To treat miasma malaria (Zhang Nue). Phlegm-saliva surges upward and obstructs, causing unconsciousness; the voice sounds like a saw being pulled; the jaw is clenched, eyes are staring straight ahead, and there is incontinence of urine and feces.
Raw Arisaema Rhizome (Sheng Nanxing, 1 liang) Raw Aconite Lateral Root (Sheng Fuzi, 1 liang) Costus Root (Muxiang, 5 qian)
Grind the above into a powder. For each dose, take 4 qian, add 9 slices of fresh ginger, decoct with water, and take warm.
In miasma malaria, if phlegm-saliva blocks the upper orifices and the voice resembles the sound of a saw being pulled, it indicates additional abnormal symptoms, all caused by phlegm. This results from the spleen and stomach contracting cold pathogens, leading to deficiency-cold of the middle qi, which fails to transform fluids, thereby generating phlegm.
Therefore, the drying property of Arisaema Rhizome (Nanxing) is used to eliminate phlegm, the acrid-hot nature of Aconite Lateral Root (Fuzi) is used to warm the middle jiao, assisted by the acrid nature of Costus Root (Muxiang) to promote qi movement; when qi moves, phlegm moves. In my treatment of phlegm syndromes, I mostly use formulas that regulate qi as the main approach. For severe cases, this formula is used alone, or combined with Er Chen Tang (Two-Aged Decoction, a basic formula for drying dampness and resolving phlegm). For those with internal deficiency, Ginseng Radix et Rhizoma (Renshen) is added in double dosage, further assisted by Agarwood (Chenxiang), achieving frequent success.
To treat miasma malaria where symptoms are more severe than described above, and to treat post-miasma phlegm syncope (Tan Jue).
Raw Arisaema Rhizome (Sheng Nanxing, 1 liang) Raw Aconite Lateral Root (Sheng Fuzi, 5 qian) Raw Sichuan Aconite Root (Sheng Chuanwu, 5 qian)
Costus Root (Muxiang, 2.5 qian)
Grind the above into a powder. For each dose, take 5 qian, add 10 slices of fresh ginger, decoct with water, and take warm. For those with deficient primordial qi, add 5 qian of Ginseng Radix et Rhizoma (Renshen) and decoct together. This formula is the previous formula plus Sichuan Aconite Root (Chuanwu), because the deficiency-cold of the spleen and stomach is extremely severe, hence Chuanwu is added. If primordial qi is deficient, adding Ginseng Radix et Rhizoma (Renshen) tonifies primordial qi and facilitates the expulsion of phlegm, possessing the miraculous effect of restoring life from death.
To treat miasma disease causing stiffness of the tongue root and inability to speak.
Scorpion Tail Tip (Xie Shao, 2.5 qian, detoxified) Poria Cocos (Fushen, 1 liang, lightly stir-fried, skin and woody core removed) Mint (Bohe, 1 liang, baked dry)
Grind the above into fine powder. For each dose, take 2 qian, mix with warm wine to swallow. Alternatively, rub between the teeth and cheeks; this is also effective. The cause of stiffness of the tongue root and inability to speak in miasma disease is due to miasma-heat invading the meridians of the Heart and Spleen. Generally, the collateral vessels of the Heart connect to the tongue root, and the meridian of the Spleen connects to the tongue root and spreads under the tongue. This formula uses Poria Cocos (Fushen) to calm the mind and benefit the spleen; Mint (Bohe) can disperse wind-heat; Scorpion Tail Tip (Xie Shao) can dispel wind-phlegm. When wind-heat is eliminated, the Heart and Spleen become clear, and tongue stiffness naturally subsides.
