The Stomach is the sea of water and grain, connecting upwards to the pharynx. If symptoms of fever accompanied by vomiting and regurgitation appear in the initial stage, this is due to Fire pathogenic factors invading the Stomach, causing Qi to rebel upwards; treatment is relatively easy. However, if these symptoms persist from the onset of the rash until it scabs over, it indicates that the toxin of the smallpox/mumps (chuang) has gathered at the gate of the pharynx and attacks the epigastrium, leading to difficulty in swallowing, making treatment difficult. Due to a lack of knowledge on how to preemptively resolve pharyngeal toxicity, the condition gradually evolves into aphonia and choking on water, becoming incurable.
Chu Yushi stated: For vomiting and regurgitation caused by chuang (smallpox/rash diseases), if there is no phlegm, Yi Huang San (Spleen) should be used; if there is phlegm, Er Chen Tang (Phlegm-Fluid Retention in General Practice) or Ju Pi Ban Xia Tang should be used. If vomiting does not cease, add Ding Xiang (Clove). This is suitable for those with Stomach Cold; for those with Stomach Heat, Lu Gen (Reed Rhizome), Mao Gen (Imperata Rhizome), Pi Pa Ye (Loquat Leaf), and Huang Lian (Coptis Rhizome) are suitable. If both vomiting and diarrhea occur, Yi Huang San, Chen Shi Mu Xiang San (Chen's Costus Powder), or Yi Gong San are also appropriate.
If vomiting is accompanied by fever, irritability, thirst, abdominal distension, panting, obstructed urination and defecation, and a red face, promoting urination is indicated. If the condition does not improve, Xuan Feng San should be used to purge (for fever during chuang). Xue Ji noted: Regarding the aforementioned syndromes, if the hands and feet are cold, there is a desire to drink hot soup, or abdominal pain occurs, this indicates deficiency-cold of the Middle Qi, and Yi Huang San is suitable. If the hands and feet are not cold, but there is vomiting, regurgitation, and phlegm-saliva, this indicates weakness of the Middle Qi, and Ju Pi Ban Xia Tang is suitable. If both hands and feet are hot, this indicates stagnation of Heat-Toxin, and Dao Chi San is suitable. If there is dry mouth and an inability to finish nursing, this indicates Qi-Heat in the Stomach Meridian, and Zhu Ru Tang is suitable. If vomiting and regurgitation prevent nursing, or if sour, foul-smelling vomit is expelled, this indicates Spleen Qi deficiency with stagnation of milk and food, and Zhi Zhu Wan is suitable. If sudden, continuous severe vomiting occurs at the initial stage of fever, this indicates rebellious Fire-Qi, and Zhu Lian San is the primary formula.

If vomiting is caused by excessive drinking of water, this is "Water Rebellion" (Shui Ni), treated with Wu Ling San.
If vomiting is caused by food injury, Ding Xiang Pi Ji Wan should be used to mildly promote bowel movement.
If there are no the above symptoms but only dry heaving or hiccups, humans rely on Stomach Qi as the root. The Stomach belongs to Earth. When Earth fails, Wood insults it. Now, Wood carries the momentum of Ministerial Fire, attacking the Stomach from above. Its Qi rises directly from below the umbilicus through the clear pathways, emerging from the Fen Men (cardia/upper opening of the stomach). Mild cases present as dry heaving; severe cases present as hiccups. This is a sign of Earth's failure. *Tai Su* states: "When a string breaks, its sound becomes hoarse and ruined; when a tree ages, its leaves fall; when disease is deep, hiccups occur. At this point, short needles are ineffective, and toxic drugs cannot attack; this is termed untreatable."
