The pathogenic toxin returning to the Heart manifests as macules and papules (banzhen), palpitations, itching and pain, high fever, dry throat with thirst, night sweats, and erysipelas-like tumors (danliu); these are all its signs. The toxin returning to the Liver manifests as mental restlessness, blisters, lower back pain, eye diseases, testicular swelling, dry retching, spasm and contracture of the hands and feet, vomiting of roundworms, chills, and teeth grinding; these are all its signs. The toxin returning to the Lung manifests as coughing, panting and urgency, epistaxis, dryness and exhaustion of the skin, and pain in the shoulders and arms; these are all its signs. The toxin returning to the Spleen manifests as vomiting and diarrhea, abdominal pain, swelling, cracked lips and dry mouth, pathological conditions of the hands and feet, and loss of appetite; this is what is meant. The toxin returning to the Kidney manifests as aphonia, cold extremities, dryness and pain in the throat, tinnitus or deafness, hunger without desire to eat, and excessive salivation. The toxin returning to the Large Intestine and Stomach manifests as dysentery with pus and blood, borborygmus with aphonia, and constipation. The toxin returning to the Bladder manifests as fullness in the lower abdomen, hematuria, urinary incontinence, swelling and pain in the forehead, and blurred vision. This discusses the symptoms of the Zang and Fu organs affected by toxins.
The most taboo symptom during exanthema is chills. If during fever there is aversion to cold accompanied by high fever, and the body trembles violently, it indicates that the toxin desires to emerge but cannot, remaining between the cou li (interstices). The pathogenic factor and righteous qi contend with each other, thus causing violent trembling. It is appropriate to administer Chai Ge Gui Zhi Tang (Bupleurum and Pueraria and Cinnamon Twig Decoction).
Foul breath rushing forcefully from the mouth is caused by fire in the Lung and Stomach. This condition often presents with aphonia, panting and vomiting, accompanied by expectoration of pus and blood; these are all its signs. Medication should primarily consist of Qing Jin Tang (Clear Metal Decoction) and Xie Huo Tang (Drain Fire Decoction).
When exanthema covers the whole body and causes itching, leading to restless scratching, this is because the toxic vapor and pathogenic fire have transmitted into the muscle layer and skin, unable to be vented out. This is similar to the itching seen in Shang Han (Cold Damage) disease when sweating does not occur. Furthermore, all sores and ulcers' pain and itching belong to Heart fire. The Classic states: "Itching is a transformation of fire." Additionally, the Liver Wood governs wind; restless scratching is caused by wind. The Liver governs the sinews and operates through the nails. These are both disorders of the Heart and Liver. Medication should primarily consist of Qing Feng Hua Du Tang (Clear Wind and Resolve Toxin Decoction). The herbal ingredients include Jing Jie (Schizonepetae Herba), Fang Feng (Saposhnikoviae Radix), Sheng Ma (Cimicifugae Rhizoma), Qiang Huo (Notopterygii Rhizoma et Radix), Da Li (Arctii Fructus/Ban Xia Lian - *Note: Original text says 'Da Li', likely referring to Niu Bang Zi*), Gan Ge (Puerariae Lobatae Radix), Ma Huang (Ephedrae Herba), and Gui Zhi (Cinnamomi Ramulus).
