New-onset Gan disease is called Heat Gan, with symptoms of yellowish complexion and red cheeks, bone heat and night sweats, dry nose and foul breath, cracked lips, irritability and thirst, mental restlessness, palpitations, and low mood. If Gan disease persists for a long time, it is called Cold Gan, characterized by swollen eyelids, abdominal distension, irregular bowel movements, greasy stools or those resembling oil droplets, accompanied by irritability, thirst, yellowish skin, and emaciation. Heat Gan typically involves the exterior, while Cold Gan involves the interior. There are also cases where cold and heat syndromes interact, neither new nor old, caused by factors that are neither internal nor external. Muxiang Wan (Bark of Aucklandia Decoction Pill) treats Cold Gan.
Herbs: Xiangmu (Aucklandiae Radix), Qingdai (Indigo Naturalis, ground separately), Binglang (Arecae Semen), Roudoukou (Myristicae Semen, peeled; each 1 fen), Shexiang (Moschus, ground separately, 1.5 qian), Xusuisi (Euphorbiae Pekinensis Radix, 1.5 liang, stir-fried), Hamo (Bufo bufo bufo, three toads, tied with string, dried, roasted until carbonized).
Preparation and Administration: Grind the above herbs into fine powder, mix with honey to form pills the size of mung beans. Take 3-5 pills up to 10-20 pills per dose, washed down with Bohe Tang (Mentha Decoction), before meals.
Mr. Tang stated: Children with Cold Gan often have excessive thirst, prefer lying on cool ground, are irritable and crying, refuse food, and gradually become emaciated. The signs are difficult to clarify, resembling Heat syndrome, but with slippery diarrhea unresponsive to various medicines. A woman tried all medicines without success, but was finally cured with Qian Yi's Muxiang Wan after just a few doses. Since then, this formula has been effective for such cases.
Huhuanglian Wan (Rhizoma Coptidis and Rhizoma Picrorhizae Pill) treats Heat Gan.
Herbs: Huhuanglian (Picrorhizae Rhizoma), Huanglian (Coptidis Rhizoma; each 0.5 liang), Zhusha (Cinnabar, 1 fen).
Preparation and Administration: Grind the first two herbs into fine powder, insert the Cinnabar powder into pig gallbladders, suspend the gallbladders in light grain water in a clay pot, and boil for the duration of one meal. Remove, add Lulü (Aloe) and Shexiang (Moschus, each 1 fen), mix with rice flour to form pills the size of hemp seeds. Take 5-7 pills up to 10-20 pills per dose, washed down with Mi Yin (Rice Drink), after meals. Another prescription adds Hamo (Bufo bufo bufo, 0.5 liang) without roasting.
Rusheng Wan (Like-Sage Pill) treats Cold-Heat Gan (see "Gan Diarrhea" section).
Chao Yuanfang stated: Children with yellowish faces, straight hair, frequent high fever, inability of food to nourish muscles and flesh, who eventually die after prolonged suffering, are said to have Wugu Gan (Innocent Gan). Legend says there is a bird in heaven named Wugu, which hides by day and roams by night, washing children's clothes and mats. After dew falls overnight, the bird flies over, takes these clothes for children to wear and mats for them to sleep on, causing this disease. *Taiping Shenghui Fang* records that children with Wugu Gan have a lump behind the head like a bean, which moves downward when pressed. This is similar to the forbidden worm under the tongue of birds and beasts. If not removed quickly, it will endanger life. Initially soft and painless, containing worms like rice flour, these grow larger with heat, eventually binding tendons. Once bound, the worms disperse with Qi and Blood, multiplying and eroding Zang-Fu organs, causing sores on muscles, or bloody purulent diarrhea, leading to gradual emaciation, dry upright hair, weak limbs, and premature death. Wugu Gan Li (Dysentery) presents with large abdomen, bloody purulent diarrhea, withered hair and skin, daily emaciation, deficient intestines and stomach, and irregular dysentery. Mr. Wang from Handong stated that the ancient belief about the Wugu bird urinating on clothes causing illness is incorrect. This condition is actually caused by Eight Evils: hunger, fullness, fatigue, labor, wind, fright, summerheat, and accumulation. Prolonged exposure leads to emaciation, failure to gain muscle from food, night crying, slight fever, thirst, lack of appetite control, or skin sores. Zhu's *Eight Piece Brocade Song* states: Child's Wugu Qi is often due to maternal misconduct. If cared for carefully, emaciation is avoided. Fright and thinness cause Wind diseases; warmth and cold cause Qi flaccidity. Must remove sick milk, do not dress too warmly; do not feed too early, speaking late is not feared. There are eight prohibitions in theory: chest strength is weak, scalp is shiny, hair is sparse. Liver congestion invades eyes; Spleen yellow enters limbs. Whole body has rashes, covered in sores. Dysentery does not cease, aversion to cold is frequent. Cannot look at embroidered balls, cannot ride bamboo horses. White halos appear in eyes, clear saliva drips from mouth. Grinding teeth requires biting nails, raising hands to comb eyebrows. Nightly eating fruits and melons, morning eating soil and mud. Stomach injury causes intestinal bloating, Lung excess causes wheezing. Food intake never stops, sleeping deeply like drunken confusion.
Fei'er Wan (Flesh-Nourishing Pill) treats Wugu Gan in children, where a lump like a bean appears behind the neck, movable upon pressure, soft and painless. Long-term use is miraculous.
Herbs: Huhuanglian (Picrorhizae Rhizoma), Shenqu (Massa Medicata Fermentata, stir-fried), Mai Fu (Wheat Bran; each 5 qian), Binglang (Arecae Semen, 3 qian), Muxiang (Aucklandiae Radix, 2 qian), Roudoukou (Myristicae Semen, baked in flour), Shijunzi Rou (Quisqualis Semen, 2.5 qian each).
