[Collected Experiences] Treatment for metrorrhagia (uterine bleeding) and leukorrhea with blood staining. Select Zhongji (CV3) using the tonification method, Zigong (EX-CA1) needled at a depth of two and a half cun. If bloody stasis does not cease, select Sanyinjiao (SP6), Bailao (EX-HN2), Fengmen (BL12), Zhongji (CV3), Shenshu (BL23), Gao Huang (BL43), Quchi (LI11), and Juegu (GB39).
[Jia Yi Jing] For women who are infertile, experiencing sudden profuse vaginal discharge, and menstrual spotting, Ran Gu (KI2) is indicated. For women with metrorrhagia, abdominal distension and fullness causing dyspnea, and yellow urine, Yin Gu (KI10) is indicated. (*Qian Jin Fang* records: for metrorrhagia, lower abdominal fullness as if obstructed, alternating chills and fever, and unilateral abdominal swelling.) For women with amenorrhea or obstruction of menstrual blood, Hui Yin (CV1) is indicated. For women with metrorrhagia, Tai Chong (LR3) is indicated.
[Pulse Diagnosis] Question: What are the categories of the Five Metrorrhagias? The Master said: White metrorrhagia resembles mucus; red metrorrhagia resembles dark red fluid; yellow metrorrhagia resembles rotten melon; green metrorrhagia resembles blue liquid; black metrorrhagia resembles coagulated blood. At the Cun Kou pulse position, if the pulse is wiry (Xian) and large, wiry indicates reduction [of yang], and large indicates hollow like a leek stalk (Kou). Reduction implies cold, and hollow implies deficiency. When cold and deficiency interact, this is named Ge pulse. In women, this leads to spontaneous abortion and metrorrhagia. (Also seen in various hemorrhagic conditions.)
[Zhang Zhongjing] At the Cun Kou pulse position, if the pulse is faint (Wei) and slow (Huan), faint indicates sparse Wei Qi (Defensive Qi), and sparse means the skin pores are open; slow indicates weak Stomach Qi lacking solidity, leading to improper digestion of food and transformation of water. When grain enters the Stomach, the pulse channels circulate; when water enters the meridians, blood is formed. If Ying Qi (Nutritive Qi) becomes excessive, the skin pores will inevitably be loose, the San Jiao (Triple Burner) meridians become severed, which is named metrorrhagia.
[Pulse Diagnosis] When diagnosing women with leukorrhea mixed with blood, if they lose several sheng of blood daily and the pulse is rapid and urgent, death ensues; if the pulse is slow, survival is possible. When diagnosing women with continuous leukorrhea mixed with blood, if the pulse is small, deficient, and slippery, survival is possible; if the pulse is large, tight, solid, and rapid, death ensues.
If the Chi and Cun positions of the pulse are deficient, it indicates metrorrhagia; if the pulse is floating, it is generally untreatable. (However, if the lower body is strongly fortified, recovery is possible.)
[Qian Jin Fang] The Chong Mai (Penetrating Vessel) and Ren Mai (Conception Vessel) in women are the Sea of the Twelve Meridians. They circulate externally along the meridians and nourish the internal Zang-Fu organs internally. If Yin and Yang are in harmony, menstruation occurs on schedule. If labor and injury cause a loss of control, sudden profuse bleeding may occur, potentially leading to syncope and confusion. If the Cun position pulse is faint and slow, it indicates cold in the Upper Jiao, leading to hematemesis or epistaxis. If the Chi position pulse is faint and slow, it indicates cold in the Lower Jiao, leading to metrorrhagia or hematochezia. Generally, a small and rapid pulse is favorable, while a洪大 (surging and large) pulse is unfavorable. The general principle is to primarily regulate and tonify the Spleen and Stomach.
[Qian Jin Fang] When a woman suffers from metrorrhagia accompanied by severe heart pain, it is called "Killing Blood Heart Pain," caused by deficiency of Heart and Spleen blood. This also applies if severe heart pain follows excessive blood loss after a miscarriage. Use Sepiae Endoconcha (Hai Piao Xiao, 海螵蛸) stir-fried into powder, taken with vinegar-infused water. Shi Yao San (Pain-Stopping Powder) is also effective.
[Xue Ji] For the aforementioned conditions, if Yin blood is depleted and scattered, use Hai Piao Wan (Sepia Pill) to astringe and stop bleeding. If bloody stasis does not dissipate, use Shi Yao San (Pain-Stopping Powder) to invigorate blood and dissipate stasis. If Heart blood is deficient, use Xiong Gui Tang (Ligusticum and Angelica Decoction) to tonify and nourish. If emotional constraint damages blood, use Gui Pi Tang (Spleen-Restoring Decoction) to regulate and tonify. A woman suffered from metrorrhagia combined with heart pain for three years without response to various medicines. Whenever the pain was severe, all deficiency symptoms manifested, and her complexion was sallow and withered. I stated: The Heart governs blood. It is likely that due to excessive blood loss, the Heart lacks nourishment, causing pain. It is appropriate to use Shi Quan Da Bu Tang (Comprehensive Great Tonifying Decoction), doubling the dosage of Renshen (Ginseng Radix) and Baizhu (Atractylodis Macrocephalae Rhizoma). After more than thirty doses, she improved slightly; after more than one hundred doses, she fully recovered.
