Xu Zhongguang treated a patient who, after smallpox, presented with vexatious heat, wheezing and abdominal distension, loose stools, dark urine, and lethargy. This was diagnosed as summerheat damaging the Qi level, and cured using Qingshu Yiqi Tang (Clear Summerheat and Benefit Qi Decoction).
Another case involved a patient with loose stools, poor appetite, and lethargy after smallpox, which was also due to Spleen and Stomach deficiency. This was cured using Liujunzi Tang (Six Gentlemen Decoction) added with Muxiang (Aucklandiae Radix) and Zaoren (Ziziphi Spinosae Semen).
Xu Zhongguang treated another patient with lethargy, vexatious heat, wheezing and fullness, dark urine, and loose stools after smallpox. At that time, it was the long summer season, indicating insufficiency of the Spleen and Stomach; Qing Shu Yi Qi Tang (Clear Summerheat and Benefit Qi Decoction) was used for treatment. Generally, lethargy and desire to sleep, loose stools, and reduced food intake after smallpox are caused by Stomach deficiency failing to generate Qi and Blood, and Spleen deficiency failing to transform nutrients. When transportation and transformation fail, physiological functions lose their duty, and it is appropriate to use Liujunzi Tang (Six Gentlemen Decoction) added with Zaoren (Ziziphi Spinosae Semen) and Muxiang (Aucklandiae Radix).
In one case of post-smallpox lethargy where the patient could not be awakened even when called, day or night, this was due to Qi deficiency and weak Spleen, preventing Clear Yang from ascending. Wuyi Yigong San (Five Agents Extraordinary Efficacy Powder) added with Danggui (Angelicae Sinensis Radix) and Baishao (Paeoniae Radix Alba), followed by Guipi Tang (Restore the Spleen Decoction), resulted in recovery.
In another case, post-smallpox lethargy and lassitude occurred during hot summer weather, accompanied by poor appetite, dry mouth and throat, and withered skin and muscles. This indicated Spleen and Stomach deficiency and weak Yuan Qi (Original Qi), unable to nourish the four limbs, enrich the skin and muscles, and moisten the hair and skin. Moreover, during the long summer, heat damages Yuan Qi; thus, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) combined with Shengmai San (Generate the Pulse Powder) was used for treatment.
Wan Mizhai treated a child who, after smallpox, had persistent fever, poor appetite, a preference for sleeping, and a liking for darkness. This indicated the internal sinking of smallpox toxins. Upon inquiry, it was found that the pus was clear and the scabs thin. Indeed, no pus formed, no scabs appeared, only fluid exuded, and the skin peeled off and dried. Wan said: Initially, high fever and drowsiness upon the eruption of smallpox are common phenomena. Now that the lesions have crusted over, one should expect the elimination of pathogens, the restoration of healthy Qi, subsidence of fever, increased appetite, and mental clarity. Since none of these occurred, the condition was deemed incurable. The patient subsequently died in a state of sudden coma.
Xu Zhongguang stated: In one case of post-smallpox vexatious heat and thirst, where the patient could eat and digest easily but did not gain muscle mass, this was due to deficiency of Qi and Blood with excessive Heart Fire encroaching. The Classics state: "When fire blazes upward, Earth becomes dry; abundant eating still leads to hunger." Treatment should tonify both Qi and Blood. On odd days, Wuyi Yigong San (Five Agents Extraordinary Efficacy Powder) added with Huanglian (Coptidis Rhizoma), Maidong (Ophiopogonis Radix), and Zhimu (Anemarrhenae Rhizoma) was administered; on even days, Siwu Tang (Four Substance Decoction) was given. Some cases failed to respond to treatment and died due to toxic fire scorching Yin fluids and drying up body fluids.
In one case of polyphagia with extreme thirst, lack of muscle growth, and mental exhaustion, with a deep and slow pulse, this indicated Spleen and Stomach insufficiency. Li Dongyuan said: "If stomach fire hides in the Qi level, the patient can eat; if the Spleen is deficient, muscles waste away." He also stated: "Spleen and Stomach deficiency-cold leads to insufficient Yuan Qi (Original Qi); Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Huanglian (Coptidis Rhizoma) should be used."
In one case of easy digestion with frequent hunger but no muscle gain, this was because Stomach Yang was excessive while Spleen Yin was insufficient, causing food to digest rapidly. The *Neijing* states: "The binding of the Two Yangs is called Xiao (Consumption)." The Two Yangs refer to the Stomach and Large Intestine. Xiaoyigong San (Minor Extraordinary Efficacy Powder) added with Huanglian (Coptidis Rhizoma) and Baizhi (Angelicae Dahuricae Radix) was used to drain the heat of the Two Yangs.
In one case of post-smallpox pale face with red lips, emaciated muscles, withered hair and skin, alternating tidal fever, and easy digestion despite eating, this was a sign of Gan Lao (Infantile Malnutrition Consumption), caused by dietary irregularities after smallpox leading to severe damage to the Spleen and Stomach. Li Dongyuan said: "When Spleen and Stomach are weak, Yuan Qi (Original Qi) flows downward, and Yin Fire attacks the Earth position." Treatment should focus on strengthening the Spleen, combined with heat-clearing agents, so that Spleen Earth becomes solid, Qi and Blood are generated, and symptoms resolve. Guishao Siwu Tang (Four Substance Decoction with Angelica and White Peony) added with Shihu (Dendrobii Caulis), Maidong (Ophiopogonis Radix), Digupi (Lycii Cortex), Jinyinhua (Lonicerae Japonicae Flos), and Chaihu (Bupleuri Radix) was used, along with Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) added with Renshen (Ginseng Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Danggui (Angelicae Sinensis Radix), Gouqizi (Lycii Fructus), and Wuweizi (Schisandrae Fructus), interspersed with Yigong San (Extraordinary Efficacy Powder).
In one case of post-smallpox pale face lacking luster, cold breath, poor appetite, and emaciation, this was a sign of Stomach Qi deficiency, treated with Liujunzi Tang (Six Gentlemen Decoction).
In one case similar to the above, accompanied by loose stools and anal leakage (fistula), Liujunzi Tang (Six Gentlemen Decoction) added with Paojiang (Prepared Ginger) and Muxiang (Aucklandiae Radix) was used.
In one case where Mingmen Fire (Gate of Life Fire) failed to generate Earth, resulting in this syndrome, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) was used.
In one case of gradual yellowing and emaciation after smallpox, facial edema, body heat, abdominal distension, and pica (eating mud and rice grains), this was due to Spleen damage from dietary irregularities after smallpox, recurring injury after initial healing, constituting Pi Gan (Spleen Infantile Malnutrition). Treatment should nurture the righteous Qi, allowing accumulation to dissipate naturally; Fei'er Wan (Fatten the Child Pill) was used. (These are all signs of severe damage to Spleen Yin, which cannot be treated with drying and tonifying herbs.)
