Light meals and clear tea help preserve health for a hundred years; being prone to worry and illness is often beyond one's control. Strive to engage with wine, as the entangled thoughts and debts of longing remain unresolved. This leads to the hyperactivity and flaming upward of fire in the Three Yang meridians, altering one's appearance and demeanor. The eyes and ears become mutually obstructed, and the head and neck suffer discomfort regardless of size. The pain is like being branded by hot iron, unbearable even for an instant. Life is but an illusory dream, and passionate blood turns into cold rapids. One asks when the incense of returning soul will arrive to scent the clothes and sash.
This condition presents with fever and aversion to cold, swelling and pain in the head and face, red eyes, excessive discharge and tearing, and an inability to sit or lie down facing light. It is caused by wind-phlegm-damp-heat combined with the flying away of Taiyin dryness. It occurs frequently during late summer and high autumn, especially in those who are deficient and obese. If neglected, heat may close off pathogens, causing the divine fluid (shen gao) to become turbid; changes will manifest after the swelling subsides. Jiwei Qianghuo Decoction (Nine-Ingredient Notopterygia Decoction) or Qingkong San should be used. If ineffective, Magui Yin or Da Wenzhong Yin may be employed. If chills precede fever initially, followed by intense heat without chills after one or two days, with a floating, large, rapid, wiry, or dry pulse, headache, body ache, deafness, thirst, and eyes burning like fire, this is an epidemic disease, not an eye disorder or Shaoyang syndrome. Dajian Yin, Puji Xiaodu Yin, and Qingping Wan should be administered. If the condition persists, continued swelling of the head and face resembling a monk's singing bowl or gourd indicates toxic pathogens lodging in the Stomach, blocking the Yuan Fu (primary gates), preventing exterior Qi from entering and interior Qi from exiting, overflowing upwards to cause strange swelling, commonly known as Big Head Plague (Datou Wen). Da Chaihu Tang should be used urgently to purge and support Qi, followed by Shishen Tang and Xiao Xuming Tang. Resolution comes through wild sweating or purpura. However, this condition is highly contagious; those with weak constitution may die. Physicians must exercise caution and not treat lightly.
Sudden onset of Leifeng (Thunderhead Wind) involves high fever and aversion to cold, head feeling as if branded, eyeballs feeling as if drilled, coarse breathing with easy phlegm ascent, constipation due to fire, and difficulty urinating. Negligence over time makes even heavenly physicians unable to save the patient.
Regardless of lateral or central headache, if there is aversion to cold and high fever resembling Shaoyang syndrome, with nodules in the head and eyes, extreme swelling and pain that is unbearable, it is likely accompanied by phlegm. If the ears sound like thunder and wind, roaring loudly, it is called Leifeng. When wind rises, the eyes follow the pathology, leading to a body feeling as if beaten, with constipation of both stool and urine, termed Dalie Touteng (Big Thunderhead Wind). When headache occurs, stool becomes moist then dry, urine clear and profuse then red and difficult, with gradual fever reduction and no pain, it is called Xiaolie Touteng (Small Thunderhead Wind). The severe form acts quickly, the mild form slowly; within two to three days, eye damage occurs, which even divine physicians cannot cure. Eye damage with unceasing pain indicates critical danger. The Nan Jing states that headaches can be Jue (rebellious) or Zhen (true). Jue means rebellion; Zhen means no other mixed pathologies. Facial swelling and heavy head, unpressable, neck pain first, corresponding lower back and spine, surging pulses with heat. This is wind-cold lurking in the Hand Three Yang meridians, lingering and causing obstruction and rebellion. As the head is the summit of Yang, it manifests as Jue pain. If it transmits to Naohu, hands and feet will become cold, nails cyanotic, and death is inevitable. Initially, regardless of severity, if Three Yang rebellious reversal occurs with five evils competing, without distinguishing fire, wind, or phlegm, and if pulse matches symptoms, administer Dachengqi Tang or Sanhuang Reqi Jian immediately. Once fire subsides, phlegm disperses, and wind stops. If exterior symptoms persist, Juhua Tongsheng San may be tried first. If the pulse is floating and hollow or deep, soft, and slow, and previous formulas worsen the condition, urgently switch to Tiaozhong Yiqi, Quanzhen Yiqi, or Dabu Yuanyuan decoctions. Acupuncture and cupping guided by diagrams are particularly effective.
Leifeng is the most dangerous condition in ophthalmology, often leading to blindness. Therefore, predecessors only discussed its danger, never investigating meridian treatment methods, leaving me with private regret to this day.
