A patient surnamed Mou developed Dian syndrome (epileptic psychosis characterized by withdrawal and depression) due to fright. For over ten years, the patient remained silent, wept sadly, and sat or lay in a state resembling idiocy. With spiritual depletion and emaciated muscles, this is a stubborn disease of lost heart-mind that is difficult to treat; only by greatly tonifying Yuan Qi (Original Qi) can success be achieved. Following the principle of Anxin Wan (Calming the Heart Pill), Ren Shen (Ginseng Root), Huang Jing (Polygonatum Rhizome), Fu Shen (Poria with Hostwood), Dang Gui (Angelica Sinensis Root), Yuan Zhi (Polygala Root), Suan Zao Ren (Ziziphus Seed), Shi Chang Pu (Acorus Rhizome), and Ru Xiang (Frankincense Resin) were each ground into an extremely fine powder. A pig heart was cut open, Cinnabar (Zhusha) was placed inside, and it was tied securely with thread, then wrapped tightly in bamboo leaves. It was boiled in wine until soft and mashed, then mixed with the herbal powders. Cooked jujube flesh was added, and the mixture was pounded into pills the size of paulownia seeds (coated separately with Cinnabar). Each dose consisted of 60-70 pills, taken with Ginseng decoction, stopping when the patient became too weak to take Ginseng. Unfortunately, full recovery was not achieved.
Patient Wang developed Kuang syndrome (psychosis characterized by agitation and aggression) due to emotional stagnation. The patient laughed, cursed, and was prone to anger, with a red face and red eyes, and a洪 Da (Surging and Large) pulse. This indicated a sudden fracture of Yang Qi triggered by anger, leading to uncontrolled Wood (Liver) and Fire (Heart) energies, with Phlegm-Heat ascending to disturb the Pericardium. The disease is named Yang Jue (Yang Reversal). Sheng Tie Luo Yin (Raw Iron Drop Decoction) was used, removing Yuan Hua (Genkwa Flower) and Fang Feng (Saposhnikovia Root), and adding Shan Zhi (Gardenia Fruit), Lian Qiao (Forsythia Fruit), Ling Yang Jiao (Antelope Horn), Zhu Li (Bamboo Sap), Shi Chang Pu (Acorus Rhizome), and Dan Pi (Moutan Cortex). After several doses, the manic behavior subsided.
A patient surnamed Zhang exhibited vague and delirious behavior, viewing her husband as an enemy, holding weapons, stripping naked, and deterring anyone from approaching. The pulse was Slippery and Wiry. Du Sheng San (Supreme Holy Powder) was administered to induce vomiting, expelling a significant amount of sticky phlegm and old foam. The patient beat their chest in pain, and the pulse became slightly calmer, yet they continued to speak and laugh alone. This indicated that while the phlegm had been removed, the Heart residence was deficient, and the Hun and Shen (Soul and Spirit) had not yet recovered. Gua Lou Ren (Trichosanthes Seed), Bei Mu (Fritillaria Bulb), Ju Hong (Tangerine Peel), Dan Nan Xing (Arisaema with Bile), Shi Chang Pu Juice, Yu Jin Juice, Jiang Zhi (Ginger Juice), Zhi Ke (Aurantium Fruit), and Fu Ling (Poria) were prescribed. After one dose, chest pain resolved. Subsequently, Long Chi Qing Hun San (Dragon Tooth Clearing Soul Powder) was imitated. Long Chi (Dragon Tooth/Fossilized Teeth), Fu Shen (Poria with Hostwood), Tie Fen (Iron Powder), Mu Li (Oyster Shell), Ru Xiang (Frankincense Resin), Yuan Zhi (Polygala Root), Suan Zao Ren (Ziziphus Seed), and Dang Gui (Angelica Sinensis Root) were used. After two doses, normalcy returned.
Patient Bao developed manic behavior due to fear, with disturbed spirit and delirious speech. The right pulse was Large, and the left pulse was Soft. The condition stemmed from Heart deficiency receiving shock, where fright-induced Phlegm confused the Ming Shen (Bright Spirit); this was not Xian Ji (Epilepsy). Epilepsy involves temporary collapse and loss of consciousness, followed by immediate clarity upon waking. Here, Dan Nan Xing (Arisaema with Bile) and Chuan Lian (Coptis Rhizome) were used to drain and descend Phlegm-Fire. Over the past month, mental clarity improved slightly. It was appropriate to administer Bai Jin Wan (White Gold Pill) six times, followed by Qing Xin Wen Dan Tang (Clear Heart Warm Gallbladder Decoction) to calm the spirit and stabilize willpower, hoping for gradual peace. Lu Shen (Codonopsis Root), Dan Zhu Ru (Bamboo Shavings), Zhi Ke (Aurantium Fruit), Ju Hong (Tangerine Peel), Fu Shen (Poria with Hostwood), Sheng Suan Zao Ren (Raw Ziziphus Seed), Zhi Xin (Gardenia Stamen), Yuan Zhi (Polygala Root), Mai Dong (Ophiopogon Tuber), Lian Zi Xin (Lotus Plumule), and Xian Shi Chang Pu (Fresh Acorus Rhizome, juice taken separately) were prescribed. After three to four doses, efficacy was observed. The prescription was changed from decoction to pills, leading to full recovery.
Patient Wang spoke and laughed alone, with excessive phlegm and Qi stagnation. Wen Dan Tang (Warm Gallbladder Decoction) was used to descend and cleanse the phlegm disturbing the Heart. After several doses, improvement occurred.
Patient Zhang, frustrated in his ambitions during youth, gradually developed vague spirit and illogical speech, with a red face, slanted eyes, and tendencies to strip and tear curtains. He had previously taken herbal medicine causing vomiting and diarrhea, which slightly settled the Phlegm-Fire. Now, at noon when Fire rises, he speaks and laughs alone, with half-conscious awareness. The left pulse is Wiry and Long, indicating Liver-Gallbladder Fire counterflow directly attacking Dan Zhong (Chest Center), obscuring the Ming Shen with Phlegm-Saliva. The Classics state: "The Liver is the source of planning; the Gallbladder is the source of decision." When Liver-Gallbladder planning is unresolved, grievance cannot be extended, and anger cannot be discharged. Wood and Fire blaze fiercely, the Monarch (Heart) loses authority, leading to Jue Ni (Reversal) and insomnia. Extreme Yang leads to Kuang (Mania). Previously, Niu Huang Qing Xin Wan (Calcareous Calculus Clearing Heart Pill) was taken. Now, the plan is to level Liver-Gallbladder Fire and cleanse Pericardium Phlegm. A temporary decoction is proposed to clear and descend counterflowing fire and sweep away sticky phlegm. Later, a pill formula will be used for slow effect. (Decoction: Long Dan Cao (Gentiana Root), Shan Zhi (Gardenia Fruit), Yu Jin (Curcuma Tuber, ground into juice), Bei Mu (Fritillaria Bulb), Lian Qiao (Forsythia Fruit), Fu Shen (Poria with Hostwood), Tian Zhu Huang (Bambusa Textilis Mite Deposit), Zhi Mu (Anemarrhena Rhizome), Shi Chang Pu (Acorus Rhizome, crushed into juice), Ju Hong (Tangerine Peel), decocted together with gold vessels. After five or six doses, manic states largely subsided. Reflecting on past actions, deep shame was felt, and everything returned to normal. The left and right pulses were even and strong upon deep palpation. Another pill formula was prescribed: Dan Nan Xing (Arisaema with Bile), Chuan Bei (Fritillaria Bulb, 2 qian each), Shan Zhi (Gardenia Fruit, 5 qian), Yu Jin (Curcuma Tuber), Long Chi (Dragon Tooth, 3 qian each), Niu Huang (Calcareous Calculus, 8 fen), Ling Yang Jiao (Antelope Horn, 2 qian), Fu Shen (Poria with Hostwood, 5 qian), Sheng Di (Rehmannia Root, 1 liang). Made into pills with Dan Zhu Li (Bamboo Sap), coated with Zhusha (Cinnabar), taken with warm water. One course prevented recurrence.)
