(Note: Qi stagnation leading to muscle necrosis.)
A woman injured her finger, resulting in swelling of the entire back, slight vomiting, and poor appetite. Others believed this was due to internal attack by toxic vapors. Upon examination, her pulse was deep and fine; this was caused by pain damaging the Stomach Qi. Pus amounting to approximately one bowl was drained via puncture. Initially, Liu Jun Zi Tang (Six Gentlemen Decoction), Huo Xiang (Patchouli), and Dang Gui (Angelica Sinensis) were administered, after which food intake improved. Subsequently, Ba Zhen Tang (Eight Treasures Decoction), Huang Qi (Astragalus), Bai Zhi (Angelica Dahurica), and Jie Geng (Platycodon) were used, and she recovered after more than a month.
A man injured his toe while trimming it; the toe turned black, was painless, and appeared ready to detach. This was due to deficiency of Yang Qi, which failed to circulate to the affected area. The necrotic tissue was urgently removed, and potent tonics were rapidly administered to strengthen Yuan Qi (Original Qi). Had he not done so, the necrosis would have spread up the leg, rendering him incurable. He did not believe this advice and indeed died as the rot extended to his lower leg.
A girl of several years old traveled to the capital during severe cold. Her feet became numb from frostbite, and she soaked them in hot water, causing skin breakdown. By spring, all ten toes had rotted and remained attached. She was first treated with formulas that supported the interior and aided Yang Qi, allowing the necrotic parts to spontaneously separate and slough off, thereby saving her life. This occurred because Cold Evil obstructed and severed the circulation of Qi and Blood, compounded by the injury from hot water soaking, leading to necrosis without pain. It is common to see people who go out in severe cold suffer frostbitten ears that feel no itch. If touched by hand, the ear may fall off. One should remain in a warm place for a long time or warm the ear with hands to ensure safety. If roasted by fire or soaked in hot water, death ensues, and by spring, ulceration and detachment will occur. Northern Cold damages the human body thusly; how can one fail to examine this carefully?
Cao Zirong from Yunjian had half of a finger bitten off when his wife, suffering from wind-stroke (Zhong Feng), was being medicated. He experienced aversion to cold and fever. External application of Da Huang (Rhubarb) and other herbs, along with internal administration of cooling and detoxifying agents, resulted in the lesion being painless and non-ulcerated, with thin pus, and worsening aversion to cold and fever. This was due to cooling medicines obstructing the meridians and collaterals. Yu Long Gao (Jade Dragon Ointment) was applied externally to disperse cold, and Liu Jun Zi Tang (Six Gentlemen Decoction) was taken internally to strengthen the Spleen and Stomach. After several days, the lesion became slightly painful, and the swelling gradually subsided, indicating the return of Yang Qi to the area. Indeed, thick pus emerged, and within a few days, half the finger ulcerated and detached. Further treatment with interior-supporting formulas led to healing.
Wang Tianjue, a Shangshe, suffered foot injury and swelling, accompanied by internal heat and thirst. Both external applications and internal medications were cooling and detoxifying. Consequently, the lesion swelled more without ulcerating, and he developed aversion to cold, poor appetite, and nausea. This was due to deficiency of both Qi and Blood, combined with cooling medicines congealing the meridians and damaging Stomach Qi, leading to these symptoms. Xiang Sha Liu Jun Zi Tang (Amomum and Atractylodes Six Gentlemen Decoction), Dang Gui (Angelica Sinensis), Chuan Xiong (Ligusticum), and Pao Jiang (Prepared Ginger) were administered, resolving all external symptoms. However, he still experienced fatigue and afternoon fever, with unpalatable diet. Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) added with Di Gu Pi (Cortex Lycii), Wu Wei Zi (Schisandra), and Mai Dong (Ophiopogon) cured him.
Wang Tingyong, a Zhou Shou, injured his finger and immediately wrapped it with cloth. This caused blood stasis to rupture internally, with pain radiating to the hand. It was advised to unwrap the wound to drain the stasis and use formulas to eliminate stale matter and generate new tissue. He refused, applying cooling medicines instead. Although symptoms temporarily ceased, they recurred the next day. Repeated application led to ulceration of the palm and dorsum of the hand after several days, discharging foul, bloody pus. He continued taking detoxifying formulas, further depleting Qi and Blood, resulting in dark complexion, thin pus, and loss of appetite. Upon reconsultation, formulas strengthening Spleen and Stomach Qi and Blood were administered. The pus gradually thickened, and he recovered.
Qian Guobin stated: In the spring of the Jiazi year, while my boat was moored at Qingjiangpu, sandboats belonging to troops conscripted for Liaodong were driving piles on both banks. The current was swift, breaking the ropes. Commander Wang Yuan fell, landing head-down and feet-up, directly onto a stone post. His skull cap, round like a large bell, was completely detached and fell to the ground, about one zhang away. Soldiers gathered around. Seeing this, I shouted, "I know bone-setting. Now the patient's brain is exposed, his soul and spirit are scattered, he fears people, and his spirit cannot return to its cavity. You stay here, and this person will die instantly." I told them to disperse temporarily. His pulse was surging and floating. Although the skull bone was detached, the dura mater was intact, so he could be saved. The skull bone was first secured. Urgently, bone-setting medicine from the boat was taken and scattered around the wound. Internally, Si Wu Tang (Four Substances Decoction) plus Tao Ren (Peach Kernel) and Da Huang (Rhubarb), each 1 qian, and Hong Hua (Carthamus), 5 fen, were given. Fearing blood attacking the heart, the patient was moved to a draft-free room and instructed to lean rather than lie down, as lying down would cause blood to rise upward. After half a day, he regained consciousness and could eat the next day. One dose of bone-setting medicine was administered daily, and he recovered in ten days. Divine Bone-Setting Formula: Tu Bie Chong (Ground Beetle), 49 pieces, prepared with wine; Gu (Bone) of a person who died suddenly, 1 liang; Xie Huang (Crab Yellow), 5 qian; Xiang Shi (Elephant Louse), 10 pieces; Ban Liang Qian (Half-Ounce Coin), 10 pieces, quenched in red vinegar to obtain powder; Zi Ran Tong (Natural Copper), 3 qian, quenched in red vinegar; Ru Xiang (Frankincense) and Mo Yao (Myrrh), each 3 qian; Mu Xiang (Aucklandia), 2 qian; She Xiang (Musk), 5 fen. Grind into powder. Take 7 fen per dose, mixed with hot wine. Add wine according to need to promote the action of the medicine. After ingestion, sounds will naturally arise from the bones. Mild cases require a few doses; severe cases require over ten doses. The bone will join as if original.
