Suddenly, the Qingjing (black cornea) suffers pain, feeling as if needles are piercing the skin. This arises from deficiency in the lower jiao and excess in the upper jiao, with internal tension and external looseness of the couli (skin pores). Alternating chills and fever come and go; wind-pathogen and phlegm appear intermittently. One must carefully identify this as a disease of the Yin aspect, ensuring the treatment principle is clear and unambiguous.
The pathogenic trend of this syndrome has declined, yet the black cornea suddenly feels like it is being pricked by chaotic needles. Black corresponds to Water; the pathology corresponds to Fire. This clearly indicates insufficient Kidney Water and excessive Heart Fire. Although the Zang-Fu organs seem balanced, why does Jue (reversal cold/heat) manifestation reappear? It is likely that the patient fails to maintain proper regimen or engages in laborious work that exhausts essence, causing Water to sink and Fire to rise, resulting in the failure of Water and Fire to interact harmoniously. Pathogenic factors clash, similar to the process of pus forming in a sore, meaning the clinical manifestations will inevitably change. The medical texts state: "Disease worsens when seemingly recovered; disaster stems from negligence." This is precisely what is meant. Physicians should investigate the root cause and appropriately administer Yangxin Tang (Nourish Heart Decoction), Quanzhen San (Complete True Powder), or Renshen Bugeng Tang (Ginseng Stomach-Nourishing Decoction), striving to relieve or stop the pain, thereby avoiding the consequence of inaction leading to missed opportunities for cure.
In cases where the eyes were previously healthy but suddenly experience needle-like or burning pain in various locations, it is due to disordered summer climate causing "flowing fire" to bring disaster. One must note which meridian wheel the pathogen originated from and which sector it currently occupies, to predict which meridian it will next invade. Individuals with weak constitutions, visual fatigue, and concurrent Linzhuo (dysuria/cloudy urine) diseases often suffer from this, as Ying Qi (nutritive energy) fails to ascend to moisten the eyes. Furthermore, if eye symptoms precede with redness, tearing, headache, alternating chills and fever, or recurring patterns resembling wind-cold, these mostly belong to deficiency and damage of Ying and Wei (defensive) Qi, with loose couli allowing external pathogens to trigger wind-phlegm. The treatment method involves using Jupi Zhuru Tang (Tangerine Peel and Bamboo Shave Decoction) and Guizhi Shenqi Tang (Cinnamon Twig and Kidney Qi Decoction) to clear Lung Metal and descend Fire; Xiaoyao San (Free and Easy Wanderer Powder) and Wuling San (Five-Ingredient Powder with Poria) to disperse wind and promote diuresis, thus achieving recovery.
Why do chronic itching conditions arise? They stem from Fire, Wind, or Blood deficiency. In some cases, the symptom is itching, but as itching subsides, the disease resolves; in others, severe itching persists without obvious disease signs, yet the illness lingers. If topical application and oral medication fail to provide relief after thorough administration, one may need to seek miraculous cures in desperate situations.
This type of eye itching is extraordinary, feeling like venomous insects crawling on the body, causing trembling and aversion. The causes are manifold: there is itching caused by Wind-pathogen; itching caused by Fire-pathogen; itching occurring as pathogens retreat and heat dissipates, allowing Qi and Blood to flow freely through unobstructed collaterals; and itching in neglected eyes that worsens with each episode of itching. In summary, itching following treatment indicates rapid elimination of the pathogen. Unprovoked itching signals dangerous disease progression. If itching is unbearable, frequent, accompanied by a sensation of sunken eyes, or so intense that rubbing causes globe protrusion, life expectancy is shortened. Excessive tearing indicates Blood deficiency generating internal Heat. One must examine whether有形 (tangible/structural) signs exist within the eye to determine disease progression. If no tangible signs are found, but itching is intolerable, temporarily apply Feixiong Dan (Flying Bear Pill). If ineffective, use Pili Huo (Thunderbolt Fire). If still uncontrolled, carefully evaluate the pulse and prescribe herbs. A wiry and floating pulse with weeping indicates Wind-pathogen, treated with Xiangsu San (Fragrant Su Powder) or Xiongsu Yin (Sichuan Lovage and Perilla Drink). A rapid and slightly hollow pulse suggests Chetiao Shugan San (Tea Regulating Liver Soothing Powder). Caused by alcohol indulgence? Use Gegua Jiexing Tang (Arctostaphylos Flower Decoction for Alcohol Intoxication). If the pulse is deep, slow, soggy, and small, major tonification and warming are required; otherwise, one risks the proverbial "being haunted by ghosts in broad daylight" or "tripping on flat ground," which refers to such negligence. Clinical practice demands utmost caution and attention.
