Wang Hongxu treated a woman who had three phlegm nodules on her neck. The condition had persisted for years, leading to the formation of sinus tracts. He inserted medication for extracting sinuses, changing it daily. After half a month, two were healed, but one sinus tract gradually became shallower. Unexpectedly, when her husband returned from afar, the depth of the sinus reverted to its previous state within two days. Later, after her mother took her home, she was cured upon treatment.
A young man had two sinus tracts on his arm. Wang asked if he had any hidden ailments regarding sexual activity. The patient replied that he had suffered from nocturnal emissions since youth. Wang prescribed Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) with Ze Xie (Alisma Rhizome) removed, and added Gui Jiao (Tortoise Shell Glue), Long Gu (Fossilized Bones), Qian Shi (Euryale Seed), and Lian Xu (Lotus Stamen). These were made into pills. Lu Han Cao (Hypoxis Root) decoction was used to wash down three qian twice daily. After taking half a dosage, he was cured. Immediately after recovery, sinus extraction medication was applied, and the previous pills were continued, resulting in the healing of both tracts.
Another patient suffered from an "evil nodule" (malignant lymph node tuberculosis) below the ear, which had been incised by another physician, forming a sinus tract. Yanghe Tang (Yang Harmony Decoction) and Xiaojin Dan (Small Golden Pill) were administered alternately. The un-ulcerated nodules completely resolved, and the patient then requested the elimination of the sinus tract. Wang said, "Eliminating the sinus is easy; once the sinus disappears, it will heal. However, if during the healing process, there is any leakage of essence, the sinus will regenerate." Fortunately, the patient was careful with his health, and he eventually recovered. The formula for eliminating sinuses consists of: Zao Jiao Ci Jian (Acacia Spine Tip) 5 qian, Zhe Shu Mo (Zelkova Tree Membrane) 5 qian, Hong Fu Jin Qian Bie (Red-bellied Money Turtle) 3 qian, Yu Shu Pi (Elm Bark) 1 qian, and Zhen Chan Su (Real Toad Venom) 1 qian, all ground into an extremely fine powder. Whenever encountering a leaking sinus, first probe it with a pig bristle to determine its depth, then roll the medicine in cotton paper into strips and insert them. Change and insert daily until healed.
Xue Lizhai treated a woman with irregular menstruation and nodular masses in her limbs, varying in size like hazelnuts or beans, numerous in count, hidden deep within the muscles, with unchanged skin color, persisting for over three years. Deep pressure caused pain. When treated with cooling fire and detoxifying herbs, pain occurred even without pressure, accompanied by fever, thirst, worsening in the late afternoon, and delayed menstruation. The pulse at the left Guan position was rapid. This indicated Liver Fire causing Blood Dryness. Qinggan Yirong Tang (Liver-Clearing and Nourishing Decoction) was prescribed for over sixty doses, resolving most symptoms. However, the cervical nodules remained. Danggui Longhui Wan (Angelica Dahurica and Gardenia Pill) was taken as a powder, followed by Bazhen Tang (Eight-Treasure Decoction) augmented with Chai Hu (Bupleurum Root) and Shan Zhi (Gardenia Fruit). After thirty-plus doses, she fully recovered.
Another woman, suffering from long-term depression and anger, developed nodules in the chest, hypochondrium, and outer thigh. She experienced alternating chills and fever, irregular menstruation, chest fullness, poor appetite, and irregular bowel movements. The left Guan pulse was wiry and surging, the right Cun pulse was wiry and rapid, and the right Guan pulse was wiry and tight. Xue explained: "The wiry and surging left Guan indicates Heat in the Liver Meridian; the wiry and rapid left Cun indicates Wood generating Fire (Liver Fire affecting the Heart); the wiry and tight right Guan indicates Wood overcoming Earth (Liver attacking Spleen); the floating right Cun indicates Wood insulting Metal (Liver Fire affecting Lung). The treatment should focus on nourishing Liver Blood." Jiawei Siwu Tang (Modified Four Substances Decoction) was used to resolve the symptoms. Jiawei Xiaoyao San (Modified Free Wanderer Powder) regulated menstruation, and Jiawei Guipi Tang (Modified Restore the Spleen Decoction) led to complete recovery.
A woman developed limb nodules due to anger, experiencing convulsions during sleep. The left Guan pulse was wiry and surging, indicating Liver Fire and Blood Dryness causing tendon spasms. Treatment involved clearing Liver Fire and nourishing Yuan Qi using Jiawei Xiao Chaihu Tang (Modified Minor Bupleurum Decoction) and Jiawei Xiaoyao San, leading to gradual improvement. Subsequently, Bazhen Tang augmented with Dan Pi (Moutan Cortex), Chai Hu, Shan Zhi, and Gou Teng (Uncaria Ramulus) cured her.
Another woman presented with limb nodules, chest and abdominal distension, relieved slightly by flatulence, indicating Liver-Spleen Qi Stagnation. Disbelieving this diagnosis, she took herbs for clearing heat, moving Qi, and resolving phlegm, which worsened her condition and depleted her Qi. Alternating Jiawei Xiaoyao San and Jiawei Guipi Tang resulted in recovery after half a year.
A woman with neck nodules experienced intermittent chills and fever, a tightly closed left eye, tremors of the head and neck, and limb convulsions. This was diagnosed as Liver Fire with Blood Deficiency and Wind-Heat. Jiawei Xiaoyao San plus Gou Teng was administered for several doses, gradually improving symptoms. Bazhen Tang was then used for conditioning, leading to full recovery.
A woman suffered from painful nodules inside and behind the ear and on the side of the neck, with alternating chills/fever, bitter taste, and irregular menstruation. This was attributed to Fire in the Liver and Gallbladder meridians damaging the Spleen and Stomach. Sijunzi Tang (Four Gentlemen Decoction), Chai Hu, Dan Pi, and Liuwei Dihuang Wan cured her.
A woman developed nodules due to anger, with continuous menstrual bleeding, fever, quietness during the day, and delirium at night. This indicated Heat in the Blood Level. Xiao Chaihu Tang plus Sheng Di Huang (Rehmannia Root) provided immediate relief. However, the nodules persisted, and menstruation came early with a wiry and rapid Liver pulse, indicating Liver Fire depleting Blood and causing tendon spasms. Jiawei Xiaoyao San plus Sheng Di Huang restored regular menstruation and resolved the nodules.
A woman had neck and arm nodules, headache, chills/fever, intermittent breast pain, and flank pain. This was Liver-Spleen Depressed Fire causing Blood Dryness. Jiawei Xiaoyao San was used first, followed by Jiawei Guipi Tang, resulting in recovery.
A woman with a history of depression and anger suffered from internal heat and afternoon fever due to nodules, persisting for a long time. If her complexion was sallow, menstruation was delayed and scanty; if red, it was early and heavy. Xue stated: "Sallow complexion with delay indicates Spleen Deficiency; red complexion with early onset indicates Spleen Deficiency with Fire Hyperactivity." Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) was taken in the morning to raise Spleen Qi and nourish Blood and Qi. Jiawei Xiaoyao San was taken in the evening to nourish Liver Blood and extinguish Yin Fire. Guipi Tang was used to resolve stagnation. After half a year, Yuan Qi recovered, and she healed. Another case with similar symptoms involved Spleen Deficiency with sinking Qi causing fever; treating only the sores led to consumption disease and death.
A young woman had nodules below the ear with pain and chills/fever, attributed to Wind-Heat in the Liver Meridian. Zhizi Qinggan Yin (Gardenia Liver-Clearing Infusion) cured all symptoms in one dose. Later, due to anger, pain occurred behind the ear and at the corners of the forehead with chills/fever, involving the Shaoyang Meridian. The previous formula plus Qiang Huo (Notopterygium Rhizome) cured her in two doses.
A woman had five neck nodules with frequent chills/fever. The Liver and Lung pulses were wiry and long, appearing at the Cun position. This indicated a condition of Excess Blood without balance (Yin-Yang disharmony). Xiao Chaihu Tang plus Sheng Di Huang and Wu Mei (Mume Fruit) cured her. (Xu Dachun noted: This pulse is common in Yin Deficiency; treatment should strengthen Water. Adding Wu Mei and Sheng Di to Xiao Chaihu Tang is merely one standard approach.)
Shi Ershou had a nodule on the right side of his neck. Applying cooling medicines caused swelling throughout the neck. Further application caused swelling to spread to the chest and hypochondrium, with cold sensations extending to the abdomen. Misinterpreting this as severe toxicity rather than damage from cooling herbs, he became exhausted and despaired. Seeing heat vapor rising from the application site, the medication was urgently removed. After some time, the lesion turned red, and pus and blood were drained. Toli medications (Supporting Interior formulas) led to recovery.
Juren Jiang Jiefu had nodules below both ears, on both arms, and ribs. Strict use of phlegm-resolving, fire-clearing, and softening-hardness agents worsened the condition. Xue Lizhai diagnosed it as Blood Deficiency and Fire Dryness in the Gallbladder Meridian, noting that the previous drugs damaged the meridian running along the sides of the body. By July of the following year, all nodules ulcerated. The pulse was floating, large, and choppy. In autumn, when Metal is prosperous and overcomes Wood, the prognosis was fatal. He indeed died.
Zhoushang had tendon traction, swelling, hardness between the ears and neck, cough, asthma, internal heat, and night sweats. Having taken phlegm-resolving and Qi-moving agents, his condition worsened. The left Guan pulse was wiry and choppy; the left Chi pulse was surging and rapid. This was attributed to Anger injuring the Liver and Sexual Exhaustion damaging the Kidney. Nourishing Kidney Water and generating Liver Blood, along with careful regulation, was required, expecting recovery during the Water-prosperous season. Seeking quick results, he used external applications of Shang Lu (Phytolacca Root) and Duan Shi (Calcined Alumen), and internal herbs like Hai Zao (Sargassum) and Peng Ru (Curcuma Zedoaria). In autumn, when Metal is strong, Yuan Qi further depleted, leading to severe swelling and death.
A Sheshe, known for being bold and prone to anger, had a nodule below the ear that ulcerated with an everted, mushroom-like wound edge, radiating pain to the head, occasional chest/hypochondriac distension, and internal chills/fever. Heat-clearing and detoxifying herbs failed for over a year. Bu Zhong Yi Qi Tang and Liuwei Dihuang Wan led to quick recovery. (An everted wound is also called Fan Hua Chuang / Flower-Everting Sore.)
The son of Sha Huangmen had a nodule at the wrist joint. Half a year later, a small pore like a millet grain ulcerated, remaining open for another year, draining clear pus a few drops daily. His face was red, pulse rapid, mouth dry, with night fever and daytime aversion to cold, yet walking normally. This indicated Dual Deficiency of Qi and Blood. Treatment was declined. A month later, they met again, and he reported spontaneous healing. Xue said, "Obeying the order not to treat was wise." Shortly after, he died.
Miu Zhongchun treated a young woman with a hard, painless lump under the chin, resembling Shi Ying (Stone Goiter). Bei Mu (Fritillaria Bulb) and Shou Wu (Polygonum Multiflorum Root) each 3 qian; Lian Qiao (Forsythia Fruit), Bai Ji (Bletilla Rhizome), and Hua Fen (Taro Tuber) each 2 qian; Niu Bang Zi (Arctium Fruit), Cang Er Zi (Xanthium Fruit), and Qing Mu Xiang (Tasmannia Stem) each 1.5 qian; Yin Hua (Chrysanthemum Indicum Flower), Xian Ju (Fresh Chrysanthemum), and Di Ding (Sonchus Herb) each 5 qian. First, Xia Ku Cao (Prunella Spike) 5 liang was decocted in 5 bowls of river water to 3 bowls, strained, and the other herbs added, decocted to 1 bowl. Ten doses resulted in complete resolution. (External formula: Nan Xing (Arisaema Rhizome) 3 liang; Hai Zao, Kun Bu (Kelp), Bing Lang (Areca Seed), Jiang Huang (Turmeric Rhizome), Bai Lian (Ampelopsis Rhizome), Ya Zao (Soap Bean) each 1 liang, powdered, mixed with vinegar.)
Xue Lizhai treated a man with mental fatigue and anger, presenting with a neck swelling that persisted despite various treatments. Bazhen Tang augmented with Chai Hu and Xiang Fu (Cyperi Rhizoma) was prescribed. Daily moxibustion through garlic slices and drinking Yuan Zhi Jiu (Polygala Wine) 2-3 cups daily led to gradual resolution.
