When the Intestines are deficient, they produce rumbling sounds; this occurs due to the clashing of Cold Qi, or when Fire agitates Water within the intestines. The Classics state: "Deficiency of Middle Qi causes painful rumbling in the intestines." It is appropriate to use Liujunzi Tang (Six-Gentleman Decoction) with roasted Licorice, or Buzhong Yiqi Tang. It is also stated: "Spleen deficiency leads to abdominal fullness, intestinal rumbling, undigested stool diarrhea (Sunxie, 飧泄), and food not transforming." Xiangsha Liujunzi Tang (Aucklandia and Amomum Six-Gentleman Decoction) with Shenqu (Medicated Leaven) may be used. It is further stated: "Rumbling like thunder in the intestines, with Qi rushing upward to the chest, indicates pathogenic factors in the Large Intestine." Banxia Xiexin Tang (Pinellia Heart-Draining Decoction) should be applied. It is also stated: "When Earth Qi is stagnant and erupts, intestinal rumbling occurs accompanied by frequent diarrhea." Pingwei San (Stomach-Leveling Powder) with Poria, Pinellia, and Aucklandia is indicated. It is also stated: "When Heat is excessive, symptoms include intestinal rumbling in the abdomen and Qi rushing upward to the chest." Tingli Muxiang San (Descurainia and Costus Powder) is used. If there is Cold in the Zang organs with Water retention, Lizhong Tang (Regulate the Middle Decoction) with Cinnamon Twig, Poria, and Plantaginis Seed is prescribed. For Spleen-Stomach Cold causing diarrhea, Zhiban Tang (Yi Zhi Ren and Pinellia Decoction) is used. If downward-moving Qi temporarily stops but rumbling recurs, Yizhong Tang (Benefit the Middle Decoction) is employed. When Fire agitates Water (Phlegm-Essence), Erchen Tang (Two-Aged Decoction) with Scutellaria, Coptis, and Gardenia is administered.
