一儿季春出痘，感疫气，亢极便秘，用十神解毒汤，次以升发药治之而愈。
一儿季春出痘，七朝顺朗，亢极便秘，狂烦，舌有黑苔刺，痘空处隐隐有黑点，此感疫失解也。先用大承
气汤治之，次以理气血而愈。
万密斋云∶嘉靖甲午春，痘毒流行，死者十八九，乃一厄也。时有预服三豆子汤、丝瓜辰砂散者，凡方书
所载，预解痘毒之法，用之无效。予思痘疹疫疠之毒，因岁运灾眚之变，难以药解，而人事未尽，又不可委之
天数也。于是检阅右方，于《韩氏医通》得五瘟丹，以五运为主。喜曰∶此解毒神药也。根据方修合，施售与人，
但服之莫不轻疏。人皆神之，因命之曰代天宣化九。甘草、（甲已年为君，土。）黄芩、（乙庚年为君，金。）
黄柏、（丙辛年为君，水。）山栀、（丁壬年为君，木。）黄连、（戊癸年为君，火。）连翘、（佐。）山豆根、
（佐。）牛蒡子，（佐。）先见其年所属者为君，次四味为臣，君药倍用，臣药减半，佐视臣又减半。共为细末，
于冬至日修合，取雪水煮升麻汁，打面糊为丸，辰砂为衣，竹叶煎汤下。
万密斋曰∶胡松山子出痘，在母黄氏怀，半夜后，此儿却在地下。萧楚梧子出痘，日中时，闻蒜气过。胡
三溪子出痘，有鸦日日聒噪。胡淑卿子出痘，近夜时，楼上忽闻棹到声，视之无他。王东楼子出痘，有蝙蝠飞
入室。后皆凶，医者病家，不可不知。

一个孩子在春季末出痘，感染了疫气，体内热势亢盛且大便秘结，使用十神解毒汤治疗，随后再用升发类药物调理而痊愈。
另一个孩子在春季末出痘，第七天时痘形顺遂明朗，但体内热势亢盛且大便秘结，出现狂躁烦乱的症状，舌头上有黑色苔藓和刺状突起，痘疹未出处的皮肤隐隐可见黑点，这是感染疫气后未能及时解除所致。先用大承气汤治疗，随后通过调理气血而痊愈。
万密斋说：嘉靖甲午年春天，痘毒流行，死者十之八九，这是一次灾厄。当时有人预先服用三豆子汤、丝瓜辰砂散，凡是医书中记载的预先解除痘毒的方法，使用都无效。我思考痘疹疫疠的毒邪，是由于岁运灾变引起的，难以用药物化解，但人事未尽，也不能完全归咎于天数。于是查阅上述方剂，在《韩氏医通》中找到五瘟丹，以五运为主。高兴地说：“这是解毒的神药。”根据方子配制，施舍售卖给人，凡是服用的人病情无不减轻。人们都视其为神药，因此命名为“代天宣化九”。甘草（甲己年为君药，属土）、黄芩（乙庚年为君药，属金）、黄柏（丙辛年为君药，属水）、山栀（丁壬年为君药，属木）、黄连（戊癸年为君药，属火）、连翘（佐药）、山豆根（佐药）、牛蒡子（佐药）。先确定当年所属五行者为君药，其次四味为臣药，君药加倍使用，臣药减半，佐药再减半。共研为细末，在冬至日配制，取雪水煮升麻汁，调和面糊制成丸剂，辰砂为衣，竹叶煎汤送服。
万密斋说：胡松山的儿子出痘时，母亲黄氏正怀着孕，半夜后，这个孩子却掉在地上。萧楚梧的儿子出痘时，中午时分，闻到蒜气经过。胡三溪的儿子出痘时，有乌鸦整天聒噪。胡淑卿的儿子出痘时，临近夜晚，楼上忽然听到划船的声音，查看却没有其他异常。王东楼的儿子出痘时，有蝙蝠飞进室内。后来这些孩子都不幸夭折，医生和患者家属不可不知道这些征兆。

A child developed smallpox in late spring and contracted epidemic pathogenic factors. The internal heat was extreme, accompanied by constipation. He was treated with Shishen Jiedu Tang (Ten Divine Toxin-Resolving Decoction), followed by treatment with herbs that promote eruption, resulting in recovery.
Another child developed smallpox in late spring. By the seventh day, the lesions were smooth and clear, but the internal heat remained extreme with constipation. The child exhibited manic restlessness, and the tongue presented with black coating and prickles. Faint black spots were visible in the skin areas where pustules had not yet erupted. This indicated that the epidemic pathogen had not been resolved in time. Treatment began with Dachengqi Tang (Major Order the Qi Decoction), followed by regulation of qi and blood, leading to recovery.
Wan Mizhai stated: In the spring of Jiawu year during the Jiajing era, smallpox toxins spread widely, resulting in nine deaths out of ten cases; this was a calamity. At that time, some people pre-administered Sandouzi Tang (Three Bean Decoction) or Sigua Chensha San (Luffa Fruit and Cinnabar Powder). All methods for preemptively resolving smallpox toxins recorded in medical texts proved ineffective. I reasoned that the toxins of smallpox and epidemics arise from changes in the annual cosmic movements and disasters, making them difficult to resolve with medicine. However, human effort is not exhausted, so one cannot simply attribute it to fate. Thus, I reviewed the aforementioned formulas and found Wuwen Dan (Five Plagues Pill) in *Han Shi Yi Tong* (Medical Communications by Master Han), which is based on the Five Movements. I rejoiced, saying, "This is a divine detoxifying drug." I prepared it according to the formula and distributed it to people. Those who took it invariably experienced relief. People regarded it as miraculous, so I named it "Daitian Xuanhua Nine" (Representing Heaven to Proclaim Transformation Nine). Licorice Root (*Gancao*, 甘草) [Ruler herb for Jia-Ji years, Earth]; Scutellaria Root (*Huangqin*, 黄芩) [Ruler herb for Yi-Geng years, Metal]; Phellodendron Bark (*Huangbai*, 黄柏) [Ruler herb for Bing-Xin years, Water]; Gardenia Fruit (*Shanzhi*, 山栀) [Ruler herb for Ding-Ren years, Wood]; Coptis Rhizome (*Huanglian*, 黄连) [Ruler herb for Wu-Gui years, Fire]; Forsythia Fruit (*Lianqiao*, 连翘) [Assistant]; Melia Seed (*Shandougen*, 山豆根) [Assistant]; Arctium Fruit (*Niubangzi*, 牛蒡子) [Assistant]. First, identify the element corresponding to the current year as the Ruler herb. The next four ingredients serve as Minister herbs. The dose of the Ruler herb is doubled, while the Minister herbs are halved. The Assistant herbs are halved again relative to the Ministers. Grind all into fine powder. Prepare on the Winter Solstice. Use snow water to decoct Cimicifuga Rhizome (*Shengma*, 升麻) juice, mix with wheat paste to form pills, coat with Cinnabar (*Chensha*, 辰砂), and administer with a decoction of Bamboo Leaves (*Zhuye*, 竹叶).
Wan Mizhai said: When Hu Songshan's son developed smallpox, his mother, Lady Huang, was pregnant. After midnight, the child fell to the ground. When Xiao Chuwu's son developed smallpox, at noon, an odor of garlic was detected passing by. When Hu Sanxi's son developed smallpox, crows made noise throughout the day. When Hu Shuqing's son developed smallpox, near nightfall, a rowing sound was heard upstairs, but nothing else was found. When Wang Donglou's son developed smallpox, bats flew into the room. All these children subsequently died tragically. Physicians and families must be aware of such omens.
