To understand the transmission patterns and syndromes of cold damage diseases in the four seasons, one must superimpose the individual's birth year stem-branch (Gan-Zhi) onto the positions of the left and right intercalary Qi. When Si Di (Earth Ruler) is in Yang, add the Left Intercalary Qi; when in Yin, add the Right Intercalary Qi. Count up to the position of Si Tian (Heaven Ruler) to see which Zang-Fu organ is affected first.
For example, consider a person born in the Geng-Shen year who falls ill on the Ji-Mao day. With Hand-Yangming Large Intestine as Si Tian, three stems-branches prior is Ren-Wu. Hand-Shaoyin Heart is Yin, serving as Zai Quan (Underground Ruler). The Hand represents Yang branches. Hand-Taiyin Lung serves as the Left Intercalary Qi. Thus, place Wu (Horse) upon the Left Intercalary Qi and count to Mao (Rabbit), arriving at Yin (Tiger), which corresponds to the San Jiao (Triple Burner). On the first day, the San Jiao is affected, becoming the primary disease. On the second day, Chou (Ox), Taiyin Spleen Earth: San Jiao Fire generates Earth, constituting Minor Evil (Wei Xie); one should tonify the Heart and drain the Spleen first. On the third day, transmission reaches Zi (Rat): Foot-Shaoyin Kidney is described as Fire in this context (note: traditionally Water, but following text logic regarding Heart meridian resolution), and San Jiao is also Fire; one should resolve the Heart Meridian pathology. On the fourth day, transmission reaches Hai (Pig): Foot-Jueyin Liver Wood, while San Jiao is Fire; Wood generates Fire, constituting Empty Evil (Xu Xie); tonifying the Liver and draining the Heart will lead to recovery. On the fifth day, transmission reaches Xu (Dog): Foot-Taiyang Bladder Water, while San Jiao is Fire; Water overcomes Fire, representing Guest overcoming Host (Ke Sheng Zhu); the patient will surely die. Additionally, the day branch Gui-Wei indicates Gui belongs to Kidney Water.
(There are two methods: one uses the person's natal stem-branch, the other uses the onset day's stem-branch. One method adds left/right intercalary Qi to the day stem-branch; another adds them to the natal stem-branch, both counting to the day stem-branch. Physicians should deduce carefully, as these methods have been verified.)
Consider another case: a person born in the Jia-Zi year falls ill on the Wu-Wu day. Shaoyin (Heart/Kidney) governs Si Tian. Three stems-branches prior is Gui-You. Foot-Yangming Stomach is Zai Quan. You is Yin, intersecting with Shaoyang, serving as the Right Intercalary Qi. Place Jia-Zi on Shen (Monkey) and count to Wu (Horse), arriving at Xu (Dog), corresponding to the Bladder. The first day is the primary disease. On the second day, transmission reaches You (Rooster): Foot-Yangming Stomach, Stomach is Earth, Bladder is Water; Earth overcomes Water, Guest overcoming Host, constituting Robber Evil (Ze Xie); death is certain. If illness occurs on a Tai Yi flow day, ten out of ten cases result in death, with only one chance of life. On the second day, the Stomach is affected on the Ji-Wei day, associated with Earth Transport (Yun) and Earth Qi. Three Earth elements overcome one Water element; the patient will perish before Wei hour, due to excessive Earth suppression.
Consider again: a person born in the Ren-Shen year falls ill on the Ji-Chou day. Hand-Taiyin Lung governs Si Tian. Three stems-branches prior is Taiyang, Hand-Taiyang Small Intestine governs Si Di. Chen is a Yang branch, combined with Hand-Jueyin Pericardium as the Left Intercalary Qi. Count Ren-Shen up to Si Tian, arriving above at Chen, corresponding to Hand-Taiyang Small Intestine Fire. On the first day, the Small Intestine is affected, becoming the primary disease. On the second day, transmission reaches Mao (Rabbit): Hand-Yangming Large Intestine Metal, Small Intestine Fire; Fire overcomes Metal, Host overcoming Guest, constituting Minor Evil; draining the Lung and tonifying the Small Intestine will lead to recovery.
Consider again: a person born in the Bing-Yin year falls ill on the Bing-Xu day. This day features Bing-Xin Water Transport, Chen-Xu Taiyang, representing Water Qi, a Tian Fu (Heavenal Match) day; the illness is difficult to cure. Here, Foot-Taiyang Bladder governs Si Tian. Three stems-branches prior governs Si Di: Xin-Chou Foot-Taiyin Spleen governs Si Di. Chou is a Yin branch, Zi is the Right Intercalary Qi. Place Bing-Yin on the Right Intercalary Qi and count to Si Tian, arriving above at Zi, corresponding to Hand-Shaoyin Heart; this is the first day of infection. "Generating me" is Mother; seeing Son constitutes Empty Evil; "I overcome" constitutes Solid Evil; generating me constitutes Minor Evil; overcoming me constitutes Robber Evil; generating me constitutes Correct Evil (Zheng Xie), self-disease. On the second day, transmission reaches Hai (Pig): Foot-Jueyin Liver Wood, Heart Fire; Wood generates Fire. Mother generating Son, constituting Empty Evil; tonifying Liver and draining Spleen leads to recovery. On the third day, transmission reaches Xu (Dog): Foot-Taiyang Bladder Water, Heart Fire; Water overcomes Fire, Guest overcoming Host, constituting Robber Evil; although distressed, the patient will not die. From onset to the third day (Xu), Zi represents Fire Transport and Fire Qi; Water cannot overcome Fire, as one Water cannot overcome three Fires. (Regarding Zi-Wu, Shaoyin Jun Huo, Taizi, originally belonging to Kidney Water, transmitting to Xu involves exterior-interior relationship, not Robber Evil.) On the fourth day, transmission reaches You (Rooster): Foot-Yangming Stomach Earth, Heart Fire; Fire generates Earth, constituting Empty Evil; tonifying Heart leads to recovery. On the fifth day, transmission reaches Shen (Monkey): Foot-Shaoyang Gallbladder Wood, Heart Fire; Wood generates Fire, Mother seeing Son, constituting Empty Evil; tonifying Liver and draining Spleen. On the sixth day, transmission reaches Wei (Goat): Foot-Taiyin Spleen Earth, Heart Fire; Fire generates Earth, constituting Solid Evil; tonifying Spleen leads to recovery. On the seventh day, transmission reaches Wu (Horse): Foot-Shaoyin Kidney Water, Heart Fire. By the seventh day of illness, reaching Ren-Chen day, Wood Transport becomes Water Qi; Wood Transport generates Fire, Water Qi overcomes Fire, reaching the midpoint; although the patient will not die, they will be severely weakened. On the eighth day, reaching Si (Snake), Hand-Jueyin Pericardium Fire; two Fires meet, encountering harmony (Bi He), or if Day and Branch harmonize, the disease resolves spontaneously.
