fire blazes, injuring Metal and forcing heat, causing blood to aggregate and congeal into an abscess. Various causes must be distinguished carefully. The presentation must include cough with vexation and fullness, rough skin over the heart and chest (like scales), no fishy taste when tasting raw beans, and a rapid yet forceful pulse. Based on these confirmations, it is termed Lung Abscess (Fei Yong, 肺痈). Initially, there is hemoptysis; prolonged stagnation leads to putrefaction and pus formation. The *Mai Jue* (Pulse Classic) states: "Abscess pulse floats like wine pouring through," referring precisely to this. The treatment method should prioritize draining pus and cooling blood, assisting with protecting the Lung and clearing Metal. Wu Shi stated that lung abscess requires cooling the blood, while lung atrophy requires nourishing the blood; this captures the essence of the matter.
