Commentary by Zhang Huan: The lung qi of infants connects with the nose. Qi belongs to Yang. If Qi is invaded by wind-cold and stagnates in the nasal cavity, it results in nasal congestion (Bi Sai). Cold Qi prevents the retention of body fluids, leading to excessive rhinorrhea. If cold qi lingers for a long time, purulent mucus accumulates, causing anosmia (loss of smell), resulting in Bi Zhi (nasal obstruction). If accompanied by heat, the nose becomes dry. All these conditions interfere with breastfeeding and eating. [Xue Ji] cites Chao Yuanfang: The nose is the orifice of the Lung; skin, hair, and pores are governed by the Lung. Nasal congestion due to wind-pathogen lodging in the Lung should be treated with Xiaofeng San (Wind-Expelling Powder). Alternatively, use Qiongbai (Scallion White) seven stalks, mixed with a small amount of greasy flour, ground and spread on silk fabric. Warm it in the palm and apply to the Fontanelle (Ximen). For epistaxis caused by fright and falling, leading to scattered Qi and uncontrolled blood flow, use Yigong San (Wonderful Function Powder) added with Chaihu (Bupleuri Radix) and Shanzhi (Gardeniae Fructus). If the left cheek is blueish-green with redness, first use Chaihu Qinggan San (Bupleurum Liver-Clearing Powder), followed by Dihuang Wan (Rehmannia Pill). Redness on the upper face indicates Excess Heat in the Lung and Large Intestine; treat with Xiebai San (Drain White Powder). Redness of the nose indicates Excess Heat in the Spleen and Stomach; treat with Xiehuang San (Drain Yellow Powder). Slight redness indicates Deficiency Heat in the Spleen Meridian; treat with Yigong San added with Shengma (Cimicifugae Rhizoma) and Chaihu. Deep yellow color indicates treatment with Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) from *Jisheng Fang*, followed by Yangshi Dihuang San (Yang’s Rehmannia Powder). Pale white color indicates treatment with Liu Junzi Tang (Six Gentlemen Decoction). Redness at the chin indicates treatment with Siwu Tang (Four Substances Decoction) added with Shanzhi; if very red, use Wulin San (Five Dampnesses Powder). Red urine indicates treatment with Liuwei Wan (Six Flavor Pill) and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). White lips indicate treatment with Liu Junzi Tang. If chronic and unhealed, use Maidong Yinzi (Ophiopogon Drink). If in the initial stage, Yuanqi (Primal Qi) is not depleted, feeding is normal, fever or high fever is present, bowel movements and urination are constipated, thirst with desire to drink water occurs, and eyes do not protrude during sleep, all belong to Excess patterns of both Form and Disease; treat by expelling pathogenic factors. If the disease has persisted, Yuanqi is depleted, appetite is poor with fever, mouth is dry with desire for hot drinks, vomiting and diarrhea occur, limbs feel cold, and eyes protrude during sleep, all belong to Deficiency patterns of both Form and Disease; tonifying Zhengqi (Upright Qi) is essential.
