[Xue Ji] Duogu Ju (Bone-Exuding Abscess) arises from chronic ulceration of sores, where Qi and Blood fail to nourish the affected area. Pathogenic factors sink and invade; over time, tendons rot and bones decay, eventually shedding and discharging. This condition belongs to deficiency syndromes of the Three Yin meridians of the foot. Treatment should employ Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) to consolidate the root. If accompanied by Yin-fire fever, it should be assisted with Liuwei Wan (Six-Ingredient Pill with Rehmannia), strengthening the master of Water (nourishing Kidney Yin) to suppress excessive Yang heat. In cases of Yang Qi deficiency and cold, it should be assisted with Bawei Wan (Eight-Ingredient Pill with Rehmannia), benefiting the source of Fire (warming Kidney Yang) to dissipate Yin shadows. Externally, Fuzi Bing (Aconite Cake) and Cong Yi Fa (Scallion Moxibustion/Warming Method) are used to dispel cold pathogens and supplement and connect Ying Qi (Nutritive Qi); then the bone will naturally shed and the sore will naturally close. The Kidney governs the bones; if Kidney Qi is deficient, the bones gradually swell, and after a long passage of time, they ulcerate and discharge dead bone. The aforementioned methods should also be used here. If attacking and reducing agents are administered, further injuring the True Qi, few cases will not go awry.
The Scholar Yu Tingqi suffered from Liuzhu (Flowing Abscess) in his leg for over a year, expelling a small amount of necrotic bone. He experienced chills before noon, fever after noon, dry mouth with spitting of phlegm, and frequent urination. I diagnosed this as a deficiency of the Three Yin meridians of the foot. In the morning, he took Buzhong Yiqi Tang; in the evening, he took Liuwei Wan formula added with Huangbai (Phellodendri Cortex), Danggui (Angelicae Sinensis Radix), and Wuweizi (Schisandrae Fructus). After taking each prescription for more than thirty doses, and using Douchi Bing (Fermented Soybean Cake) externally, all symptoms gradually improved. He subsequently took formulas similar to Shiquan Dabu Tang (Perfect Golden Great Tonifying Decoction). Because he was cautious in managing his illness, he recovered.
A Confucian scholar suffered from Fugu Ju (Bone-Adhering Abscess). Due to a lack of timely regulation and tonification, the sore did not heal, discharging a small amount of clear pus daily. Later, three pieces of necrotic bone were frequently discharged. His pulse was rapid but without pathogenic signs, indicating Blood deficiency causing the sore to form a sinus tract that could not heal. He was treated with Wujin Gao (Black Gold Ointment) externally and Shiquan Dabu Tang internally once daily, resulting in recovery.
Shangshe Wang Tingzhang suffered from the aforementioned condition for three years without healing. His limbs were emaciated, food digestion was difficult, hands and feet were cold, and bowel movements were obstructed, with coldness in the Yin aspects of hands and feet. I determined this to be Yang Qi deficiency and cold. He was treated with Buzhong Yiqi Tang and Bawei Wan, along with moxibustion applied to the affected area, leading to recovery.
A man had swelling and hardness in his lower leg for over a year before ulceration. He felt internal heat and thirst, his limbs were emaciated, and his six pulses were surging and large, especially on the left hand. He was treated with Buzhong Yiqi Tang and Liuwei Wan, which expelled a piece of necrotic bone. Continuing the previous medications, all symptoms disappeared, and the sore closed.
A sixteen-year-old man had swelling and dark discoloration in his foot. After ulceration, bone was exposed. He was thin with night sweats, fever, and dry mouth. He was treated with Shiquan Dabu Tang and Liuhuang Dihuang Wan (Six-Ingredient Pill with Rehmannia), taking each for more than fifty doses, resulting in recovery. Otherwise, the condition might have transformed into Zhai (Consumption/Tuberculosis) or led to death due to exhaustion of Qi and Blood.
A man suffered from sprain-induced swelling and pain in his foot starting at age fourteen. He took Liuqi Yin (Flowing Qi Decoction) and applied cold-natured herbs externally, leading to rotting and ulceration. By age sixteen, the sore opened, with diffuse swelling and dark black discoloration on the dorsum of the foot, bone exposure, emaciation, incessant night sweats, fever, and dry mouth and tongue. His innate vitality was lost. He was treated with Shiquan Dabu Tang and Liuhuang Dihuang Wan, taking each for more than fifty doses. His Yuan Qi (Original Qi) gradually recovered, the affected area gradually turned red, and one piece of bone detached. After another fifty doses of each, he finally recovered.
A woman in her thirties, constitutionally weak, had gradual swelling on the back of her left hand. Two years later, it ulcerated, discharging clear pus. The swelling and dark discoloration extended to her arm. She experienced internal heat and afternoon fever, spontaneous sweating, and night sweats, with menstruation occurring only once every two months. This indicated Liver and Spleen Qi and Blood deficiency. In the morning, she took Guipi Tang (Restore the Spleen Decoction); in the evening, she took Xiaoyao San (Free and Easy Wanderer Powder). The swollen area was frequently treated with Cong Yi Fa (Scallion Moxibustion/Warming Method). After more than two months, all symptoms gradually improved. The sore discharged necrotic bone, and she continued the previous medications. A total of more than three hundred doses were taken before full recovery.
