Qi rebellion occurs in all five zang organs. The first case involves the Heart and Small Intestine. The Classics state: "All rebellious qi rushing upward belongs to Fire." It also states: "When the lower abdomen pulls into the testicles, draws upon the lumbar spine, and qi rushes upward to the heart, the pathogen is in the Small Intestine. Treat by selecting the Yuan-Source point of the Sanjiao (Huangyuan) to disperse it, needle the Taiyang channel to supplement it, select Jueyin points to descend it, select Shangjuxu and Xiajuxu to remove it, and regulate according to the channels traversed." This is the correct method. The second case involves the Stomach. The Classics state: "Stomach qi rebellion is this condition." Stomach qi rebellion manifests as retching, vomiting, or hiccups; thus, for treating vomiting, one selects Zhongwan (CV12) and Zusanli (ST36). For treating hiccups, one selects the end of the black root under the nipple (referring to Ronggu ST17 area) and Guanyuan (CV4), which are highly effective. Specific treatments are found in their respective sections. The third case involves the Lung and Large Intestine. The Classics state: "The Lung dislikes qi rebelling upward; quickly consume bitter foods to drain it." It also states: "Frequent borborygmus in the abdomen, qi rushing upward to the chest, wheezing inability to stand long, the pathogen is in the Large Intestine. Needle the Yuan-Source of the Sanjiao (Shenque/CV8 contextually implied or specific back-shu/front-mu relation), select Juxu and Xiajuxu." For lung qi rebellion causing cough and wheezing, select Tiantu (CV22) and Renying (ST9) for draining treatment. Treatments are also in their respective sections. The fourth case involves the Liver. The Classics state: "Liver qi rebellion causes deafness and lack of clarity, swelling; treat by selecting blood points." The fifth case involves the Kidney. This refers to conditions like Ben Tun (Running Piglet) qi rebellion and Jiaoqi (Beriberi) rushing to the heart. Treatments are in their respective sections. The sixth case again mentions the Kidney (actually referring to the Du Mai/Kidney system). The Classics state: "Disease from the collateral branch of the Du Mai arises, with qi rushing from the lower abdomen to the heart and pain, inability to pass stool or urine, this is called Chong Hernia."
There are three types of Yunqi (Five Movements and Six Qi) Jue Ni (Rebellious Qi): First is Wind-Fire. The Classics state: "When Shaoyang governs the Heaven, Fire qi descends and overlooks, Wind moves on the Earth, causing Jue Ni and obstruction at the Diaphragm." Second is Cold. The Classics state: "When Water depression erupts, frequent Jue Ni occurs." Third is Dampness. The Classics state: "When Taiyin governs the Heaven, Damp qi descends and overlooks, causing Jue Ni."
The Classics state: "The manifestation of rebellious qi resembles Yang patterns." Whenever qi rebels upward, Yang-like symptoms must appear, such as a red face and surging pulse. One must use appropriate methods to descend the rebellious qi for recovery. If one uses cold-natured herbs to attack based on the appearance of Yang excess, it will be incurable. When qi rushes upward to the heart and throat, obstructing respiration, the treatment method is seen in the Shaoyin/Jueyin disease sections of Shanghan Lun. Li Dongyuan stated: "In autumn and winter months, the four pathways of the Stomach pulse are rebelled against by the Chong Mai, merging with the two pathways of the Shaoyang pulse under the hypochondrium and reversing upward. This disease is named Jue Ni." The Classics say: "Rebellious qi moves upward, filling the pulses, wasting the form, causing the Seven Spirits to become confused and severed, leaving the form and dying." Symptoms include qi rushing to the throat, obstructing breath, audible wheezing, and inability to lie down. Add Wu Zhu Yu (Evodiae Fructus) half to one qian to Tiao Zhong Yi Qi Tang, adjusting dosage based on the degree of rebellious qi. If this symptom occurs in summer, it indicates severe Heat. Since this condition varies in cold, heat, warmth, and coolness with the seasons, it is suitable to use equal parts of Wine-processed Huang Lian (Coptidis Rhizoma), Wine-processed Huang Bai (Phellodendri Cortex), and Wine-processed Zhi Mu (Anemarrhenae Rhizoma), grind into fine powder, make pills with warm water, size of Sophora seeds. Take 200 pills each time, swallowed with plain warm water on an empty stomach. Drink plenty of water afterward. Shortly after taking the medicine, consume delicious food to suppress the medication, preventing it from lingering in the Stomach, ensuring it reaches the Lower Jiao to drain the pathogen from the Chong Mai. Dai Fu'an stated: "For deficiency fire syndromes, where Yin and Yang ascending/descending are dysfunctional, with deficiency below and excess above, causing shortness of breath and wheezing, Su Zi Jiang Qi Tang (Perilla Fruit Descending Qi Decoction) without Qian Hu (Peucedani Radix) is suitable, taken with Hei Xi Dan (Black Tin Pill) or Yang Zheng Dan (Nourish Righteousness Pill). For severe急促 (rapid) breathing unable to lie down, Chen Fu Tang (Aucklandiae and Aconite Decoction) or Zheng Yuan San (Correct Origin Powder), or Si Zhu San (Four Pillar Powder) replacing Mu Xiang (Aucklandiae Radix) with Chen Xiang (Aquilariae Lignum), decocted with salt, followed by Hei Xi Dan, Ling Sha Dan (Spirit Sand Pill), or San Chao Dan (Triple-Fried Pill). If ineffective, use previous medicines to deliver Zhu Sha Dan (Cinnabar Pill)." (Note: These formulas mostly use aromatic, drying, rigid, and hot agents. If used mistakenly for patients with Yin deficiency and internal heat, danger will ensue immediately. Physicians must exercise caution.)
