Hongjiao (a disease characterized by swelling and pain in the feet, commonly known as "Jiaqi" or beriberi-like syndrome) initially arises from dampness pathogen invasion, subsequently complicated by wind, cold, summer-heat, and heat pathogens. Therefore, the symptoms first appear on the legs and feet, inevitably presenting with weakness in foot flexion and extension, muscle spasms during movement, pain in limb joints and feet, insensitivity in the lower abdomen, palpitations, and a sensation of stagnation in the chest and urethra. Although there are also symptoms of fever, headache, and body ache, which closely resemble Shanghan (Cold Damage), people often misdiagnose and mistreat it. Zhang Zhongjing considered Hongjiao symptoms similar to Shanghan, thus establishing separate chapters to distinguish them, likely for this reason. Luo Qianfu stated: The etiology of Hongjiao differs between the North and South. In the South, the climate is humid; dampness easily attacks deficient areas and diseases originate from below, meaning Hongjiao is contracted externally. In the North, the terrain is high and dry without dampness. Local customs favor drinking rich dairy products, wines, and noodles, regarding large intake and rapid eating as prowess. Or, excessive nourishment generates dampness internally. Since water nature is moistening and downward-flowing, Qi cannot restrain it, so dampness sinks to the feet causing swelling and pain; this means Jiaqi is contracted internally. One must investigate the cause and treat accordingly.
Hongjiao Ascending Attack (formation of swollen nodules and all kinds of swelling/toxins):
Apply Gan Sui powder (Kansui Root) mixed with water topically; take Gan Cao juice (Licorice Decoction) orally to eliminate it. (From *Bai Yi Xuan Fang*)
Damp Hongjiao (foot pain, calf swelling, must consume Fang Ji Yin):
Bai Zhu (Stir-fried Atractylodis Macrocephalae Rhizoma), Xi Jiao (Rhinoceros Horn - *Note: Substitute with Buffalo Horn in modern practice*), Cang Zhu (Stir-fried Atractylodis Rhizoma), Sheng Di (Wine-fried Rehmanniae Radix), Huang Bo (Wine-fried Phellodendri Cortex), Mu Tong (Akebiae Caulis), Fang Ji (Stephaniae Tetrandrae Radix), Bing Lang (Arecae Semen), Chuan Xiong (Ligustici Chuanxiong Rhizoma) each 8 fen; Gan Cao Shao (Terminal part of Licorice Root) 7 fen.

Take as one dose, decoct in two zhong of water until half remains. Take before meals, two to three times daily. (From *Yi Fang Xuan Yao*)
Foot and Knee Pain (causing inability to walk):
Shan Zha Rou (Hawthorn Flesh) and Bai Ji Li (Tribuli Fructus) in equal parts. Steam twice and sun-dry twice, then grind into powder. Form pills with honey the size of Sophora seeds. Take 3 qian per dose, swallow with warm water. Whether taking one jin or two jins, all will be cured. (Copied by Zhou)
Foot Callus Pain:
When walking long distances urgently, stepping on stones causes sole swelling and inability to walk, unbearable pain. Urgently soak old straw sandals in a urine bucket overnight. Heat a new brick red-hot, place the wet sandal on the brick, then step the swollen foot onto it. Use fire to force the urine vapor into the body; swelling will subside. If not treated early, it may ulcerate and become hard to heal.
Yi Li Jin (Treats wind, cold, summer-heat, dampness causing all wandering pains, regardless of distance, unbearable pain):
Bai Jiao Xiang (Elemi Resin, ground separately), Cao Wu (Aconiti Kusnezoffii Radix, peeled and navel removed), Wu Ling Zhi (Trogopterori Faeces), Di Long (Pheretima, cleaned of soil), Mu Bie Zi (Semen Strychni, pounded to remove oil) each 1 liang 5 qian; Ru Xiang (Olibanum), Mo Yao (Myrrha), Dang Gui (Angelicae Sinensis Radix) each 7 qian 5 fen; She Xiang (Musk) 2 qian 2 fen; Jing Mo (Beijing Ink, burned until smokeless) 1 qian 2 fen.
Mix with glutinous rice flour to form pills the size of Euryale Seed. At onset, take one pill dissolved in wine on an empty stomach. The medicinal effect reaches the foot surface; if redness and swelling persist, take another pill. The effect reaches the sole; black sweat discharges, rooting out the disease. If the disease is in the upper body, take with wine after meals and before bed, natural sweating will occur. For stroke paralysis, numbness, loss of motor control, hemiplegia, take two pills with wine, three times daily. For sudden coma/fainting, grind one pill, mix with wine, and administer; upon waking, feet will feel hot. (From *Wan Bing Hui Chun*)
