(Applied to Jue-Ni / Cold Extremities)
[From *Jin Gui Yao Lue*] Question: There are three diseases in women shortly after childbirth: one is Jing disease (spasm), the second is Yu Mao (depression and dizziness), and the third is difficult defecation. What does this mean? The Master said: After new childbirth, blood deficiency occurs, with excessive sweating and a predisposition to wind-stroke, thus causing Jing disease (spasm). Loss of blood followed by sweating leads to excess cold, thus causing Yu Mao (depression and dizziness). Depletion of body fluids causes dryness in the Stomach, thus leading to difficult defecation. Note: Yu Mao in postpartum women is what is currently known as Xue Yun (blood syncope).
[From *Da Quan*] Postpartum Xue Yun (blood syncope) is caused by敗血 (retained blood/toxic blood) flowing into the Liver Meridian, resulting in blacking out of vision, floaters, spinning dizziness of the head and eyes, inability to rise or sit, and even coma and loss of consciousness, termed Xue Yun (blood syncope). It is best treated with Hei Shen San (Black Divine Powder) mixed with fine wine. Quack doctors sometimes treat it as An Feng (dark wind) or Zhong Feng (stroke). All fainting is due to blood-heat riding on deficiency to rush upward to the Heart, causing coma and loss of consciousness, with Qi blockage nearing extinction. However, there are three causes: fainting due to excessive mental/physical exertion, fainting due to excessive bleeding, and fainting due to insufficient bleeding. Although the symptoms of fainting are the same, the treatments differ, so they must be carefully distinguished. For fainting due to excessive bleeding, there is only coma, oppression, irritability, and breast distension; treat with herbs that tonify blood and clear the Heart. For fainting due to insufficient bleeding, it is because lochia is not discharged, rushing upward to harass the Heart, causing fullness and urgency below the heart, coma, locked jaw, and complete loss of consciousness; treat with herbs that break stasis and promote blood circulation. Ancient methods state: Immediately after delivery, pre-burn a weight or yellow stone, heat charcoal until red-hot, place in a vessel, and urgently pour vinegar over it near the bed; the vinegar vapor can remove blood syncope. It is best to use this occasionally within one month postpartum. Cui Shi said: All fainting is due to deficient heat, with Qi and Blood rushing to the empty space in the abdomen. For those about to deliver, first apply strong vinegar to the mouth and nose, keep vinegar nearby for inhalation, and drink it slowly; this is the superior method. If one feels faint, immediately apply vinegar to the face; upon recovery, drink vinegar, and give a little water to resolve it. (Another version says, give a little water to resolve it.) Another method is to burn Gan Qi (dry lacquer) to produce thick smoke and fumigate the parturient's face to awaken her. (If Gan Qi is unavailable, burning an old lacquer vessel to produce smoke is also excellent.)
Guo Jizhong discussed: What is postpartum Xue Yun (blood syncope)? Answer: Postpartum Qi and Blood suddenly become deficient. Without rest, Blood follows Qi upward, confusing the Spirit of the Heart, causing visual spots. In extreme cases, it causes coma and loss of consciousness, locked jaw, spirit confusion, and cold Qi. If physicians do not recognize this and call it An Feng (dark wind), treating it as such will make the disease hard to cure. Simply taking Qing Hun San (Clear Soul Powder) will lead to recovery.
[Xue Ji] Postpartum Yuan Qi (Original Qi) is damaged, and E Lu (lochia) attacks upward riding on the deficiency, causing blurred vision, dizziness, chest oppression, coma, locked jaw, or phlegm obstruction. Urgently use Shi Xiao San (Pain-Relieving Powder). If fainting occurs due to excessive bleeding, or if there is coma and irritability, use a large dose of Xiong Gui Tang (Ligusticum and Angelica Decoction) to tonify, or Yun Tai Zi San (Rapeseed Seed Powder), or Tong Zi Xiao Bian (Child's Urine). Add Er Chen Tang (Two-Cured Decoction) if there is phlegm. If caused by mental/physical labor, use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) with Xiang Fu (Cyperus Rhizome). If unconscious due to extreme Qi and Blood deficiency, use Qing Hun San (Clear Soul Powder), followed by Xiong Gui Tang (Ligusticum and Angelica Decoction) and other formulas to greatly tonify Qi and Blood. For all deliveries, vinegar or lacquer vessel fumigation can be used, or Ban Xia (Pinellia Rhizome) powder mixed with cold water into pills inserted into the nostrils; this prevents the aforementioned ailments. Dan Xi Mr. said: Xue Yun (blood syncope) is due to both Qi and Blood deficiency, with Phlegm-Fire surging upward. Use Er Chen Tang (Two-Cured Decoction) to guide out phlegm, or Jia Jian Zhu Sha An Shen Wan (Modified Cinnabar Calming the Spirit Pill), taken with Mai Men Dong Tang (Ophiopogon Decoction). Generally, postpartum conditions like facial deviation should primarily be treated by greatly tonifying Qi and Blood, while simultaneously treating phlegm. If Spleen and Stomach deficiency fails to consolidate, use Liu Jun Zi Tang (Six-Gentlemen Decoction). At five to seven months, take An Tai Yin (Fetus-Calming Infusion). At eight to nine months, add Da Fu Pi (Areca Peel) and Huang Yang Nao (Buxus Fruit). Near delivery, take Bao Sheng Wu You San (Protect Life Without Worry Powder) to prevent prior ailments. A family member had lower abdominal pain after childbirth, sudden locking of the jaw; administered Shi Xiao San (Pain-Relieving Powder), recovered after a long time, then cured with Si Wu Tang (Four Substances Decoction) plus Pao Jiang (Roasted Ginger), Bai Zhu (Atractylodes Rhizome), and Chen Pi (Tangerine Peel). One parturient had numbness in both hands; after taking Yu Feng Dan (Wind-Dispelling Pill) and Tian Ma Wan (Gastrodia Pill), the whole body became numb, with mental fatigue, afternoon fever and thirst, spontaneous sweating, and night sweats; this was deficiency of both Qi and Blood. Used Shi Quan Da Bu Tang (Ten Complete Great Tonifying Decoction) plus Pao Jiang (Roasted Ginger) for several doses; all symptoms receded. Removed Pao Jiang, continued for several more doses, and recovered. Only internal heat remained, indicating Blood deficiency; cured with Xiao Yao San (Free and Easy Wanderer Powder). A woman at eight months felt fetal descent or movement, with pale face, yellow complexion, fatigue, and poor appetite; this was Spleen Qi deficiency. Used Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) with doubled Bai Zhu (Atractylodes Rhizome) and Su Geng (Perilla Stem) for over thirty doses, achieving stability. Postpartum dizziness, chest fullness, and cough were cured with Si Wu Tang (Four Substances Decoction) plus Fu Ling (Poria), Ban Xia (Pinellia Rhizome), and Jie Geng (Platycodon Root). One parturient drank alcohol during childbirth, had excessive lochia, suffered from Xue Yun (blood syncope), and exhaled alcohol odor; this was Blood moving recklessly due to alcohol-heat, causing fainting due to deficiency. Used Fo Shou San (Buddha's Hand Powder) plus Gan Ge (Kudzu Root) 2 qian; cured in one dose. Alcohol is fierce; it should not be consumed during pregnancy or postpartum to avoid such conditions. Crowded delivery rooms with noise and heat can also cause this condition.
Dan Xi treated a woman with a pale face and long stature, who was mentally depressed. She gave birth at midnight and fainted at dawn. Urgently moxa Qi Hai (CV6) 15 cones, and she recovered. Later, with Ren Shen (Ginseng) and Bai Zhu (Atractylodes Rhizome) etc., she took medicine for two months and stabilized. This was Yang deficiency.
Lu Yan from Fenghua treated Mrs. Xu from Xinchang who died suddenly after childbirth, with only slight warmth in the chest and diaphragm. Lu diagnosed: This is Blood Stasis. Used dozens of jin of Hong Hua (Safflower), boiled in a large pot. When boiling, placed it in a wooden bucket, laid the patient on top to fumigate, adding more hot liquid as needed. Soon, the woman's fingers moved, and she recovered after half a day. This aligns with Xu Yinzong's treatment of Empress Dowager Wang.

Zhong Jing said: Postpartum Yu Mao (depression/dizziness), pulse is weak and thin, unable to eat, stool is hard, but only head sweats. The reason is Blood deficiency leading to Jue (cold extremities/reversal); reversal leads to Mao (dizziness). For Mao to resolve, there must be profuse sweating. Due to Blood deficiency and lower reversal, solitary Yang rises outward, hence head sweat. Therefore, postpartum women welcome sweating; Yin blood is lost and deficient, Yang Qi stands alone and abundant, so sweating occurs, restoring Yin and Yang. Hard stool, vomiting, inability to eat: Xiao Chai Hu Tang (Minor Bupleurum Decoction) governs. If disease resolves and eating resumes, but fever returns after 7-8 days, this is Stomach Excess; Da Cheng Qi Tang (Major Order the Qi Decoction) governs. Now note: Yu Mao (depression/dizziness) is synonymous with Yun (fainting). Observing this, postpartum Xue Yun (blood syncope) has three methods: Sweating, Purging, and Harmonizing. One must distinguish exterior/interior and deficiency/excess precisely.
Zhang Shi Formula treats postpartum Xue Yun (blood syncope) with total loss of consciousness, extremely critical.
Cut Chive (Jiu Cai) into a porcelain bottle with a spout, heat it, pour vinegar over it, tightly seal the bottle mouth. Point the spout toward the parturient's nostril to allow vinegar vapor to enter. Must first help the patient sit up. Evil blood rushes to the Heart, causing this condition. Chive removes stagnant blood in the Heart; combined with vinegar vapor to circulate, it is always effective.
Another formula: Wash Ban Xia (Pinellia Rhizome), amount unspecified, grind to powder, form pills like soybeans, insert into nostrils; patient revives. Also treats Five Extinctions.

One woman ate over twenty bowls of tea porridge daily after childbirth. After one month, several patches of her body were ice-cold. Pressing these cold spots with a finger caused coldness to rise from the finger to the Heart. This lasted two years; various treatments failed. Used Ba Wu Tang (Eight Substances Decoction) removing Di Huang (Rehmannia Root), adding Ju Hong (Tangerine Red Pith), Jiang Zhi (Ginger Juice), and Zhu Li (Bamboo Sap) [one wine cup]. Ten doses resulted in warmth.
