Postpartum suffering from strangury (Lin syndrome, characterized by painful and frequent urination) is caused by postpartum weakness, with heat pathogens invading the bladder. Internal deficiency leads to frequent urination; heat causes urine to be scanty, difficult to pass, and painful, a condition named "strangury." It is advisable to take Baogen Tang (White Reed Rhizome Decoction).
Baogen Tang: Treats postpartum alternating cold and heat, as well as various types of strangury such as gao lin (fatty strangury) and shi lin (lithiasic strangury).
Ingredients: Bai Mao Gen (Imperatae Rhizoma, 3 qian), Qumai (Dianthi Herba, 2 qian), Fuling (Poria, 2 qian), Renshen (Ginseng Radix et Rhizoma, 1 qian), Kuizi (Abelmoschi Semen, 1.5 qian), Gancao Shao (Glycyrrhizae Radix et Rhizoma caulis, 0.5 qian), Huashi (Talcum, 2 qian), Zi Bei (Concha Arcae, 2 pieces), Shishou Yu Tou (Cynoscion squamipinnis head, 2 pieces), Tao Jiao (Pruni Persicae Gum, 1.5 qian).
Usage: Use fresh ginger and Jixin (Junci Medulla) as guides, taken on an empty stomach. Additionally, add 0.5 qian of powder made from carp teeth.
Another prescription: Treats postpartum strangury, painful urination, and also xue lin (hematuria).
Ingredients: Bai Mao Gen (Imperatae Rhizoma), Cheqianzi (Plantaginis Semen), Qumai (Dianthi Herba), Dongkuizi (Abelmoschi Semen), Tongcao (Tetrapanacis Medulla), Liyu Lin (Cyprini carpio squama, 100 scales).
Usage: Decoct in water, add powder made from carp teeth, and take.
Yet another method: Using Dao Chi San (Guide Out Red Powder) with modifications is most excellent.
Postpartum frequent urination is due to pre-existing cold qi residing in the bladder. Upon the onset of labor, cold qi enters the bladder, causing frequent urination. It is advisable to use Sangpiaoxiao San (Mantis Egg-case Powder).
Sangpiaoxiao San:
Ingredients: Renshen (Ginseng Radix et Rhizoma, 2 liang), Huangqi (Astragali Radix, 2 liang), Sangpiaoxiao (Mantidis Oötheea, 30 pieces), Lurong (Cornu Cervi Pantotrichum, 5 qian), Muli (Concha Ostreae, 5 qian), Chishizhi (Lapis Kaolinicus, 5 qian).
Usage: Grind into fine powder, take on an empty stomach with millet gruel broth. (Standard dose is 2 qian per administration.)
Another prescription: Treats urinary incontinence or enuresis.
Ingredients: Yizhi Ren (Alpiniae oxyphyllae Fructus, 30 pieces, stir-fried).
Usage: Grind into powder, take with rice water, 1 qian per dose.
Postpartum liuzhu (flowing obstruction, often referring to abscesses or swelling due to stagnant lochia) occurs when lochia stagnates in the joints of the waist, arms, hands, and feet. This may present as diffuse swelling or nodules. Over time, swelling rises and becomes painful, accompanied by limb fatigue and weakness. The treatment method should include Congyi Fa (Scallion Moxibustion/Ironing Method). Externally apply formulas for treating surface swelling; internally administer Shen Gui Shenghua Tang (Ginseng and Angelica Birth-Invigorating Decoction) to dissipate stagnant blood without delay.
Shen Gui Shenghua Tang:
Ingredients: Chuanxiong (Ligustici Chuanxiong Rhizoma, 1.5 qian), Danggui (Angelicae Sinensis Radix, 3 qian), Zhi Gancao (Prepared Glycyrrhizae Radix et Rhizoma, 0.5 qian), Huangqi (Astragali Radix, 1.5 qian), Rougui (Cinnamomi Cortex, 0.6 qian), Renshen (Ginseng Radix et Rhizoma, 2 qian), Mati Xiang (Costi Costus Root, 2 qian).
If there is diffuse swelling with slight pain, it indicates insufficiency of both form and qi, which is the most difficult to treat. If pus has not formed or has formed, or if the wound does not ulcerate/heal, it is due to qi and blood deficiency. It is advisable to take Renshen Tang (Ginseng Decoction).
If experiencing aversion to cold with high fever, it is due to qi deficiency; use Shiquan Dabu Tang (Perfect Ten Great Tonifying Decoction). If internal heat occurs at dusk (shen hour, 3-5 PM), use Siwu Tang (Four Substances Decoction) plus Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), and Mudanpi (Moutan Cortex). If vomiting and regurgitation occur, it belongs to stomach qi deficiency; use Liujunzi Tang (Six Gentlemen Decoction) plus Paojiang (Prepared Ginger Rhizome). If appetite is poor and body is tired, it is spleen qi deficiency; use Buzhong Yiqi Tang (Middle-Boosting Qi-Supplementing Decoction) plus Yizhi Ren (Alpiniae oxyphyllae Fructus, 1 qian).
Congyi Fa (Scallion Ironing Method): Take a handful of scallions, heat them, pound into a paste to make cakes. Place two or three layers of thick cloth over the swollen area, and iron it with a hot iron.
Shenxian Hui Nong San (Immortal Pus-Returning Powder) (Formula found under Breast Carbuncle in Volume 8): Treats postpartum liuzhu where lochia turns into pus after a long duration. Use this formula first to guide the discharge of pus. If the patient has not been tonified and qi/blood are robust, this formula is not suitable. (Formula found under Breast Carbuncle, Yin Erosion, and Five Gangrene sections.)
Method for treating Yin Erosion and Five Gangrene: When sores appear on the female genitalia, it is called "Du" sore. Symptoms include pain or itching, sensation like insects crawling, dripping purulent discharge, or even complete erosion. This is caused by heart and kidney vexation and depression, and weak stomach qi, leading to stagnant and sluggish qi and blood. The Classics state: "All sores and itching belong to heart fire." Treatment should focus on nourishing the heart and stomach, combined with external washing, fumigation, and guiding therapies.
Qianjin Yao Fang Yin Shi Chuang Xi Fang (Thousand Gold Prescription for Washing Yin Erosion Sores):
Ingredients: Chuanxiong (Ligustici Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix), Shaoyao (Paeoniae Radix), Diyu (Sanguisorbae Radix), Gancao (Glycyrrhizae Radix et Rhizoma).
Usage: Use five sheng of water, boil down to two sheng, remove dregs, fumigate and wash. Three times daily, once at night.
Another prescription: Puhuang (Typhae Pollen, 1 liang), Shuiyin (Hydrargyrum, 1 liang).
Usage: Grind into powder and sprinkle on the lesion.
Yet another prescription: Use ten jin of fatty pork, boil water to soak the sore. Replace when cool. It will heal within two or three changes. Alternatively, use ten jin of meat, boiled with several dou of water, allowing easy replacement due to large volume.
Yet another prescription: Treats dan chong (jaundice worms) eroding the lower body and five zang organs. Take thirty-seven branches of peach tree from the southeast direction. Gently beat the branches until loose, tie with cotton thread. Grind realgar (Shiliuhuang, Realgar) powder. Bind the cotton-wrapped branch, twist to concentrate the powder slightly. Cut a short bamboo tube, insert it into the lower part, heat the medicine-coated peach branch, and twist/fumigate.
Yet another prescription: Toad feces and rabbit feces, equal parts ground into powder, applied to sores is effective.
