Inside the eye, if both ends (inner and outer canthi) are red with polyps present, they should be hooked up and excised completely using a Pi needle (Biphen, 铍针). If not entirely removed, it may be repeated until fully cleared. Alternatively, use a sickle or thread a fine needle, hold the thread end in the mouth to pull it up, fold it back with a Pi needle to separate it from the Heizhu (Black Pearl, referring to the pupil/cornea area), and cut it under sunlight. After cutting, cauterize with fire to stop its growth potential, preventing recurrence; otherwise, it will flare up again in two or three years. If the grown tissue adheres to the eyeball and does not fall off, it is considered difficult to treat: careful examination is required. Severe cases invade the Shuilun (Water Wheel, referring to the pupil), so use a fine needle comb to lift it carefully, avoiding damage or traction on the Tongren (Pupil). One must be steady and safe, never careless or bold, as the area anterior to the pupil is thin and easily injured. For any hooking or cutting procedures, do not perform them in the morning when the stomach is empty, as this may cause dizziness, fainting, or collapse; someone should support the patient's head. Patients requiring such conditions are generally weak and should be regulated gradually. When using hook needles, do not act hastily; one must exercise judgment and gradually remove the obstruction to avoid dizziness or fainting. If there are red vessels and fleshy growths, acupuncture and sickling are suitable. Observe whether the affected area corresponds appropriately; handle with extreme care and do not apply this method casually, lest it cause invasion of the eyeball, swelling, and pain. Be cautious, be cautious.
The efficacy of treating eyes is unparalleled. For instance, when black flowers (Heihua, referring to floaters or vitreous opacities) appear in the eye, it may seem like no eye disease exists. How to distinguish it? Careful observation is essential. If black angular spots measuring five or six fen appear within the pupil, these are true Heihua. This condition arises from deficiency due to excessive sexual activity. If minute black qi-like formations appear between the pupils, they are also Heihua, caused by irregular eating and mental overexertion. If "hot flowers" occur, only white halos invading the eyeball are visible, which can be distinguished. If nebulae (Yi, corneal opacities or cataracts) form in the eye, those growing from top to bottom are difficult to treat, while those growing from bottom to top are easier. Those growing from head to tail are also treatable. Generally, for eyes with Rouzhang (Fleshy Obstruction, referring to pterygium or similar obstructions), careful and slow examination is needed. If whiteness appears within the pupil, it indicates Rouzhang. In all cases of Wufeng (Five Winds: Qing, Lv, Wu, Huang, Hei winds), the symptoms manifest entirely within the pupil area. This condition requires slow and detailed deduction to restore the patient to health.
