[Dai Yuanli] Postpartum irregular menstruation is caused by the injury to Qi and Blood during childbirth, where the deficiency has not yet recovered, and Wind pathogens along with Cold and Heat pathogenic factors invade the meridians. The alternating effects of Cold and Heat cause Cold to coagulate the blood while Heat dissipates it, resulting in varying amounts of menstrual flow, either occurring before or after the expected time; hence it is termed irregular menstruation.
[Xue Ji] In the aforementioned conditions, if pain occurs after the period is overdue, it indicates a deficiency of both Qi and Blood; treat with Bazhen Tang (Eight Treasures Decoction) added with Chaihu (Bupleuri Radix) and Danpi (Moutan Cortex). If bleeding arrives ahead of schedule, it indicates Blood Heat; treat with Siwu Tang (Four Substances Decoction) added with Shanzhi (Gardeniae Fructus) and Chaihu (Bupleuri Radix). If pain occurs before the onset of menstruation, it indicates Blood Excess (stasis); treat with Siwu Tang (Four Substances Decoction) added with Taoren (Persicae Semen) and Honghua (Carthami Flos). If bleeding arrives after the period is overdue, it indicates Blood Deficiency; treat with Siwu Tang (Four Substances Decoction) added with Renshen (Ginseng Radix) and Baizhu (Atractylodis Macrocephalae Rhizoma). If the blood is dark purple and forms clots, it indicates Blood Heat; treat with Siwu Tang (Four Substances Decoction) added with fried Shanzhi (Gardeniae Fructus), fried Lian (Coptidis Rhizoma), and Danpi (Moutan Cortex). If there is pain accompanied by pale color, it indicates excessive Phlegm-Dampness; treat with Siwu Tang (Four Substances Decoction) combined with Erchen Tang (Two Aged Substances Decoction). Treatment should be adapted to clinical circumstances. A postpartum patient presented with irregular menstruation, internal heat, dry mouth and thirst. After taking cold-natured medicines, the bleeding worsened into a flooding pattern, accompanied by abdominal distension, alternating chills and fever, vomiting, and poor appetite. After administering Liu Junzi Tang (Six Gentlemen Decoction) for over twenty doses, all symptoms resolved, and she was stabilized using Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) for regulation.
