The finger's thumb develops a ding (a type of acute localized infection), initially small like millet grains, gradually enlarging to the size of a bean, or resembling a peach or plum. The color may be green, purple, suddenly yellow, black, or white. Sensations may include itching, numbness without pain, or severe pain penetrating to the heart. This condition is named Diao Ju (Tuning Carbuncle - *Note: likely a specific local name for thumb carbuncle*). It belongs to the accumulation of toxins in the Hand-Taiyin Lung Meridian. In cases of excessive toxin strength, amputation of the affected digit is advisable. If blood and pus can be drained by pricking on the fourth day, the patient will survive; if dark blood is discharged, death ensues. Urgently administer Wu Jin San (Black Gold Powder) and Zi Jin Dan (Purple Gold Pill) to induce sweating. For robust individuals, Yi Li Jin Dan (One-Pill Golden Pill) should be used to purge downwards. If the toxic vapors attack the Heart, accompanied by vomiting, anorexia, and abdominal distension, death occurs. Bleeding from the gum margins also indicates a fatal prognosis.
A certain person suffered from this condition; his skin was black and painless, and the finger had already died. He was advised to have it amputated. He was urged to quickly take tonic medicines, fearing that if the blackness spread up the arm, it would be incurable. He did not believe this and took other anti-toxic drugs instead. His hand eventually turned completely black, and he could not be saved.
When a toxic nodule forms on the middle finger of the hand, presenting with redness, swelling, and pain (or affecting any finger), it is named Tian She Tou (Heavenly Snake Head). If pus ruptures the skin, and the wound edges resemble lips shaped like a snake head, hence the name. This condition is caused by accumulated heat in the Hand-Jueyin Pericardium Meridian. It is suitable to administer Huo Ming Yin (Life-Saving Decoction) added with Chai Hu (Bupleuri Radix), Qiang Huo (Notopterygii Rhizoma et Radix), and Jie Geng (Platycodonis Radix). Alternatively, select from Huang Lian Xiao Du Yin (Coptis Detoxification Decoction), Zi Jin Dan (Purple Gold Pill), or Wu Jin San (Black Gold Powder). Even if the tissue becomes black, stubborn, and numb, leading to ulceration and loss of the finger, death does not necessarily follow.
Zhu Danxi stated: Dandelion herb (Taraxaci Herba), during the Qingming Festival, resembles Shepherd's Purse (Capsellae Bursa-Pastoris Herba) in appearance, with a single flower opening in the center resembling a chrysanthemum. Take dried Dandelion and Xuan Er Cao (Xanthii Herba) in equal proportions, grind into powder, decoct strongly with good vinegar, and soak/wash the area until healed.
Dandelion and Honeysuckle Wine (Yin Chen Hao Jiu variant context implied) is extremely effective for treating Tian She Tou, verified repeatedly. (See section on Breast Abscess).
[Worldly/Popular prescriptions] To treat Tian She Tou, crush fresh Dandelion finely, mix with water, strain, and ingest the liquid. Apply the dregs to the affected area; this has yielded repeated success.
