Xu Zhongguang treated a child who, after smallpox, developed fever after noon, with a red face, delirium, and mania. This was fire arising from deficiency. He used Baoyuan Tang (Preserve the Origin Decoction) added with Danggui (Angelicae Sinensis Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), and Huanglian (Coptidis Rhizoma), resulting in recovery.
Another child, after smallpox, had reduced body heat and appetite, sallow complexion, and emaciation, with a deep and slippery pulse on the right side. Although it was an internal injury, the Spleen and Stomach were deficient; thus, regulation should precede elimination. He used Liujunzi Tang (Six Gentlemen Decoction) added with Shanzha (Crataegi Fructus) and Qingpi (Aurantii Fructus Pericarpium Viride), resulting in recovery.
A third child, after the scabs of smallpox fell off, could eat but suffered from constipation, body heat, and thirst. This was accumulated heat in the Stomach; if delayed, complications would arise. He used Sanhuang Wan (Three Yellows Pill) and Qingwei Tang (Clear the Stomach Decoction) to promote purgation, resulting in recovery.
A fourth child, after smallpox, experienced fullness and dry heat in summer. The physician used Baiduo (Crotonis Fructus) to purge, which slightly relieved epigastric fullness but caused dry intestines, abdominal pain, diarrhea resembling rotten fish brains, vomiting upon forced eating, persistent fullness and belching, pain in the feet and elbows, red tongue, and difficult urination. This indicated damage to Yuan Qi (Original Qi), further injured by hot herbs, leading to the loss of Yin and the survival of Yang alone.
A fifth child, after smallpox, suffered from food stagnation. Overuse of harsh draining medicines led to sallow complexion, poor appetite, afternoon tidal fever, and worsening diarrhea when cooling agents were used. This was due to insufficient Middle Qi (Middle Jiao Qi). He used Liujunzi Tang (Six Gentlemen Decoction) and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), resulting in recovery.
A sixth child, after smallpox, had body heat, irritability, red face, red lips, and convulsions. This was wind generated by extreme heat due to excessive warmth from clothing or proximity to fire. He used Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) combined with Daochi San (Direct Flowing Powder), resulting in recovery.
Wan Mizhai treated the son of the Xu family, who had fever after smallpox. Xiao Chaihu Tang (Minor Bupleurum Decoction), Zhuye Tang (Bamboo Leaf Decoction), and Huanglian Jiedu Tang (Coptis Detoxification Decoction) were ineffective. He prescribed Baoyuan Tang (Preserve the Origin Decoction) added with Danggui (Angelicae Sinensis Radix) and Chaohei Ganjiang (Stir-fried Blackened Zingiberis Rhizoma). One dose eliminated the fever.
Fei Jianzhong treated a one-year-old girl of the Shen family, who had burning fever, profuse sweating like rain, restless eyes, and fear as if facing knives and saws. He used Dahuang (Rhei Radix et Rhizoma) 4 fen, Huanglian (Coptidis Rhizoma) 3 fen, and Shigao (Gypsum Fibrosum) 2 qian, assisted by Qingpi (Aurantii Fructus Pericarpium Viride), Mutong (Akebiae Caulis), Shanzha (Crataegi Fructus), Danpi (Moutan Cortex), and Chantui (Cicadae Periostracum). After two doses, her spirit settled, and a few distinct spots appeared. After another two doses, the heat subsided, she felt refreshed, and the lesions were sparse and moist. With harmonizing agents, she recovered in eight days.
Wu Qiao treated Wang Yilong, who contracted smallpox in childhood. On the seventh day, the lesions were numerous but did not rise, limbs were cold, and the lesions resembled floating duckweed, dry, red, withered, and purple. He was extremely thirsty, drinking dozens of sheng of tea water daily, with uncontrollable watery diarrhea. All physicians considered it a cold syndrome and administered Muxiang Yigong San (Aucklandia and Four Gentlemen Powder) and Dushen Tang (Single Ginseng Decoction) for tonification. Subsequently, several clots of blood were passed, and the physicians left. Wu Qiao arrived and diagnosed it as a fire syndrome, which others disputed. They said: "Cold body, high water intake, and continuous diarrhea indicate that even if there is heat, it is not harmful." Wu Qiao said: "Watery diarrhea indicates accompanying heat; cold body indicates heat jue (reversal)." Examining his mouth, he saw one red spot and three black spots on the tongue. He told the crowd: "How can this not be heat?" He then administered strong cold-cooling agents. After one dose, he slept, and thirst ceased. By the next morning, after four doses, the lesions became slightly red and vibrant. On the ninth day, they rose. (Taihan Ji.)
