(The tongue protruding) is caused by phlegm-fluids obstructing the heart and chest. Cinnabar powder may be applied externally. Secretly throwing basins or jars to make a sound of falling to the ground can startle the patient, causing the tongue to retract.
It is due to blood deficiency leading to insufficiency of Heart Qi. When Heart Qi is deficient and exposed to wind pathogens, palpitations and fright ensue, termed "Heart Wind." The presentation includes immobility of the eyeballs, with a pulse that is restless yet weak; it is appropriate to greatly tonify Qi and Blood. If treated as a wind disorder, it will rapidly lead to danger.
The patient appears timid, as if someone were about to capture them, which indicates Heart Blood deficiency. If pain from blood stasis masses has not subsided, administer Shenghua Tang (Formula 24). Once the masses dissipate and pain ceases, Ji Jian Yang Rong Tang (Modified Nourishing Younger Yin Decoction, Formula 53) should be used.
Stagnant blood rushing to the heart, Liver Fire flaring upward, phlegm knotting in the chest, and heat entering the Blood Chamber all result in delirious speech and mania.
When the Spirit fails to reside within its dwelling, Bazhen Tang (Eight-Treasure Decoction) is prescribed with added Fushen (Poria with Hostwood), Maidong (Ophiopogon Root), Zaoren (Suanzaoren/Sour Jujube Seed), Shengjiang (Fresh Ginger), and Dazao (Jujube). (Formula 5)
Chills and fever alternating like malaria, being milder during the day and featuring delirium at night, indicate heat entering the Blood Chamber. Hupo Dihuang Wan (Amber and Rehmannia Pill) with added Chaihu (Bupleurum Root) (Attack Formula 14), or Yiling Sansheng San (Yiling Three-Sage Powder) (Attack Formula 15) are indicated.
Hypochondriac pain accompanied by bloody stools results from Liver Blood deficiency causing Fire to flare upward. Ji Wei Xiao Yao San (Modified Free and Easy Wanderer Powder) (Tonify Formula 21) is appropriate.
For chest oppression due to fright and palpitations, use Chen Sha (Calcined Cinnabar) two qian, ground into fine powder, mixed with milk juice. Take one live earthworm with white necks, boil it in hot wine for two or three boils, remove the worm, add the Chen Sha, and simmer again over boiling water. Divide into three doses.
Stagnant blood flowing into the zang-fu organs, filling the skin and muscles, extending to the limbs, where heat knots and stagnates without returning, causes macules to erupt. This condition is fatal in nine out of ten cases.
The Liver stores the Hun (Ethereal Soul), and the Heart stores the Shen (Spirit). Blood deficiency leads to the Heart losing nourishment, and Liver Fire flaring upward prevents the Hun and Shen from residing in their dwellings, resulting in delirious speech and visual hallucinations of ghosts.
Stagnant blood stopping in the Heart causes irritability and confusion. Xiao Tiaojing Tang (Small Regulating Menses Decoction) or Da Tiaojing Tang (Large Regulating Menses Decoction) with added Bohe (Mint) is used. (Harmonize Formulas 38, 39)
For Heart Blood deficiency and damage, Shenghua Tang with added Zaoren (Suanzaoren), Baiziren (Biota Seed), Fushen (Poria with Hostwood), and Maidong (Ophiopogon Root) is prescribed. (Harmonize Formula 24)
Excessive blood loss reduces body fluids, leading to irritability and thirst. It is appropriate to nourish Yin and descend Fire, with a small addition of Tongbian (Child's Urine).
Remaining blood rushing to the Heart is treated with Heye San (Lotus Leaf Powder) (Attack Formula 16).
Yin deficiency causing Fire to move recklessly is treated with Renshen Danggui Tang (Ginseng and Angelica Decoction) (Tonify Formula 54).
Thirst due to Qi deficiency is treated with Shengmai San (Generate Pulse Powder) (Tonify Formula 23), with added Lianzi (Lotus Seed).
Thirst due to Blood deficiency is treated with Siwu Tang (Four Substances Decoction) with added Huafen (Tianhuafen/Trichosanthes Root) and Maidong (Ophiopogon Root). (Tonify Formula 7)
Dizziness, weakness in the legs, and lack of taste for food indicate simultaneous deficiency of Qi and Blood. Ren Shen (Ginseng), Di Huang (Rehmannia Root), Maidong (Ophiopogon Root), Gan Cao (Licorice Root), Huafen (Trichosanthes Root), Nuomi (Glutinous Rice), Shengjiang (Fresh Ginger), and Dazao (Jujube) are decocted and taken orally.
