血入胞中，胞胀而不能下，其症面垢颊赤，寒热盗汗，上连胸腹胀痛，若因气力衰惫不能送出者，胸腹必不胀而痛，
总当忍痛，正身俟之，不可任意欠伸，致血上冲心也。
血壅而胞衣不下，失笑散（攻七）。牛膝散（攻八）。生化汤（和二十四）。加牛膝煎汤，送益母丸（和二十七）。又，
牛膝一两，黄葵子一合，煎服即下。
气血虚弱，不能送下，保生无忧散。（和二十五）
风冷乘虚而入，胞冷血凝而不下，牛膝散去硝，加桂、附。（攻八）
胎胞既下，气血暴虚，忽而眼黑神昏，不知人事，速投人参、芎、归，以回其气。眼闭口开，面白手冷，六脉微弱，
是其验也，若恶露上行，心下满闷，口噤而神脱者，宜破血下血。二者错治，则立毙矣。
恶露上攻，急用失笑散。（攻七）米醋炖热，和生韭汁，贮有嘴瓶内，将瓶口包裹，以瓶嘴对鼻孔触之，极效。
败血流入肝经，急饮童便，下生化汤。
瘀血奔心，虚火上炎，童便下益母丸。（即济阴丹，和二十七。）或童便送鹿角炭，清魂膏。（寒三十一）
阴血暴亡，心神无所养，而君相二火乘于肺经，则目瞑不省人事，宜四物汤去熟地，加生地、白薇。（补七。）
饮酒血晕，血得酒热而妄行也，川芎、当归、葛根。
使力过多而晕，清魂散。（和二十六）
气血两虚，痰火上涌，二陈汤加朱砂。（和八）
虚风血晕，当归、川芎、桃仁、荆芥。
血液进入胞宫，导致胞宫胀满而胎衣无法排出。其症状表现为面部污垢、两颊发红，伴有寒热往来和盗汗，疼痛向上连及胸腹部。如果是由于气力衰惫而不能将胎衣送出，则胸腹部必然不会感到胀满和疼痛。此时总应当忍受疼痛，端正身体等待其自然脱落，不可随意伸懒腰或用力，以免导致血液上冲心脏。
若因血瘀壅滞而导致胞衣不下，可使用失笑散（攻下类第七方）。牛膝散（攻下类第八方）。生化汤（调和类第二十四方）。加用牛膝煎汤，送服益母丸（调和类第二十七方）。另外，可用牛膝一两，黄葵子一合，煎煮后服用，胎衣即可下行。
若是气血虚弱，无力推动胎衣下行，使用保生无忧散（调和类第二十五方）。
若是风冷之邪乘虚侵入，导致胞宫寒冷、血液凝滞而胎衣不下，使用去除了芒硝的牛膝散，并加入桂枝、附子。（攻下类第八方）
胎胞既已排出，气血突然极度虚弱，忽然出现眼前发黑、神志昏迷、不省人事的症状，应立即投用人参、川芎、当归，以挽回元气。如果见到眼闭口开、面色苍白、手脚冰冷、六脉微弱，这是病情危重的征象。如果是恶露上行，导致心下部胀满闷痛，牙关紧闭且神气涣散，适宜采用破血下血的方法。如果这两种情况治疗错误，患者会立即死亡。
恶露上攻，紧急使用失笑散（攻下类第七方）。米醋炖热，混合新鲜韭菜汁，储存在有嘴的瓶子里，用瓶口包裹住，将瓶嘴对准鼻孔嗅闻，效果极佳。
瘀血奔注于心，虚火上炎，用童便送服益母丸（即济阴丹，调和类第二十七方）。或者用童便送服鹿角炭、清魂膏（清热类第三十一方）。
阴血突然大量亡失，心神失去滋养，而君火与相火二火乘袭肺经，导致眼睛闭合、不省人事，适宜使用四物汤去掉熟地黄，加入生地黄、白薇。（补益类第七方）
Blood enters the uterus (Zigong, 子宫), causing uterine distension and retention of the placenta. The symptoms include a dirty complexion, flushed cheeks, alternating chills and fever, spontaneous sweating, and pain radiating upward to the chest and abdomen. If the inability to expel the placenta is due to exhaustion of Qi and strength, the chest and abdomen will not feel distended or painful. In such cases, one must endure the pain, maintain an upright posture, and wait for natural expulsion; one should not stretch or exert force arbitrarily, lest blood rush upward to the Heart.
If placental retention is caused by blood stasis and obstruction, use Shixiao San (Pain-Dispersing Powder) [Attack Category, Formula 7]. Niuxi San (Achyranthes Root Powder) [Attack Category, Formula 8]. Shenghua Tang (Transforming Stasis Decoction) [Harmonize Category, Formula 24]. Add Niuxi (Radix Achyranthis Bidentatae) to decoct and take with Yi Mu Wan (Motherwort Pill) [Harmonize Category, Formula 27]. Alternatively, decoct Niuxi (Radix Achyranthis Bidentatae) 1 liang and Huangkui Zi (Abutilon Seeds) 1 he, and take orally to facilitate expulsion.
If placental retention is due to deficiency of Qi and Blood lacking the power to push it down, use Baosheng Wuyou San (Preserve Life Without Worries Powder) [Harmonize Category, Formula 25].
If Wind-Cold invades due to deficiency, causing coldness in the uterus and coagulation of blood leading to retention, use Niuxi San (Achyranthes Root Powder) without Mangxiao (Natrii Sulfas), adding Guizhi (Ramulus Cinnamomi) and Fuzi (Radix Aconiti Lateralis Preparata) [Attack Category, Formula 8].
Once the placenta is expelled, Qi and Blood are suddenly and severely depleted. Suddenly, the patient experiences blackouts, coma, and loss of consciousness. Immediately administer Renshen (Radix Ginseng), Chuanxiong (Rhizoma Chuanxiong), and Danggui (Radix Angelicae Sinensis) to restore Qi. Signs of critical condition include closed eyes, open mouth, pale face, cold hands, and faint pulse in all six positions. If lochia flows upward, causing fullness and oppression below the heart, clenched jaw, and dissipation of Shen (Spirit), methods to break blood and purge stasis are indicated. Misdiagnosis or mistreatment of these two conditions leads to immediate death.
For lochia attacking upward, urgently use Shixiao San (Pain-Dispersing Powder) [Attack Category, Formula 7]. Heat rice vinegar, mix with fresh juice of Chinese chives (Jiu Cai, Allium tuberosum), store in a bottle with a spout, wrap the spout, and hold it to the nostrils for inhalation; this is highly effective.
If toxic blood flows into the Liver Meridian, urgently drink Tongbian (Child's Urine), then administer Shenghua Tang (Transforming Stasis Decoction).
If blood stasis rushes to the Heart and deficient fire flares upward, use Tongbian (Child's Urine) to deliver Yi Mu Wan (Motherwort Pill) (also known as Jiyin Dan, Harmonize Category, Formula 27). Or use Tongbian (Child's Urine) to deliver Lu Jiao Tan (Charred Deer Horn) and Qinghun Gao (Soul-Clearing Paste) [Clear Heat Category, Formula 31].
If Yin Blood is suddenly lost, the Heart Spirit loses nourishment, and Ministerial Fire and Sovereign Fire attack the Lung Meridian, resulting in closed eyes and unconsciousness, use Siwu Tang (Four Substances Decoction) removing Shudi (Prepared Rehmannia Root), adding Shengdi (Raw Rehmannia Root) and Baiwei (Cortex Dictamni) [Tonify Category, Formula 7].
For syncope caused by alcohol consumption, where blood runs wild due to the heat of wine, use Chuanxiong (Rhizoma Chuanxiong), Danggui (Radix Angelicae Sinensis), and Gegen (Puerariae Lobatae Radix).
For syncope caused by excessive physical effort, use Qinghun San (Soul-Clearing Powder) [Harmonize Category, Formula 26].
For syncope due to dual deficiency of Qi and Blood with Phlegm-Fire surging upward, use Erchen Tang (Two-Aged Decoction) added with Zhusha (Cinnabar) [Harmonize Category, Formula 8].
For syncope caused by deficient wind, use Danggui (Radix Angelicae Sinensis), Chuanxiong (Rhizoma Chuanxiong), Taoren (Persicae Semen), and Jingjie (Schizonepetae Spica).
