Note: In the section on Cold Damage, Zhanglingshao treated a woman who exhibited delirium and madness. Because her voice was heavy and long, he diagnosed it as an Excess Heat syndrome and cured her with purgation. This case (referring to another case cited by Wei Zhixiu) is also powerfully argued, diagnosing Deficiency Cold and curing it with tonification. However, in Zhang's case, the pulse was hidden and completely absent, indicating Heat Jue (Cold Extremities due to Heat). In this case, the pulse was empty and chaotic, indicating Yang floating outward. Therefore, clinicians must not rigidly adhere to one aspect as the standard.
Zhu Xiangbo's mother suffered from a Heat syndrome with abundant phlegm, rapid panting, irritability, thirst, a sensation of fire/smoke rushing up the throat, cracked and scorched lips, burning soles, and frequent urination. Dong Anyu intended to use Shiquan Dabu Decoction (Complete Great Tonifying Decoction), decocted to swallow Bawei Wan (Eight-Ingredient Pill). Zhu Xiangbo, considering it was midsummer and the condition involved Fire, dared not let her take it, and invited Yang Shangzhi for consultation. Yang palpated the pulse, finding it flooding, rapid, and chaotic without order, but soft and deficient upon deep pressure. The face showed wandering redness, the tongue had prickles, and was contracted and bundled like a lychee shell. Yang said: "This is Kidney Deficiency with Fire failing to return to its source; the pulse follows the symptom manifestation but the pathology is opposite." One should set aside seasonal and symptomatic conventions and treat according to the pulse. The formula used was Bawei Yin combined with Shengmai San (Generate the Pulse Powder), with doubled doses of Renshen (Ginseng), Dihuang (Rehmannia), and Fuzi (Aconite). Seeing that the prescription matched Dong's, Zhu showed his own proposed formula to Yang. Yang said: "The weather is hot, the syndrome is hot; without unique insight, how dare one use such warming tonics? Do not doubt." It was decocted strongly, cooled, and administered, resulting in recovery.
Mrs. Li, over sixty years old, suffered from a Heat syndrome with chest pain and oppression, mental lethargy, coarse breathing, and constipation. Symptoms worsened after using dispersing and digesting methods. The pulse was slippery and rapid, forceful upon deep pressure. The face was congested and bright red, the tongue covered with yellow coating, black and scorched in the center. This was food stagnation in the Middle Jiao, obstruction at the cardia, blocking Taiyin Qi transport and suppressing Yangming Qi extension, leading to郁 (stagnation) transforming into Fire. A large dose of Shugan Yishen Tang (Soothe Liver and Benefit Kidney Decoction) with doubled Shudi (Prepared Rehmannia) was given. That night, dozens of pieces of black stool were passed, and symptoms greatly reduced. On the second day, the pulse was floating and flooding, the scorched yellow coating on the tongue had completely fallen off, but the tongue color turned black as charcoal. Asked: "Why is the tongue black when symptoms have improved?" Answered: "Previously, food stagnation and constipation blocked the upper and lower orifices, trapping Fire which could not rise. Now, pure Yin-nourishing agents allowed bowel movement, draining the obstructed Fire. Residual Fire rose again, turning the tongue black." Ziren (Jujube Seed), Danggui (Angelica Sinensis), and Shanzhi (Gardenia) were added to nourish Water and clear Liver Fire. After the black tongue receded, Shengjin Zishui (Generate Metal and Nourish Water) and Liu Junzi Tang (Six Gentlemen Decoction) plus Danggui and Baishao (White Peony Root) were used for complete recovery.
An elderly woman surnamed Zhu suffered from a Heat syndrome with fullness and oppression, red and light-sensitive eyes, body-wide sores, dry hard stools, painful and scanty urination, and startle response to noise. Doctors administered Jiedu Qinghuo Daochi (Detoxify, Clear Fire, Guide Out Redness) formulas for over ten doses, worsening the Fire, causing loss of appetite and insomnia. Palpation revealed superficial pulsating pulse, deep slow and large pulse, and flooding, soft, and slow pulses at both Guan positions. The external symptoms were identified as False Fire. Shenfu Yangrong Tang (Ginseng and Aconite Nourish Spirit Decoction) was prescribed, but the patient refused. Yang said: "This syndrome originates from worry damaging the Three Yin, exacerbated by cold medicines driving Deficient Fire to wander. Rising upward causes red eyes; flowing downward causes difficult urination and defecation; manifesting externally causes sores; chilling internally causes chest oppression. Thus, the branch appears as Excess Heat, but the root is extreme Deficiency Cold. If it were true Excess Heat, why would she startle at noise and worsen with cold medicines?" After taking the medicine, she slept until the fifth watch, then cried out from hunger. From Yin to Si hours, she drank congee three times. Bowels moistened, urine flowed, and all symptoms subsided. Fuzi was removed from the original formula, and after ten more doses, she fully recovered.
Fei Zichou treated a widow who, in May, suddenly developed high fever, profuse sweating, irritability, thirst, deafness, and hypochondriac pain. Doctors used Chaihu, Gegen, Guizhi, etc., increasing the heat and sweating, causing chest fullness, lethargy, and belching. The right pulse was rapid and large; the left pulse lacked spirit. The tongue coating was slightly black. This was latent Qi emerging from Shaoyang, causing deafness and pain. The method should have been Baihu Tang (White Tiger Decoction) to clear heat, but instead, exterior-releasing agents were used, lifting the pathogen, worsening heat and thirst. Profuse sweating depleted the left pulse spirit; stomach fluid consumption caused lethargy and chest fullness. Belching resulted from chronic depression. With Yuanqi already deficient and latent pathogen unresolved, Liangge San (Cool the Diaphragm Powder) was modified by removing Xiaokun (Mirabilite) and Dahuang (Rhubarb), adding Gualougen (Trichosanthes Root), Danpi (Moutan Cortex), and Zhuye (Bamboo Leaf). One dose reduced heat and induced sleep. However, sweating continued, fatigue prevented turning over, and occasional nausea indicated deficiency. Shengmai San (Generate the Pulse Powder) plus Ziren, Fushen (Poria with Hostwood), and Baishao was used to support Yuanqi and astringe Yin. Congee was taken to support Stomach Qi. When thirst stopped and sweating ceased, the pulse became weak and nearly vanished, indicating restored Zhengqi and extinguished Deficient Fire. Sijunzi Tang (Four Gentlemen Decoction) plus Danggui and Dihuang completed the cure.
Chai Yuqing treated Liang Da, a charioteer for Chen Goushan, suffering from rash, fever, delirium, thirst, and urinary incontinence. Medication worsened the condition. Both pulses were hidden and deep. Suspecting the rash had not yet erupted, Xiaodu Yin Zi (Detoxifying Drink) was planned. The patient did not believe it. When critically ill and seeking treatment again, pulses remained hidden. The tongue was dry, cracked, and prickle-covered, with a dirty face and scorched lips. He then believed it was Fu Shu (Latent Summerheat, i.e., Latent Qi manifesting in Yangming) Excess Heat. Baihu Tang was urgently administered for two doses. The illness resolved, and the pulse became flooding. Thus, when pulses are unreliable, observing the tongue is crucial.
Wang Jiezhai often treated a servant with fever, thirst, dry lips, and delirium. The pulse was fine and slow. Sijunzi Tang was used, adding Huangqi (Astragalus), Danggui, Baishao, and Shufuzi (Prepared Aconite). After one dose, heat intensified, and he walked wildly speaking nonsense. Someone said: "Fuzi was wrong." The pulse remained unchanged. Wang increased the Fuzi dose significantly. After one large dose, sweating occurred, heat receded, and the pulse returned to four beats per breath.
Chen Sansan treated a man with severe fever, eyes appearing to emit fire, dry mouth and tongue, desire to immerse hands and feet in water, and cursing without discrimination. The pulse was wide and large. Huanglian Jiedu Tang (Coptis Detoxification Decoction) worsened the condition. This was a Heart-Spleen disease with dissipating Heart Qi. Chaohei Ganjiang (Stir-fried Black Ginger Rhizome) 1 liang, Renshen 3 qian, Baizhu 1 qian were used. Gancao (Licorice) was omitted to avoid clearing Heart Qi (raw) or slowing the middle (roasted), which would prevent the dispersion of Fire in the Spleen-Stomach. Decocted and taken, within an hour the patient pulled covers over himself, experienced chills and sweating, and recovered. Stir-fried Ginger warms the middle; stir-fried black Ginger cools the Kidney and stops diarrhea.
Zhang Luyu treated a patient named Tong whose latent Qi manifested during midsummer. During examination, he was extremely restless. Pulses were flooding, full, and restless. His wife reported intense thirst, requesting water for two days but being denied, leading to madness. Water was quickly provided. After two cups, he calmed slightly. Soon after, he requested another large cup. Upon putting down the cup, he sweated profusely throughout the body, slept peacefully, and recovered without further medication. Two Western merchants with similar symptoms also recovered with water.
Xue Lizhai treated a man with summer heat fever, chest and back pain, sweating after drinking warm fluids. Exterior-releasing medicines caused mental confusion, delirium, loose stools, and excessive phlegm. One dose of Shiquan Dabu Decoction instantly reduced the fever. Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) plus Paojiang (Roasted Ginger) for two doses completed the cure.
Wang Kentang treated Yu Yunqu, a Grand Secretary. He was robust and ate well. In June of the Xinmao year, he suffered from a Heat syndrome. Limbs were not very hot, but he occasionally threw his hands and feet about, appearing restless, confused, unaware of people, speaking indistinctly but not deliriously. The pulse was faint and fine, nearly vanishing. Some suggested Yin syndrome requiring warming; others suggested purgation. His mentor Lu Kuiri and other scholars deferred to Wang. Wang said: "Yang syndrome showing Yin pulse indicates untreatable condition. However, given his constitution, extreme summer heat, and overindulgence in wine and meat, he should have had burning fever and seven-day constipation. Let us try Daxiehu Tang (Major Bupleurum Decoction) to purge. Use 2 qian of Shudahuang (Prepared Rhubarb)." Imperial Physician Wang Leian argued this was too little, suggesting Dachengqi Tang (Major Order the Qi Decoction). Wang said: "With such pulse and symptoms, harsh purgation is inappropriate. Wait if Daxiehu fails, then use Tiaowei Chengqi Tang. If that fails, then Xiao Chengqi Tang or Dachengqi Tang." After medication, bowels moved, pulse emerged, and limbs warmed. Wang told Leian: "Had we used Dachengqi, would we not regret it later?" Several doses of Huanglian Jiedu Tang brought stability. In July, he accompanied Mr. Lu to Nanjing for examinations, with no recurrence. The next year, Wang retired and read Liu Hejian's *Shanghan Zhige Lun* (Direct Grid of Cold Damage), which stated: "Accumulated heat internally causes rapid pulse... due to extreme heat obstructing pulse vessels, causing faint, fine, nearly vanishing pulse. Laypeople mistake this for extreme Cold/Yin toxicity. Or initial Yang heat is violent... or simultaneous contraction of Heat... generally detoxify and purge with Dachengqi. If heat reduces but doesn't resolve, use Huanglian Jiedu. If slight heat remains, use Liangge San. If missed purgation leads to extreme heat, cold body, faint pulse, and coma, urgent purgation will cause sudden death of residual Yin due to exhausted Yang. Not purging also leads to death; use Liangge San or Huanglian Jiedu to nourish Yin, retreat Yang, and gradually disperse accumulated heat, warming the chest and restoring the pulse." Wang sighed: "The ancients anticipated my thoughts. My patient suffered exactly from missed purgation leading to extreme heat, cold body, faint pulse, and coma. To purge or not, heavily or lightly, life and death hang by a hair. Alas! How cautious one must be."
Ma Yuanyi treated Grand Secretary Feng. Bored in lodging, he enjoyed concubines. During internal deficiency and summer heat, he developed day-and-night fever, irritability, thirst, and restlessness. Pulse was fine, rapid, urgent, with wiry Chi position, and unclear spirit. This was excessive sexual activity depleting True Yin, allowing Yang to invade. With irritability, fever, and coma, Fire pathogen disturbed internally, and external invasion was extreme. In midsummer, with Fire blazing and Earth dry, how much Yin remained to withstand burning? Without urgent Yin rescue, disaster was imminent. Shouwu (Polygonum multiflorum) 2 liang as sovereign to save Liver-Kidney Yin, assisted by Huanglian, Zhimu, Chaihu, Huangqin, Zhike, Banxia Qu, Juhong, Xingren to transform phlegm. One dose cleared spirit; two doses resolved bowels, reducing heat by half. Then Renshen, Zhi Shouwu, Biejia, Danpi, Baishao, Gancao to harmonize Yin-Yang, reducing heat. Finally, Renshen Xiaoyao San stabilized him.
Zhang Yitian treated a man surnamed Jiao from Yongjiang. In July, he had high fever, red tongue, lower abdominal fullness, free urination, red eyes, and madness for over thirty days. Initial dispersing and subsequent purging gave slight sweat but failed to cure. Pulse was deep and faint, rapid and urgent upon deep pressure. Surface symptoms remained, but pulse was deep/faint, indicating pathogen sinking into Yin. Rapid/urgent deep pulse indicated Yin unable to control Yang, causing rapid pulse flow and madness. This was collateral blood stasis binding in Shaoyin. Didang Tang (Definite Blood-Purgation Decoction) was indicated. He prepared Machong (Blister Beetle), Shuizhi (Leech), Taoren (Peach Kernel), and Dahuang, decocted and taken. Massive bleeding followed. Shudi (Prepared Rehmannia) was then decocted alone and drunk frequently to save Yin fluids. Once unblocked, Renshen, Fuzi, and Zhigancao were slowly administered to secure True Yuan. Total intake: over 2 jin Shudi, 0.5 jin Renshen, 4 liang Fuzi. Gradual recovery ensued.
Shi Lizet treated Tang Houpo, a successful candidate. Symptoms included alternating chills/fever, red face, severe head and tooth pain. Pulse was flooding and rapid, indicating Heat syndrome. Baihu Tang was indicated, with 1 liang Shigao (Gypsum) per dose. One dose cured head/tooth pain and chills/fever. However, pulse still beat forcefully against fingers. Shi said: "Must continue with another dose, otherwise macules will erupt in two days." The patient doubted and consulted specialists, who asked: "How bold! Can Gypsum bear heavy doses?" He refused. Half a month later, he sought help again, reporting macules had erupted, lasting ten days, with lingering burning heat. Shi said: "Did preventing the fire work?" He administered Chai Ling Shaoyao Tang (Bupleurum, Pinellia, and Peony Decoction). One dose reduced heat. Later, Renshen and Baizhu regulated him to recovery.
Friend Zhang Shenzhi suffered from Heart Meridian Heat, dry mouth/lips, unresponsive to many medicines. Someone advised grinding Xijiao (Rhinoceros Horn - note: now substituted with Niuxi or other herbs in modern practice, but text says Xijiao). He drank about two bowls, and symptoms instantly disappeared. (From *Youhuan Jiwen*, Song Dynasty, Zhang Shinan.)
Wei Yuhuang treated his cousin Ling Erguan, over twenty. After recovering from Heat syndrome in Bingzi year, doctors immediately gave Sijunzi, Ganjiang, Bajiji, and other Qi-warming tonics. Over time, he became emaciated, acting mad, angry while eating, biting cups, breaking tendons, uncontrollable. He took Renshen Yangrong Wan (Ginseng Nourish Spirit Pills). He was bedridden for a year. His mother asked me. I said: "Residual pathogens were not cleared; premature warming tonics caused this." Ganlu Yin (Sweet Dew Drink) was given for over ten doses, resulting in cure. In summer of Jiashen year, he relapsed with vomiting, nausea, and insomnia. By the seventh day, Baihu Tang was planned. However, previous use of Xijiao Dihuang Tang caused vomiting, leading to suspicion of Cold. A doctor surnamed Lu was invited. Pulse was hidden; abdominal pain upon pressure. He diagnosed Hernia syndrome, not external contraction. Pulse completely absent, very dangerous. Tentatively gave Huiyang (Restore Yang) formula. Only then did pulse recover. Ingredients included Huluba, Wuzhuyu, Rougui, Ganjiang, Muxiang, Xiaohuixiang, Dingxiang, Qingpi, Juhe, totaling over 3 liang, decocted urgently. That night, chills and sweating were expected, causing hidden pulse and cold pain. The doctor misdiagnosed Cold and used this formula. Taken at dusk, it caused agitation, thirst, thrashing, and delirium, but no sweating. Yin fluids were seized by dry heat, unable to evaporate. Early morning re-examination showed pulse emerged, flooding and rapid. Eyes leaked blood, nose coal-like, lips scorched, tongue turning black, no urine. Fresh Dihuang 4 liang, juiced, was given. He slept soundly briefly. Waking, still agitated. Baihu Tang plus 2 liang fresh Dihuang was decocted and taken. Heat gradually receded, spirit cleared. Next day, congee introduced. Whites of eyes red as dove eyes, followed by sores on mouth/nose. Large dose Ganlu Yin was changed to. After twenty-plus days, massive black stool passed, though occasional agitation remained. Taking the previous formula for fifty-plus days, sudden profuse sweating occurred from head to toe, extremely foul-smelling. Complete recovery followed.
Lu Ansheng said: The nose is the orifice of the Lung; the Large Intestine is the residence of the Lung. Childhood study exhausts Heart Qi, consuming Heart Blood. Blood deficiency and Fire prosperity damage Metal (Lung), causing dryness above and below. Er Dong (Ophiopogon and Asparagus) nourishes Metal and clears Fire to treat the branch. Excessive Fire/Dryness implies insufficient Yuanqi. Rectal prolapse and bleeding indicate Yuanqi cannot hold. Shengdi guides Renshen to cultivate the origin of Tianyi (Heaven One) Qi to treat the root; otherwise, childhood consumptive disease is inevitable.
Xu Lingtai said: There are deceitful physicians who profit from patients' wealth, seeking temporary effect regardless of life or death, called "Robbing Agents" (Jie Ji). Robbing agents use heavy drugs to forcibly intercept pathogens. Pathogens entering the body cannot exit instantly; they must be gradually eliminated and pushed out. Seeking one-day results necessitates fierce drugs fighting pathogens, or strong tonics suppressing pathogens. Fierce drugs subdue pathogens but injure Zhengqi. Strong tonics cause Zhengqi to surge, trapping pathogens inside. Temporarily effective, but when drug power fades, pathogens return, and Yuanqi is severely damaged. Examples: Severe heat suppressed by cold drugs; severe abdominal pain treated with aromatic/dry drugs without investigating cause; severe diarrhea sealed with astringents without removing accumulation. This is the method of severity. If pathogens are strong and large doses of Renshen/Fuzi are given, Yang Qi surges temporarily, hiding the illness and calming spirit. After one or two days, Yuanqi combines with pathogens, aiding their toxicity, causing greater harm. This is the method of strong tonification. Such harmful techniques deceive and scam five out of ten physicians. Ignorant physicians imitating them harm others, also five out of ten. Physicians must self-reflect; patients must discern.
Internal Palace Secret Formula for Qinglin Wan: Use 10 jin of Mianwen Dahuang (Cotton-pattern Rhubarb). Soak in rice washing water for half a day, slice, and dry. Soak in non-ash wine for three days, remove, and dry partially. First steaming: Line steamer bottom with Cebaiye (Platycladi Cacumen), spread Dahuang, steam for duration of one incense stick, remove, and dry. Subsequent steamings use Cebaiye lining only, discarding the vaporized air. Second: Mix Dahuang thoroughly with concentrated Moudou (Mung Bean) decoction, steam, remove, dry. Third: Mix with concentrated Damai (Barley) decoction, steam/dry. Fourth: Mix with Heilidou (Black Soybean) decoction. Fifth: Mix with Huaishuoye (Locust Twig/Leaf) decoction. Sixth: Sangye (Mulberry Leaf). Seventh: Taoye (Peach Leaf). Eighth: Cheqiancao (Plantago Herb). Ninth: Houpu (Magnolia Bark). Tenth: Chenpi (Tangerine Peel). Eleventh: Banxia (Pinellia). Twelfth: Baizhu (Atractylodes). Thirteenth: Xiangfu (Cyperus). Fourteenth: Huangqin (Scutellaria). All above decocted, soaked, steamed, dried as before. Fifteenth: Mix with non-ash wine, steam for three incense sticks, remove, dry thoroughly, grind to fine powder. For every 1 jin of Dahuang, add 2 liang Huangniuru (Yellow Cow Milk), 2 liang Ouji (Lotus Root Juice), 2 liang Lizhi (Pear Juice), 2 liang Tongbian (Child Urine). If no child urine, substitute 2 liang refined honey. Add 6 liang refined honey, mix, and form pills size of Wu Zi (Chinese Wolfberry). Dosage: 2 qian per administration. Treats all Heat syndromes.
