[Chapter 15. Lei Tou Feng Internal Cataract]
When this eye disease first occurs, the head and face are often affected by cold and heat pathogens, with toxic wind rushing upward. The patient experiences dizziness and vertigo, resembling symptoms of a febrile disease, commonly known as "Lei Tou Feng" (Thunder Head Wind). Accompanied by vomiting or nausea. If the condition persists for many years, the pathogen rushes into the eyes, leading to blindness. It may initially affect one eye, with the pupil size being irregular; subsequently, the other eye is damaged, resulting in dimness and darkness before the eyes, unable to distinguish the Three Lights (Sun, Moon, Stars). Upon initial onset, it is advisable to administer Xie Gan Tang (Liver-Draining Decoction) and Ci Shi Wan (Magnetite Pill), which yield immediate results.
Poem:
Commonly called the hot toxic wind of Thunder Head, after many years it rushes into the eyes.
The pupil becomes slightly large or minute, sitting facing the Three Lights, seeing only blackness without redness.
Brain heat flows with grease to form white opacities; if physicians do not understand, they mistakenly use needling.
Although the cataract droops, following previous methods brings recovery, yet I feel ashamed that mediocre doctors lack diligence.
Xie Gan Tang (Liver-Draining Decoction)
Fangfeng (Saposhnikoviae Radix), Chongweizi (Chuanxiong Herba seeds? Note: Context implies Chuanxiong or similar, but text says Chongweizi - Elsholtziae Fructus/Perilla Fructus variant? Standard WHO: Elsholtziae Fructus), Wuweizi (Schisandrae Chinensis Fructus), Xixin (Asari Radix et Rhizoma), Huangqin (Scutellariae Radix), Dahuang (Rhei Radix et Rhizoma), Mangxiao (Natrii Sulfas), Cheqianzi (Plantaginis Semen), Jiegeng (Platycodonis Radix). (Dosages: Fangfeng and Chongweizi 2 liang each; Wuweizi, Xixin, Huangqin, Dahuang, Mangxiao 1 liang each; Cheqianzi 1.5 liang; Jiegeng 1 liang).
Grind all ingredients into powder. Take 1 qian of powder with 1 cup of water, decoct until reduced to 0.5 cup. Strain and take warm after meals.
Ci Shi Wan (Magnetite Pill)
Cishi (Magnetitum, calcined red and quenched in vinegar three times), Wuweizi (Schisandrae Chinensis Fructus), Mudanpi (Moutan Cortex), Ganjiang (Zingiberis Rhizoma), Heishen (Ginseng Radix nigra? Text says Heishen, likely processed Ginseng), Fuzi (Aconiti Lateralis Radix Praeparata, prepared 0.5 liang). (Dosages: Cishi, Wuweizi, Mudanpi, Ganjiang, Heishen 1 liang each; Fuzi 0.5 liang).
Grind all ingredients into powder, mix with refined honey to make pills the size of tung tree seeds. Take 10 pills with tea before meals.
[Chapter 16. Jing Zhen Internal Cataract]
When this eye disease first occurs, it suddenly arises from deficiency and exhaustion of the Five Zang organs, or due to insufficiency of Liver Qi, allowing toxic heat to rush into the brain. Alternatively, it may result from physical trauma or concussion, causing evil blood from the brain to flow downward and gradually enter the eyes. After two or three years, it transforms into a white opacity, resembling the appearance of an internal cataract. Needling manipulation is contraindicated. For the initially affected eye, and the other eye potentially compromised by traction, wait until the opacity matures, then apply needling according to standard methods for immediate effect. Subsequently, administer Zhen Gan Wan (Liver-Calming Pill) and Huan Jing San (Eye-Recovering Powder) for complete recovery.
Poem:
Suddenly struck by vibration causing incomplete injury, pain is slight but lasts long.
The condition changes so brain grease turns white, like an internal cataract obscuring the Three Lights.
Do not err by using needle manipulation, lest disaster be unbearable.
If later traction does not damage the other eye, the physician should prescribe according to proper methods.
Zhen Gan Wan (Liver-Calming Pill)
Shijueming (Haliotidis Concha, 1 liang separately ground), Xixin (Asari Radix et Rhizoma), Gansanyao (Dioscoreae Rhizoma dried), Chongweizi (Elsholtziae Fructus), Renshen (Ginseng Radix), Cheqianzi (Plantaginis Semen), Baiziren (Biota Semen), Fuling (Poria), Fangfeng (Saposhnikoviae Radix, 1.5 liang). (Dosages: Shijueming 1 liang; Xixin, Gansanyao, Chongweizi, Renshen, Cheqianzi, Baiziren, Fuling 1 liang each; Fangfeng 1.5 liang).
Grind all ingredients into powder, mix with refined honey to make pills the size of tung tree seeds. Take 10 pills with tea after meals.
Huan Jing San (Eye-Recovering Powder)
Renshen (Ginseng Radix), Cheqianzi (Plantaginis Semen), Kugeng (Platycodonis Radix? Text says Kugeng, likely Platycodon or Scrophulariae Radix variant, usually Platycodon in this context), Chongweizi (Elsholtziae Fructus), Xiong (Chuanxiong Rhizoma), Fangfeng (Saposhnikoviae Radix), Xixin (Asari Radix et Rhizoma). (Dosages: Renshen, Cheqianzi, Kugeng 1 liang each; Chongweizi, Xiong 1 liang each; Fangfeng, Xixin 1.5 liang each).
Grind all ingredients into powder. Take 1 qian of powder with 1 cup of water, decoct until reduced to 0.5 cup. Strain and take warm before meals.
