Taking herbs to open the stomach and awaken the spleen, the metrorrhagia (uterine bleeding) and dizziness were instantly alleviated. Subsequently taking Dabu Pi Wan (Greatly Tonify the Spleen Pill), after just half a month, diet and daily activities returned to normal. If one sticks rigidly to "blood diseases" and exclusively uses blood-tonifying or hemostatic herbs, what difference is there from carving a mark on a moving boat?
Zhang Feichou treated the wife of Guo Xiaowen. During the summer months, while menstruating, she lay coolly in the wind. Initially, there was dripping menstrual flow; aggravated by anger, falling, and crying, it progressed to unstoppable metrorrhagia. There was a sensation of something hanging down like a thread in the lower abdomen, pulling pain extending to the heart, frequent fever and headache, and general body soreness and irritability. Hemostatic herbs were ineffective. Upon administering Ginseng Radix et Rhizoma (Renshen, 人参) and [missing herb], she suddenly became comatose and unconscious, and the metrorrhagia worsened. Summoned urgently late at night, her pulse was wiry, large, and hollow (Kou Mai, 芤脉), with the left Cun position particularly slippery. It was determined that the Chong and Ren meridians were diseased, clearly caused by Wind pathogen invading the uterus (Bao Men, 胞门). Over time, Wind transformed into Wood-Fire, injuring blood further and intensifying Fire. Without simultaneously administering Xuanfu Hua Tang (Inula Flower Decoction) and Jinlingzi San (Corydalis Fruit Powder), the Wind-Heat could not be cleared, nor the rebellious Qi lowered. Moreover, such cases often involve Heat harassing the Blood Chamber, Dampness knotting in the Zi Hu (uterus), emotional stagnation injuring the Spleen, anger stirring Liver Fire, or fright and falling. Using generic ascending, dispersing, tonifying, or astringent herbs would certainly fail. Thus, treating according to this method resulted in a cure.
Female metrorrhagia and hypomenorrhea are often caused by Warm-Heat damaging the Spleen and Stomach. The Spleen controls the blood; when damaged, it loses its holding capacity. Physicians who do not recognize that this arises from Spleen Dampness transformation and administer astringent/depletion-stopping agents may stop the bleeding, but the Dampness becomes stagnant. Consequently, post-metrorrhagia often leads to distension and jaundice, a condition many physicians fail to identify.
Shen Yaofeng stated: Metrorrhagia cases have more Heat than Cold. If the volume is large and the color bright red, it is Heat, not Cold. If the color is purple-black and clots upon exiting the vessels, there is a type of Yang Deficiency. The Classics state: Yang Qi guards the exterior and secures the interior. Ying (Nutritive) Qi circulates within the vessels, while Wei (Defensive) Qi circulates outside. When exterior Yang is deficient and fails to guard, Ying blood within the vessels leaks out. Once out of the collaterals, the vessels congeal and flow stagnates, gradually turning purple and black. However, only if there is cold aversion in the lower abdomen should warming formulas be administered.
A woman with metrorrhagia took Ginseng Radix et Rhizoma (Renshen, 人参) and Donkey-hide Gelatin (Ejiao, 阿胶) daily, but bleeding continued. She was prescribed Diyu Herba (Yusheng, 地榆) 2 qian, Shengdi Huang (Rehmanniae Radix Recens, 生地) 4 qian, Baishao Radix (Raw White Peony Root, 生白芍) 3 qian, Chuan Lianzi (Szechwan Chinaberry Seed, 川黄连 - *Note: Text says Chuan Huanglian, likely Coptidis Rhizoma*), Huanglian (Coptidis Rhizoma, 黄连) 5 fen, Huangqin Radix (Scutellaria Root, 黄芩) 1.5 qian, Chao Gancao (Fried Licorice Root, 炒甘草) 8 fen, Lianxu (Lotus Stamen, 莲须) 1 qian, Danpi (Moutan Cortex, 丹皮) 1.5 qian, Hei Zhizi (Black Gardenia Fruit, 黑栀子) 1 qian, and Sheng Muli (Raw Oyster Shell, 生牡蛎) 2 qian. Decocted and taken, it was effective immediately. Because she had excessive leukorrhea, Kushen Radix (Sophora Root, 苦参) was occasionally substituted for Huangqin, causing bleeding to resume; using Huangqin stopped it again. Removing Huanglian caused bleeding to resume; adding Huanglian stopped it. Another woman suffered from metrorrhagia for over a month, experiencing dizziness nearly leading to collapse. Adding 1 qian of Renshen to this formula stabilized her, and ten doses brought peace.
A fifty-year-old woman with metrorrhagia did not respond to Renshen and Ejiao. Taking 5 fen of Huanglian for one or two days caused significant discomfort. A physician diagnosed this as a Qi disorder and prescribed Jiuchao Xiangfu (Wine-fried Cyperi Rhizoma, 酒炒香附), Danggui Radix (Angelica Sinensis Root, 当归), Baishao Radix (White Peony Root, 白芍), Danpi, Huangqin, Muli, Zaoren (Ziziphus Seed, 枣仁), and Hei Jingjie (Black Schizonepeta, 黑荆芥) each 2 qian, Yujin (Curcuma Tuber, 郁金) 1.5 qian, JuPi (Tangerine Peel, 橘皮) 1 qian, Shang Chenxiang (Agarwood, 沉香) 3 fen ground and mixed, Chaihu (Bupleurum Root, 柴胡) 5 fen, and Zonglutan (Palmae Fructus Carbonisatum, 棕榈炭) 8 fen. One dose stopped the bleeding. Removing Chaihu, Zonglutan, and Jingjie allowed appetite to return over several doses. Later, Baizhu (Atractylodes Macrocephala Rhizome, 白术) was added as a powder; abdominal distension occurred, which resolved upon reduction.
One case of metrorrhagia with cold aversion in the lower abdomen was fully cured using Guifu Bawei Wan (Cinnamon and Aconite Eight-Ingredient Pill,
The wife of Mao Dakao, an elderly woman, experienced sudden, massive uterine bleeding, termed Daojing (Reverse Menstruation, 倒经). Jiaohong Yin (Gelatin and Safflower Decoction, 胶红饮) was highly effective. The formula consists of Chen Ejiao (Aged Donkey-hide Gelatin, 陈阿胶) stir-fried with rice flour until pearl-like, 1 liang; Quan Danggui (Whole Angelica Sinensis Root, 全当归) 1 liang; Honghua (Carthamus Flower, 红花) 8 qian; Dongguazi (Winter Melon Seed, 冬瓜子) 5 qian. Decocted in Tianquan Spring water, one dose stopped the bleeding. If fever persisted, Lu'an Tea (Lu'an Green Tea, 六安茶叶) 3 qian decocted once reduced the fever. Subsequently, Liujunzi Tang (Six Gentlemen Decoction, 六君子汤) plus Danggui and Baishao regulated her health. Previously, the mother of Wang Yubo, a幕友 (private secretary) to Vice Minister Zhang, over seventy years old, suffered this condition with no response to various medicines. Li Lianfang treated her; this formula cured her instantly. Ye Tianshi said: Initial metrorrhagia should be treated with unblocking methods, prolonged metrorrhagia with astringent methods; this illustrates that principle. I frequently treat elderly women with Daojing with great efficacy. Later, I saw a young woman with unstoppable massive bleeding who failed numerous heavy tonics, resulting in increased flow and near-collapse from syncope. I used this formula, reducing Honghua by half, which worked immediately. Following this regimen, she recovered completely.
