Xue Lizhai treated a puerpera who presented with lower back pain, abdominal distension, frequent belching, and vomiting of all administered medicines. Xue diagnosed this as Spleen deficiency (deficiency of the Spleen organ) and prescribed a single herb, Baizhu (Atractylodis Macrocephalae Rhizoma), stir-fried until slightly yellow, at a dosage of one liang per dose. It was soaked in rice washing water and administered in small spoonfuls occasionally. After four doses, symptoms gradually improved, and after more than a hundred doses, she recovered completely.
Another puerpera developed distending pain in both hypochondria due to anger, accompanied by massive hematemesis, fever, aversion to cold, and chest and hypochondriac distending pain. This was identified as a deficiency of both Qi and Blood. She was treated with Bazhen Tang (Eight-Treasure Decoction) augmented with Chaihu (Bupleuri Radix), Danpi (Moutan Cortex), and Paojiang (Prepared Zingiberis Rhizoma), which stopped the bleeding immediately. Subsequently, Shiquan Dabu Tang (Ten-Ingredient Great Tonifying Decoction) was used to resolve the alternating chills and fever. For this condition, without using acrid-warm herbs like Jiangxin (Dried Ginger and Galangal) to assist the Spleen and Lung in promoting the action of the medicine, not only would the treatment fail, but it might also exacerbate the distension. (Commentary: The pulse must also be consulted before applying this method.)
The wife of Wang Shiheng suffered from swelling and pain in the waist after childbirth, particularly severe in the hypochondria. This was caused by stagnant blood obstructing the collaterals. If not treated promptly, it would inevitably develop into bone abscess (Guju). Taoren Tang (Peach Kernel Decoction) was prescribed, resulting in slight improvement after two doses. She then took several doses of Moyao Wan (Myrrha Pill) and recovered. There are also cases where lochia remains incomplete, causing stabbing pain in the umbilical and abdominal regions, or where blood stasis flows to the limbs or inner thighs, causing pain like needle pricks; or swelling and pain in both thighs due to imbalance of cold and heat; or due to excessive worry and physical exertion leading to Qi stagnation and blood accumulation in the collaterals. These should be treated with Moyao Wan. Cases of swelling and pain due to amenorrhea should be treated with Danggui Wan (Angelica Sinensis Pill). Any stagnation of lochia poses a serious threat; if treatment is delayed, it may flow and form bone abscesses, often leading to death.
Yanyi recorded the treatment of a woman who gave birth during a cold month, presenting with fullness and unbearable tenderness in the umbilical and abdominal regions, unresponsive to various treatments. Some suspected blood stasis and proposed Didang Tang (Resolute Decoction). The physician stated: "This is not the correct treatment. This is Spleen deficiency-cold with pathogenic factors lodging in the Zi Men (uterine opening)." He prescribed lamb meat (4 liang), Danggui (Angelicae Sinensis Radix), Chuanxiong (Chuanxiong Rhizoma), Chenpi (Citri Reticulatae Pericarpium) (5 qian each), and Shengjiang (Fresh Zingiberis Rhizoma) (1 liang), decocted and taken orally. The patient stabilized after two or three doses.
Zhou Shenzhai treated a puerpera with abdominal distension and pain. After taking blood-invigorating and stasis-resolving drugs, her lower abdominal pain worsened, with hardness extending into the greater abdomen. She was treated with Shengjiang (Fresh Zingiberis Rhizoma), Guirou (Cinnamomi Ramulus), Wuzhuyu (Evodiae Fructus), and Bibo (Long Pepper Fruit) for several doses, resulting in recovery. (Same source.)
A puerpera suffered from lower abdominal pain, occasional vomiting, and faintness. This was diagnosed as Spleen Qi deficiency-cold. Renzhen Lizhong Tang (Ginseng Regulate the Middle Decoction) led to gradual improvement. She was then fully cured using Buzhong Yiqi Tang (Tonify the Middle and Augment Qi Decoction) augmented with Fuling (Poria) and Banxia (Pinelliae Rhizoma). Later, pain recurred accompanied by asthma. Buzhong Yiqi Tang was used again to tonify the Spleen and Lung, leading to recovery. (*Liangfang*.)
A puerpera presented with lower abdominal pain, difficult urination, internal heat, afternoon tidal fever, and physical fatigue. Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) was used to clear Liver fire and generate Liver blood, while Buzhong Yiqi Tang (Tonify the Middle and Augment Qi Decoction) was used to tonify the Spleen and Stomach and raise Yang Qi, resulting in recovery. (Same source.)
Zhu Danxi treated a woman in the Feng household who had postpartum fever, abdominal pain, palpable masses, spontaneous sweating, and aversion to cold. She had previously taken Heishen San (Black Spirit Powder). Zhu prescribed Baizhu (Atractylodis Macrocephalae Rhizoma) and Baishao (Paeoniae Radix Alba) (3 qian each), Huashi (Talcum) (5 qian), Huangqin (Scutellariae Radix) and Danpi (Moutan Cortex) (2.5 qian each), Renshen (Ginseng Radix), Chuanxiong (Chuanxiong Rhizoma), Guiwei (Angelicae Sinensis Radix Tail), Chenpi (Citri Reticulatae Pericarpium), Jingjie (Schizonepetae Herba), and Ganjiang (Zingiberis Rhizoma) (1 qian each), with a small amount of Gancao (Licorice Root).
Xue Lizhai treated a woman with postpartum lower abdominal pain and a palpable mass. Her pulse was hollow (Kou) and choppy (Se). She was treated with Siwu Tang (Four-Substance Decoction) augmented with Yuanhu (Corydalis Rhizoma), Honghua (Carthami Flos), Taoren (Persicae Semen), Niuxi (Achyranthis Bidentatae Radix), and Muxiang (Aucklandiae Radix), resulting in recovery.
The mother of Zhou Yuwen suffered from incessant abdominal pain for over a month postpartum, progressing to nausea, anorexia, aversion to cold, and fever, unresponsive to medication. Someone advised burning 4 liang of lychees (with seeds and shells) to ash while preserving their medicinal properties, weighing exactly 4 liang, and decocting them in good wine. This could be taken in divided doses or all at once. Note: This practice is specific to Pingyang. Postpartum women there regularly consume old ginger soup or ginger-vinegar mixtures because the local waters and mountains are cold. Consuming less leads to significant harm. (Commentary: This is not unique to Dong'ou but is even more prevalent in Guangdong, driven by custom. Poor families may suffer little harm, but those living in comfort often die from such damage, unaware of the cause.)
