[Xue Ji] Bian Yong (abscess in the genital/perineal region) is caused by Liver Fire, Gan Gan (liver-related parasitic or wasting disease), or congenital Heat-Toxin in the Liver Meridian. If at the initial stage the swelling is hard and painful, first administer Long Dan Xie Gan Tang (Gentian Decoction to Drain the Liver) for one or two doses. If the swelling and pain do not subside, use Xian Fang Huo Ming Yin (Immortal Life-Rescuing Powder) for two doses. If after five to seven days there is no improvement and the swelling remains hard, also use the aforementioned two medicines, one dose each. If the swelling does not resolve, or if pain increases indicating suppuration is about to occur, use one dose of Xian Fang Huo Ming Yin, followed by Tuo Li Xiao Du San (Supporting the Interior and Detoxifying Powder) with added Chai Hu (Bupleuri Radix) and Shan Zhi (Gardeniae Fructus), for one or two doses. If pus has formed but has not ulcerated, this indicates deficiency of Blood and Qi; use Tuo Li Xiao Du San for one or two doses. If the abscess has ulcerated but pain persists, this indicates that the Toxin-Wa (pathogenic factors) has not been resolved; use one dose of Xian Fang Huo Ming Yin. If pain occurs after pus discharge, this indicates deficiency of Blood and Qi; use Nei Bu Huang Qi Tang (Internal Nourishing Astragalus Decoction). If fever and irritability occur after ulceration, this indicates Qi deficiency and Blood depletion; use Dang Gui Bu Xue Tang (Astragalus and Angelica Decoction for Tonifying Blood). If chills and fever occur after ulceration, this indicates deficiency of both Blood and Qi; use Shi Quan Da Bu Tang (Great Tonifying Decoction of Ten Ingredients). If only chills occur after ulceration, this indicates deficiency of Yuan Qi (Original Qi); use Bu Zhong Yi Qi Tang (Decoction to Supplement the Middle and Augment the Qi). If granulation tissue fails to grow after ulceration, this indicates Spleen Qi deficiency; use Liu Jun Zi Tang (Six Gentlemen Decoction). For those with innate frailty, or who suffer from illness due to dietary irregularities and overwork, Bu Zhong Yi Qi Tang with She Gan (Belamcandae Rhizoma) can often cause spontaneous resolution. If one fails to distinguish meridians and does not differentiate between excess and deficiency, applying indiscriminate attacking methods, thereby damaging Qi and Blood, then mild cases will be difficult to treat, and severe cases will inevitably transform into Zhai Zheng (consumptive disease/tuberculosis-like syndrome), potentially leading to death.
