Whenever postpartum patients experience alternating chills and fever (alternating cold and heat) that occur at regular intervals daily, with symptoms resembling malaria (a disease characterized by paroxysms of cold and fever), one must absolutely not treat it with medicines intended for phlegm-type malaria. This is because deficiency of Qi and Blood leads to alternating chills and fever, while weak Yuan Qi (Original Qi) allows external pathogens to invade. Although the patient may shiver intensely when chills come, unable to be warmed even by hot water or fire; and although they may feel as if burning when fever arises, unable to be cooled even by ice water. The pattern may present as milder during the day and heavier at night, or with chills and fever occurring in the late afternoon (Shen hour). Although the observed symptoms resemble those of malaria, the treatment must focus on nourishing Ying (Nutritive) Blood and tonifying Qi to resolve the chills and fever. If sweating occurs, immediate antiperspirant measures are required, such as using Mahuang Gen Tang (Ephedra Root Decoction). If sweating is confined only to the head and does not extend to the hands and feet, this indicates a critical condition of "Lone Yang and Absolute Yin" (severe Yin deficiency with floating Yang); in such cases, Di Huang (Rehmanniae Radix) and Dang Gui (Angelicae Sinensis Radix) should be added to the formula. If the cause is Cold pathogen invasion leading to headache and absence of sweating, it is appropriate to add Qiang Huo (Notopterygii Rhizoma et Radix), Fang Feng (Saposhnikoviae Radix), Lian Xu (Nelumbinis Stamen), and Cong Bai (Allii Fistulosi Bulbus) within Sheng Hua Tang (Invigorate the Blood Decoction) to disperse the exterior. One must strictly avoid treating it as malaria to prevent medical error. Chai Hu Tang (Bupleurum Decoction), Qing Pi Yin (Green Tangerine Peel Drink), Chang Shan (Polygalae Tuber), and Cao Guo (Tsaoko Fructus) and other formulas and herbs are all contraindicated.
Mahuang Gen Tang (Ephedra Root Decoction)

In women postpartum suffering from malaria-like illness, where there is sweating on the head but not on the body or feet, if one relies solely on Ginseng tonics without including sufficient blood-nourishing herbs, the condition is often incurable.
Zirong Yiqi Fuzheng Tang (Nourish Blood and Tonify Qi to Support Righteousness Decoction): Treats postpartum alternating chills and fever with sweating, occurring regularly every afternoon.

For prolonged postpartum malaria with no sweating and non-resolution, concurrently decoct Ba Zhu Ren Shen Gao (Atractylodes and Ginseng Paste) to assist the medicinal efficacy.
