[Xue Ji] Zhang Zhongjing said: When fire enters the Lung, it causes Fan (restlessness); when it enters the Kidney, it causes Zao (agitation). The Heart belongs to Sovereign Fire. When Fire is excessive, Metal (Lung) is scorched and Water (Kidney) is depleted, leaving Fire alone. Thus, Lung and Kidney combined manifest as Zao. The *Huoren* book states: If there is only heat and restlessness, it is Xu Fan (deficiency restlessness). Various deficiency-related heat and restlessness resemble Shanghan (Cold Damage), but without aversion to cold and body ache, so it is known not to be Shanghan; without headache and tight pulse, so it is known not to be Interior Excess. Do not use sweating or purging methods. Instead, give Zhuye Tang (Bamboo Leaf Decoction); if accompanied by vomiting, give Jupi Tang (Tangerine Peel Decoction). Furthermore, Heart Qi deficiency leads to Fan first and then thirst, accompanied by slight fever and a floating, tight, and large pulse. Generally, Fan refers to internal mental restlessness and heat, thus belonging to Yang. Zao refers to physical agitation or nakedness with a desire to enter a well, representing external heat, thus belonging to Yin. External heat is rootless fire, hence it is deficiency. In children, one must distinguish between sitting/lying uneasily as Fan, and erratic movement as Zao. "Jian" (here referring to a state of anxious discomfort) indicates Heat in the Heart Meridian, causing spiritual trance, mental fullness, and fright. "Zao" indicates Wind in the Heart Meridian, causing irritability and convulsions. For severe heat, use Huanglian Jiedu Tang (Coptis Decoction for Detoxification); for mild cases, use Daochi San (Guide Out Red Powder); for Wind-Heat, use Zhibao Dan (Precious Treasure Pill); for rapid and forceful pulse with constipation and heat, use Shenxiong Wan (Shenxiong Pill). These are all treatments for Excess-Heat syndromes. If there is irritability, headache, shortness of breath, dry mouth and throat without thirst, it is Deficiency; use Sijun Zi Tang (Four Gentlemen Decoction) plus Chuanxiong and Danggui. If thirst arises due to harsh medicinal attack, use Zhuru Tang (Bamboo Shavings Decoction). If irritability prevents sleep, use Suanzaoren Tang (Sour Jujube Seed Decoction). If spirit is disordered, with heat and urge to vomit, use Zhusha Anshen Wan (Cinnabar Calming Spirit Pill). If the face appears flushed like Yang floating outward, eyes are red inside, and six pulses are flooding but empty upon deep pressure, it is Zao caused by Blood Deficiency; use Danggui Buxue Tang (Angelica Tonifying Blood Decoction). If there is agitation, nakedness, and a desire to enter a well, with sinking-thin or floating-large pulses that feel empty upon pressure, this is Zao caused by Yin Excess repelling Yang; use Shenfu Tang (Ginseng and Aconite Decoction), which has the merit of restoring life.
