In the third month of the previous year, a female servant in Zhu Xiji's household fell ill with fever and was consulted. I prescribed a mild formula to vent the pathogen. The next day, the fever intensified, yet no rash appeared. I continued with venting treatment. By the third day, the condition worsened. The crowd identified it as scarlet fever (Hongre) and urged immediate transfer to an infectious disease hospital for treatment. Zhu Xiji's landlord was particularly terrified and strongly advocated for this course of action. Unable to decide, Zhu Xiji requested that I prescribe medication decisively. I agreed. Upon careful examination, I found that the sha toxin (Sha Du) had erupted but was not smooth; there was swelling and pain in the throat with signs of white putrefaction; wheezing was loud, and breathing was severely difficult; phlegm could not be expectorated; the body was hot with chest tightness; the eyes could not be opened or used for vision; and there was irritability and insomnia. This was actually a critical presentation of Lanhou Sha (a severe acute infectious disease characterized by throat ulceration and skin eruption). I prescribed: Jing Mahuang (Cinnamomum Cassia Twig? No, Ephedrae Herba Purified) 1.5 qian, Sheng Shigao (Gypsum Fibrosum) 5 qian, Guang Xingren (Armeniacae Semen Amarum) 4 qian, Sheng Cao (Glycyrrhizae Radix et Rhizoma) 1 qian, plus some Lugen (Phragmitis Rhizoma), Zhuru (Bambusae Caulis in Taeniam), Chanyi (Bombyx Batryticatus), and Xiuxiu (Paris Polyphyllae Rhizoma) to promote venting, clear heat, and resolve phlegm. After taking the medicine, the patient slept peacefully immediately. The rash erupted fully and brightly, and the sore throat gradually subsided. Subsequent conditioning led to complete recovery within three days. Afterwards, the maid knelt and thanked me, saying that when her illness was at its worst, after taking this medicine, she felt extremely cool and comfortable beyond words.
[Commentary] It is well known that for measles (Mazhen), smooth eruption is auspicious, while internal retention is ominous. However, the method of eruption requires differentiation.
Because the sha toxin is internally retained, it must exit along with sweat. Sweat resides within the sweat glands and must be secreted externally driven by body heat. Therefore, fever prior to the appearance of sha is a normal phenomenon. Yet, this fever has a certain standard; too low is inadmissible, and too high is also undesirable.
In practice, cases of excessive fever are rare, while those of insufficient fever are common. Thus, medication should lean towards warmth; one must never recklessly use cooling agents to suppress it. When the sha toxin is erupting, children's body heat often becomes excessive, inversely proportional to pre-eruption levels. Not realizing that excessive heat hinders the external eruption of the sha toxin.
At this time, heat forcing into the lungs causes wheezing and urgency; heat steaming the sweat glands causes sweating; heat scorching the Heart Mind (Xin Jun) causes unconsciousness; and heat steaming turbid phlegm causes dry cough. These are precise indications for Mahuang Xingren Gancao Shigao Tang (Ephedra, Apricot Kernel, Licorice, and Gypsum Decoction). Administering it in heavy doses yields hundred percent success; how can mild medications achieve this? When a rash disease involves the throat, TCM calls it Lanhou Sha, while Western medicine calls it Scarlet Fever. Mr. Ding Ganren excelled in treating this disease. His general treatment principle stated that for Hou Sha, one should treat the sha as the root and the throat as the branch; once the sha is vented, the throat heals itself. This is concise and accurate. The reason this decoction achieves special effects in treating Hou Sha is precisely due to this principle.
The text of this decoction states: "After sweating (or purging), Guizhi Tang should not be used again. If there is sweating, wheezing, and no significant heat, Mahuang Xingren Gancao Shigao Tang may be given." Some wish to change this to "no sweating, wheezing, and significant heat." They do not realize that Mahuang Tang syndrome progresses from no fever to fever, with the pathological trend moving from stagnation to eruption. Mahuang Xingren Gancao Shigao Tang syndrome progresses from significant body heat to no significant body heat, with the trend moving from exterior to interior. Because it gradually moves from exterior to interior, and from cold to heat, lack of sweating gradually turns into sweating. Only the wheezing remains unresolved. Thus, we understand that "heat-induced wheezing" is indeed the main syndrome of this decoction. Grasping this aspect allows one to infer others. Why tamper with the classic text?
**Mahuang Xingren Gancao Shigao Tang Syndrome Case 4 (Student's Clinical Experience)**
Patient Wang suffered from bilateral tonsillitis (Rumei), with redness, swelling, and pain, hindering swallowing and drinking. There was body heat, slight aversion to wind, bowel and bladder functions were acceptable, pulse was slightly rapid, and the tongue was slightly crimson. The appropriate method is Acrid-Cool and Sweet-Moistening.
Bohe (Menthae Haplocalycis Herba) 1 qian (added later), Xingren (Armeniacae Semen Amarum) 3 qian, Lianqiao (Forsythiae Fructus) 2 qian, Xiangbei (Fritillariae Thunbergii Bulbus) 3 qian, Sangye (Mori Folium) 2 qian, Shengcao (Glycyrrhizae Radix et Rhizoma) 1.5 qian, Chishao (Paeoniae Radix Rubra) 2 qian, Chanyi (Bombyx Batryticatus) 1 qian, Jiangcan (Bombyx Batryticatus) 3 qian, Zhi Jiegeng (Platycodonis Radix) 1 qian, Mabo (Belamcandae Rhizoma) 0.8 fen, Niubangzi (Arctii Fructus) 2 qian, Huo Lugeng (Phragmitis Rhizoma) 1 chi (nodes removed, prepared separately), and Yuyaochi powder blown into the throat.
[Commentary] During the months of September and October, when dryness qi dominates, throat diseases are frequent. Mild cases feel only a lump in the throat hindering swallowing. Slightly more severe cases develop tonsillitis (Rumei) at both sides of the throat, red and swollen like peaches. Western medicine calls this tonsillar swelling. With appropriate modifications to the above formula, improvement is seen in one dose, and cure in two. After the tonsils recede, everything returns to normal. If the tonsils gradually turn from red to white putrefaction or develop white spots, adding Xuanshen (Scrophulariae Radix) is effective. If they further suppurate, puncture therapy to drain pus is required. However, early use of Acrid-Cool and Sweet-Moistening methods would prevent such progression, highlighting the value of this approach.
One school of throat specialists prefers bitter-cold herbs like Banlangen (Isatidis Radix), Chuanlian (Coptidis Rhizoma), Diding (Violaris Herba), and Renzhonghuang (Hominis Excrementum Depuratum). While these may temporarily subdue pathogens, they often cause stagnation and failure to disperse, prolonging the illness or leading to severe complications. In my experience, if Rumei does not resolve with bitter-cold herbs, subsequent use of Acrid-Cool and Sweet-Moistening methods yields slow results, sometimes even no effect in the first few doses. Another school prefers heavy, cloying yin-nourishing herbs like Shengdi (Rehmanniae Radix), Maidong (Ophiopogonis Radix), Shihu (Dendrobii Caulis), and Shashen (Glehniae Radix), sometimes using large doses of 8 qian to 1 liang. This also fails to quickly cure Rumei. My friend Mr. Xie Weiqi from Wu County suffered from minor throat pain. Famous doctors prescribed heavy yin-nourishing formulas with little effect. I ultimately used a light Acrid-Cool formula, which worked in one dose and cured him in two. This demonstrates the value of Acrid-Cool and Sweet-Moistening. This is Zhang Zhongjing's great method, merely elaborated by the Warm Disease school.
Ye Tianshi's *Pediatric Medical Cases* states: "In spring, sudden warmth followed by cold leads to initial warm pathogen invasion constrained by cold, resulting in frequent cough and phlegm-wheezing... Mild cases are cough, severe cases are wheezing; severe wheezing involves nasal flaring and chest protrusion. This is a precise indication for Mahuang Xingren Gancao Shigao Tang. Timely administration of heavy doses prevents the danger of reverse transmission to the Pericardium." Based on my clinical experience, this syndrome occurs mostly in children during winter and spring, opposite to Baihu Jia Guizhi Tang syndrome which affects adults in summer. This aligns with Ye Tianshi, proving he understood Mahuang Xingren Gancao Shigao Tang. Wu Jutong also knew this, though he hid it in the Upper and Middle Jiao chapters of *Wenbing Tiaobian*, revealing it in Chapter 48 of the Lower Jiao chapter: "Wheezing, cough, shortness of breath, spitting thin saliva, surging and rapid pulse, right greater than left, hoarse voice indicates Heat-Fluid Retention, treated with Mahuang Xingren Gancao Shigao Tang." Thus, Warm Disease theorists who recognize this syndrome can build upon it to expand their theories.
It is a pity they do not reveal the root source. I offer a fair opinion: Warm Disease theories are not invalid, and contemporary light formulas are not useless. However, one must understand the lineage connection between them and Shanghan Jing formulas. Mastering Jingfang methods allows the use of Shifang herbs with greater efficacy, as proven by my experience.
**Gegen Qinlian Huangqin Tang Syndrome Case 1 (Student's Clinical Experience)**
Child Li had incomplete rash eruption, foul diarrhea occurring over twenty times daily, crimson tongue with white putrid coating, dry lips, red eyes, rapid pulse, and restless sleep. Gegen Qinlian Tang with modifications is indicated.
Fen Gegen (Puerariae Lobatae Radix) 6 qian, Xi Chuanlian (Coptidis Rhizoma) 1 qian, Huaishanyao (Dioscoreae Rhizoma) 5 qian, Shengcao (Glycyrrhizae Radix et Rhizoma) 3 qian, Dan Huangqin (Scutellariae Radix) 2 qian, Tianhuafen (Trichosanthis Radix) 6 qian, Shengma (Cimicifugae Rhizoma) 1.5 qian.
[Commentary] After taking this, Child Li's diarrhea became looser, and the rash erupted more fully. Gradual conditioning led to recovery. A Hunan senior colleague also frequently encountered children with measles complicated by diarrhea at the Red Cross Hospital, always treating them with this formula with good results. Diarrhea occurring after rash eruption can also be treated similarly.
Measles-related diarrhea is heat-type in 70-80% of cases and cold-type in 20-30%. Thus, Gegen Qinlian Tang is suitable for 70-80%, while Lizhong Tang or Guizhi Renshen Tang for 20-30%. Mistakes here lead to immediate disaster. Modern practitioners often call Gegen Qinlian Tang diarrhea "Xi Re Li" (Collateral Heat Diarrhea). Actually, Zhongjing did not call this Xi Re Li; he called the cold-type diarrhea of Guizhi Renshen Tang "Xi Re Er Li". "Xi Re" means accompanying exterior heat. This is crucial to know.
Taiyang disease requires exterior resolution. If purgatives are used instead, complications arise, either transforming into cold or heat. Strong righteous qi tends toward heat transformation; weak righteous qi tends toward cold. Zhongjing said: "Taiyang disease, exterior symptoms unremoved, frequent purging leads to Xi Re Li, unceasing diarrhea, epigastric fullness/hardness, exterior and interior unresolved, Guizhi Renshen Tang governs." This is cold transformation. He also said: "Taiyang disease, Guizhi pattern, doctor wrongly purges, diarrhea continues, pulsation is cu (surging/rapid), exterior unresolved, wheezing and sweating, Gegen Qinlian Huangqin Tang governs." This is heat transformation.
This clause implies omitted text. To clarify without redundancy, add "If diarrhea persists, use Gegen Tang" after the word "ye". Meaning: Guizhi Tang syndrome damaged fluids by purging; persistent diarrhea further damages fluids. Pulse cu near floating indicates unresolved exterior. Thus, Gegen Tang resolves exterior and nourishes fluids. If exterior resolves but internal heat is intense, lung scorched by heat causing wheezing, and sweat steamed by heat, Gegen Qinlian Tang directly reduces heat.
Previously, I stated Guizhi Tang transforming into heat becomes Baihu Tang, and Mahuang Tang transforming into heat becomes Mahuang Xingren Gancao Shigao Tang. Now I add: Gegen Tang transforming into heat becomes Gegen Qinlian Huangqin Tang. Clinical and textual evidence supports this.
Cao Yingfu said: Unresolved exterior means no sweating. Persistent diarrhea with cu pulse indicates unresolved exterior; Gegen Tang is indicated. Persistent diarrhea with wheezing and sweating indicates interiorization; Gegen Qinlian Tang is indicated. Cu pulse is a variation of Jin (tight) pulse. Gegen Qinlian Tang is suitable only for heat-transformed cases. With white putrid coating, dry lips, and red eyes, it is universally suitable, not just for heat diarrhea.
**Gegen Qinlian Huangqin Tang Syndrome Case 2 (Student's Clinical Experience)**
Baby Sun, living on Tingxi Road, presented with mouth ulcers covering the tongue, cracked lips around the mouth, inability to suckle, crying upon drinking, body heat, scanty urine, surging and rapid pulse, constant irritability, and normal stool. Gegen Qinlian Tang with modifications is planned.
Fen Gegen (Puerariae Lobatae Radix) 4 qian, Dan Huangqin (Scutellariae Radix) 1.5 qian, Xiao Chuanlian (Coptidis Rhizoma) 0.6 qian, Shengcao (Glycyrrhizae Radix et Rhizoma) 3 qian, Dengxin (Junci Medulla) 3 bundles, Huo Lugeng (Phragmitis Rhizoma) 1 chi.
[Commentary] Mr. Sun Weihuan is a friend. His young son, under two years old, was lively and chubby, resembling his father. I usually cured his minor illnesses. Due to busy schedule, I often prescribed remotely based on Mr. Sun's description. One day, he reported fever, cough, thirst, and restlessness. I remotely prescribed a light Acrid-Cool formula. After two days, the condition worsened, developing mouth ulcers. Upon face-to-face consultation, I realized this was originally a Gegen Tang syndrome that transformed into heat, becoming Gegen Qinlian Tang syndrome. Why did it transform? The light Acrid-Cool formula was insufficient for the severity. Would one dose have cured him? No. The next day, the condition remained static.
Second visit: Mouth ulcers persisted. Gegen Qinlian Tang showed little effect. Step further, combine with Chengqi method.
Fen Gegen (Puerariae Lobatae Radix) 4 qian, Xi Chuanlian (Coptidis Rhizoma) 0.8 fen, Sheng Chuanjun (Rhei Radix et Rhizoma) 2 qian, Shengcao (Glycyrrhizae Radix et Rhizoma) 3 qian, Dan Huangqin (Scutellariae Radix) 1.5 qian, Zhishi (Aurantii Fructus Immaturus) 1.5 qian, Xuanmingfen (Natrii Sulfas) 1 qian (mixed in).
[Commentary] The next day, Mr. Sun reported this formula was unsurpassed. Within an hour, he could drink without pain and slept peacefully. Waking up, tongue ulcers receded significantly, and he suckled milk. Reduced dose cured him. Oral Dahuang proved superior to topical Bingpeng San.
Gegen Tang transforming into heat becomes Gegen Qinlian Tang; Gegen Qinlian Tang transforming into heat becomes Chengqi Tang. I delayed treatment initially, so I urgently supplemented later. I used Xiaosha (Mirabilite) and Dahuang before constipation set in, anticipating it. This is urgent supplementation. Had I used heavy Gegen Qinlian Tang earlier, Xiaosha/Dahuang would not have been needed. Had I used heavy Gegen Tang earlier, Qinlian/Huangqin would not have been needed. Tracing back, physicians should not act thus. I once treated a woman with a white spot on the tip of her tongue that eroded inward, painful with food/salt/heat. I diagnosed heat and prescribed Baihu Tang (Shigao). Days passed, no change, appetite worsened. Hearing Chuanlian helped, she took it and cured in one dose. I realized Gegen Qinlian Tang and Baihu Tang are parallel, each with specific indications; swapping them fails.
Cao Yingfu said: Gegen Qinlian Tang is for heat transformation. If ineffective, intestinal dryness/excess is the result. Hence, combining with Chengqi is the next step.
**Gegen Qinlian Huangqin Tang Syndrome Case 3**
Ms. Xu, Meiya Factory, June 12th: Pain in the yin after urination. Originally caused by Large Intestine dryness qi scorching the Bladder, indicating Da Chengqi Tang per *Shanghan Lun*. Improper treatment with certain pills caused daily stool stagnation, frequent urination, persistent yin pain, sore legs/back. Stomach appetite remained unaffected due to Yangming dryness qi dominance. Slight pain above the glabella indicates residual Yangming heat. Right pulse slippery and large still suggests Da Chengqi Tang. However, since the patient is deficient, avoid re-deficiency. Temporarily use Gegen Qinlian Tang plus Lv Dou (Mung Bean) to clear sinking heat and detoxify the pills.
Gegen (Puerariae Lobatae Radix) 1.5 liang, Dan Qin (Scutellariae Radix) 3 qian, Chuanlian (Coptidis Rhizoma) 1 qian, Lv Dou (Phaseoli Radiatus Semen) 1 liang, Shengcao (Glycyrrhizae Radix et Rhizoma) 1 qian.
[Commentary] My master's prescription of Da Chengqi Tang for post-urination yin pain is detailed in *Shanghan Fawei*. Adding Lv Dou to this formula is akin to adding Jingmi (Oryza Sativa) to Gegen Tang, achieving similar goals through different means.
This case required Da Chengqi Tang, but due to deficiency, we stepped back to Gegen Qinlian Tang. In the previous case, due to excess, we stepped forward to combine with Chengqi. Advancing cures quickly; retreating prevents danger. This进退 (advance/retreat) method is military strategy applied to medicine.
Cao Yingfu said: I used this formula for heat diarrhea, not intending to retreat. But isn't it a retreat? Post-urination yin pain requires purging. If not for prior wrongful purging, why avoid Xiaosha/Dahuang? Combining them with Zuojing's formula contradicts this. Treating requires flexibility; sticking rigidly to formulas makes one a wooden idol.
**Da Chengqi Tang Syndrome Case 1**
Patient Fang, illness lasted two weeks. Pain above the glabella, thirst/drinking, no bowel movement for eight days, solid pulse. Although presenting with heart pain radiating to the back, Da Chengqi Tang is primary.
Sheng Chuanjun (Rhei Radix et Rhizoma) 4 qian (added later), Xiao Zhishi (Aurantii Fructus Immaturus) 4 qian, Zhong Chuanpu (Magnoliae Officinalis Cortex) 1 qian, Mangxiao (Natrii Sulfas) 2 qian (added later), Quan Gualou (Trichosanthis Fructus) 5 qian.
*Zhuochao Note*: After purging, chest/diaphragm widened instantly. Residual pathogen remained, head still dizzy. Removed Xiaosha/Dahuang, administered another dose, cured.
[Commentary] The Great Treatise asks: What are Yangming exterior symptoms? Answer: Body heat, spontaneous sweating, aversion to neither cold but aversion to heat. This generally covers Baihu and Chengqi. For full Da Chengqi Tang symptoms: 1. No bowel movement, abdominal pain refusing pressure, due to dry feces in stomach/intestines. 2. Pain above glabella. *Neijing* associates glabella with throat; here it refers to complexion/qi color. If Yangming dryness qi rushes to brain, glabella hurts; otherwise, just distension. 3. Tendon pulling in right thigh, pain outside right knee (unique verification by my master). 4. Surging, large, solid pulse (sometimes slow). 5. Tidal fever at dusk. Other expected signs: yellow, dry, thick coating, extreme thirst for cold drinks. These are norms; variations exist in following cases. Knowing norm and variation allows discussion of the Great Way.
My master excels in Chengqi formulas, curing 90% of Yangming excess cases. His use of Mahuang/Guizhi/Shigao/Dahuang/Chaihu/Qinjin/Jiang/Fuzi follows the syndrome, unlike famous doctors who stick to one bias. I dare say: Any formula Zhongjing designated "governs" works in one dose for improvement, two for cure. With correct dosage, curing in one dose is ordinary. Yangming disease occupies one-quarter of *Shanghan Lun*, with repeated discussion of Chengqi, far exceeding Three Yin formulas. Why doubt Xiaosha/Dahuang? Understanding this honors my master and Zhongjing.
**Da Chengqi Tang Syndrome Case 2**
Ruohua suddenly developed headache, dry vomiting. Took Wuzhuyu Tang, pain worsened. Sleep brought slight relief; sitting caused severe whole-head pain; cough pulled abdominal pain; pressing made it unbearable. No body heat, weak pulse, only afraid of firelight, dry mouth. Does not resemble Yangming Fu-excess. The disease focuses on the brain, not just intestines. This is Zhang Yinan's "Yangming fierce heat qi ascending to brain" syndrome, akin to Western meningitis. At the time of no heat and weak pulse, urgent purging is "drawing firewood from under the pot." If waiting for high heat and strong pulse, it is too late.
Sheng Chuanjun (Rhei Radix et Rhizoma) 3 qian, Mangxiao (Natrii Sulfas) 3 qian, Zhishi (Aurantii Fructus Immaturus) 4 qian, Houpo (Magnoliae Officinalis Cortex) 1 qian.
[Commentary] Ms. Ruohua defecated about three hours after taking this formula. The discharge was not dry feces but turbid water, and the illness vanished completely. I was studying with my master daily then, so I knew well.
Severe whole-head pain indicates brain involvement. Cure after one purge indicates intestinal origin. Combined, this is "treating upper disease by lowering," seeking the root. Since intestines are dry, the stomach above transforms into heat via communication. Stomach nerves connect to the brain, transmitting heat. Brain nerves scorched cause severe headache. Furthermore, intestinal dryness/excess heats blood, affecting all vascular nerves. As the brain is the nerve hub, consequences are severe: not just headache, but confusion, delirium, mania.
