American Ginseng (suitable for decoction) Raw Gypsum Fibrosum (crushed and wrapped in cloth) Large Ophiopogon Tuber (heart removed, split open) Raw Licorice Root (sliced) Bitter Apricot Seed (skin and tip removed, crushed)
Winter Mulberry Leaf (used raw) Loquat Leaf (brushed to remove hairs, used raw; dew-exposed is better) Honeysuckle Flower Dew (taken separately by mixing) Jin Zhi (pure substance preferred, taken separately by mixing)
The above total nine herbs. Strict dosage in liang is not fixed, as the disease varies in severity, and should be assessed according to the situation at hand. According to the method, decoct with clear water until it boils twice, filter out the dregs, and mix in the flower dew and Jin Zhi. Take warm in two doses. Administer twice daily; if severe, three times daily. If one usually has heavy phlegm-dampness, or after taking several doses feels a slight chill in the stomach, do not be surprised; continue taking the medicine, but moderately add Trichosanthis Fructus (Gualou), Fritillariae Bulbus (Beimu), Aurantii Fructus (Zhike), Citri Reticulatae Pericarpium (Chenpi), Pinelliae Rhizoma (Banxia), and Inulae Flos (Xuanfu).
Ophiopogon Tuber (1 qian 2 fen) Hu Mudan Cortex Moutan Radicis (8 fen) Raw Glycyrrhizae Radix et Rhizoma (5 fen) Fritillariae Bulbus (8 fen) Large Rehmanniae Radix Preparata (2 qian) Raw Paeoniae Radix Alba (8 fen)
Original note: For those with weak constitution, add Prepared Rehmanniae Radix or use both Raw and Prepared together; for extreme heat, add Forsythiae Fructum cor (Lianyao), remove Paeoniae Radix Alba; for extreme dryness, add Asparagi Tuber (Tiantong) and Poria (Fuling). (Commentary: This addition is inappropriate.) If there is internal heat and fever, there is no need to use exterior-releasing drugs; treat according to the previous formula, and the heat will subside on its own.
The symptoms of Baihou (Diphtheria) are as described previously: redness, swelling, and pain in the throat, and in severe cases, swelling of the neck. At the onset, there is often excessive saliva/phlegm. This is caused by Taiyin Dryness-Fire combined with Shaoyang Ministerial Fire. The primary treatment is Baihu Qinglong Tang (White Tiger and Azure Dragon Decoction). Intermittently use Qinghuang San (Green-Yellow Powder), blown into the throat from time to time, to reduce swelling and eliminate phlegm. For mild cases, Zhang's Shengong Pixie San (Divine Efficacy Demon-Expelling Powder) can be used, excluding Ge Gen (Puerariae Radix), Qing Guo (Canarium Fruit), Ma Bo (Amanita Mushroom), and Jiang Can (Bombyx Batryticatus).
This is a disease caused by Taiyin Dryness-Fire combining with Shaoyang Ministerial Fire. Although the appearance of white membranes is due to latent fire in the Lung meridian, it often occurs accompanied by Shaoyang Wind-Heat. The classics state that "the pulse of One Yang" originally refers to the Hand-Shaoyang Sanjiao (Triple Burner). However, Wang Bing's commentary on the section regarding "shortness of breath and frequent coughing" in this chapter states that it generally governs both the Gallbladder and the Sanjiao. Thus, the Sanjiao and the Gallbladder jointly govern Ministerial Fire, so their pathologies often involve each other. Specifically, when Gallbladder Fire moves, it invariably carries Wind. The Gallbladder resides within the Liver; when Fire blazes, Wind inevitably stirs. This case precisely belongs to Wind and Fire fanning each other, moving upward along the meridians, and struggling with external seasonal pathogens, unable to disperse suddenly. It stagnates in the throat causing redness and swelling, and in severe cases, extends to the neck. The upward pressure of Fire-Heat creates a trend of rising without descending, causing phlegm-saliva to surge upwards. These are all Shaoyiao Mark Diseases (biao bing). Therefore, the treatment method must reference bitter-draining techniques, while constantly protecting against Dryness-Fire, ensuring that treating the mark does not hinder the root, which is the appropriate approach. Baihu Qinglong Tang uses Shigao (Raw Gypsum Fibrosum) as the sovereign herb, still emphasizing the root disease, assisted by Banlangen (Isatidis Radix) to unblock and directly descend Shaoyang Fire; Jinyinhua (Lonicerae Japonicae Flos) and Lianqiao (Forsythiae Fructum) are good at resolving constrained heat in the Upper Jiao; Chantui (Cicadae Periostracum) and Sangye (Mori Folium) simultaneously clear wind-heat and guide it downward; Beimu (Fritillariae Bulbus) descends rebellious Qi and eliminates phlegm; Dongdi (Ophiopogon Tuber and Rehmanniae Radix) preserves Lung fluids and moistens dryness; Niubangzi (Arctii Fructus) drains the accumulated heat of Shaoyang; Huangqin (Scutellariae Radix) and Huanglian (Coptidis Rhizoma) reach the Sanjiao and expel upward-pathogenic factors, so these two categories are not contraindicated. This is a comprehensive treatment method for Shaoyang and Taiyin. Since the condition erupts suddenly, methods to reduce swelling, resolve phlegm, and blow powder into the throat cannot be delayed. However, bitter-drying agents must not be added arbitrarily. If the case is mild and wind-heat is shallow, the mark disease is naturally light; even the modified Shengong Pixie San is sufficient to hit the pathology. Although it lacks Shigao to cool the root meridian, the sweet-cold nature of Dongdi already achieves moistening and drying-resolving effects. However, Gegen (Puerariae Radix) is warm and ascending to Yangming, and Qingguo (Canarii Fructus) is sour and astringent to Jueyin; for ordinary warm-heat diseases, these are already contraindicated, let alone for dryness-Qi stagnation. Mabo (Amanita Mushroom) and Jiangcan (Bombyx Batryticatus) both assist dryness, so they are removed entirely.
