[General Principles]
Thirst caused by sores and ulcers, regardless of whether it is in the swelling stage or the ulceration stage, if the pulse is rapid, accompanied by fever and thirst, use Zhuye Huangqi Tang (Palm Leaf and Astragalus Decoction). If the pulse is not rapid, without fever, or if the pulse is rapid but forceless with thirst, or merely dry mouth, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If the pulse is rapid with constipation, use Qingliang Yin (Cooling and Clearing Drink). If the radial pulse at the cun position is large and forceful upon deep pressure with thirst, use Jiaban Bawei Wan (Modified Eight-Ingredient Pill with Rehmannia). For treating dry mouth, yellow tongue coating, and incessant drinking, this pill is particularly effective.
[Xue Ji] Thirst caused by sores and ulcers: if there is pain, fever, and normal bowel and bladder movements, it indicates heat in the Upper Jiao; use Zhuye Shigao Tang (Palm Leaf and Gypsum Decoction). If there is swelling, pain, fever, and hard, dry stools, it indicates internal heat in the five viscera; use Sishun Qingliang Yin (Four-Obedience Cooling and Clearing Drink). If the swelling and pain are severe, it indicates accumulation and binding of heat-toxin; use Xianfang Huoming Yin (Immortal Formula for Saving Life Decoction). If there is diffuse swelling with slight pain, it indicates deficiency and stagnation of Qi and Blood; use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If thirst is due to Stomach Fire consuming fluids, resulting in shortness of fluids, use Zhuye Huangqi Tang (Palm Leaf and Astragalus Decoction). If thirst is due to deficient Stomach Qi failing to generate fluids, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If thirst is due to injured Stomach Qi and loss of internal fluids, use Qiwei Baizhu San (Seven-Ingredient Atractylodes Powder). If thirst is due to depletion of Kidney Water, or dry mouth and tongue, use Jiaban Bawei Wan (Modified Eight-Ingredient Pill with Rehmannia). If one first experiences dry mouth and thirst with frequent urination before developing a carbuncle, or experiences thirst and drinking after the carbuncle heals, or has a yellow, dry, and stiff tongue with frequent urination and development of a carbuncle, these are all ominous signs. If the cause can be anticipated in advance, taking Jiaban Bawei Wan (Modified Eight-Ingredient Pill with Rehmannia) and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) beforehand to nourish the source of transformation can prevent subsequent complications.
[Zhu Danxi] Thirst occurring during the onset of abscesses and carbuncles indicates deficiency of both Qi and Blood. Use Renshen (Ginseng Root) to tonify Qi, Danggui (Angelica Sinensis Root) to nourish Blood, or use Rendong Wan (Honeysuckle Pill) or Huangqi Liuyi Tang (Astragalus Six-to-One Decoction).
Wu Shi stated: When thirst from sores is extremely urgent, administer Shenxian Zhuidu Wan (Immortal's Poison-Chasing Pill) to purge evil toxins. Qingchan Tang (Clearing Epigastric Decoction), Qianjin Lulu Tang (Thousand-Gold Desmodurus Decoction), Wuxiang Lianqiao Tang (Five-Aroma Forsythia Decoction), Liuwei Cheluo San (Six-Ingredient Ark Shell Powder), and Wanjina San (Ten-Thousand-Gold Powder) can all be selected. If thirst persists after purgation, then use herbs that generate fluids and tonify Qi. Once fluids are generated and Qi and Blood are complete, thirst will cease naturally. Danxi said: Administering purgatives for extreme thirst, without clearly indicating the pulse and symptoms, how does one know when purgation is indicated? The later statement that one should use tonics only after purgation if thirst persists, without specifying the pulse and symptoms, suggests that perhaps only extreme thirst necessitates purgation, and tonics are given only if thirst remains afterward. Is this appropriate for the ancient principle of treating disease before it arises? Moreover, thirst belongs to the Upper Jiao; it might be applicable during the ulcerative stage of swelling, but after ulceration, thirst is mostly due to Qi and Blood deficiency. Why wait until after purgation to discuss deficiency?
The *Jingyao* states that dry mouth and thirst are different. It is inappropriate to use mineral pills for suppression; doing so brings disaster like turning over a hand. Instead, use Sangzhi Jian Wuwei Tang (Mulberry Twig Decoction with Schisandra) to rescue Yin water. Danxi said: This is an excellent formula. However, it does not mention fire arising from dietary tastes, anger, or sexual exhaustion. I fear that thirst may not be easily eliminated under such circumstances.
Li Shi stated: Thirst appearing after the healing of carbuncles often leads to untreatable conditions; only Jiaban Bawei Wan (Modified Eight-Ingredient Pill with Rehmannia) is most effective. Generally, abscesses and carbuncles arise from deficiency. For those whose carbuncles have settled but who experience thirst, taking this pill stops the thirst. For those who are stable without thirst, taking this pill prevents future thirst. For those who experience thirst before developing a carbuncle, taking this medicine not only stops thirst but also prevents the carbuncle from forming. Xue Ji stated: The previous condition belongs to depletion of Kidney Water with deflection of deficient fire upward, causing dry mouth and thirst with excessive drinking. Or yellow tongue coating with cracks, frequent urination. Or phlegm-Qi obstructing upwards, restlessness and irritability. Or experiencing thirst and drinking for two or three years prior. Or sores on the mouth and tongue, heat in the soles of the feet, phlegm-Qi obstructing upwards. Or dry mouth and thirst with frequent urination three or four years after the sore heals. Urgently using the aforementioned medicines often results in recovery. By anticipating the cause and taking the aforementioned pills in advance, this affliction can be avoided. If accompanied by cold extremities, it indicates deficiency of True Yang, and Bayi Wan (Eight-Ingredient Pill) is suitable. Generally, after sores, if Kidney Water is insufficient, use Jiaban Bawei Wan (Modified Eight-Ingredient Pill with Rehmannia); if Middle Qi is weak, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction); if Qi and Blood are weak, use Shiquan Dabu Tang (Complete Great Tonifying Decoction); if Yang Qi is deficient and cold, add Shengjiang (Fresh Ginger) and Rougui (Cinnamon Bark); if ineffective, use Bayi Wan (Eight-Ingredient Pill).
Modifications to Tuoli Xiaodu San (Interior-Supporting and Toxin-Expelling Powder):
Drinking cold liquids causes thirst, indicating Heat-Toxin; add Chixiaodou (Adzuki Bean) and Zhimu (Anemarrhena Rhizome). If ineffective, temporarily use Zhuye Huangqi Tang (Palm Leaf and Astragalus Decoction).
Excessive appetite with thirst indicates Stomach Fire; add Shigao (Gypsum Fibrosum), Shanzhi (Gardenia Fruit), and Tianhuafen (Trichosanthes Root). If ineffective, temporarily use Zhuye Shigao Tang (Palm Leaf and Gypsum Decoction).
Poor appetite, fatigue, and thirst indicate deficient Stomach Qi; remove BaiZhi (Angelica Dahurica Root), Lianqiao (Forsythia Fruit), and Jinyinhua (Lonicera Flower), and add Renshen (Ginseng Root) and Baizhu (Atractylodes Macrocephala Rhizome). If ineffective, temporarily use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
Profuse pus with thirst indicates deficiency of Qi and Blood; remove the previous three herbs, and add Shudihuang (Prepared Rehmannia Root) and Wuweizi (Schisandra Berry). If ineffective, temporarily use Shiquan Dabu Tang (Complete Great Tonifying Decoction) added with Wuweizi (Schisandra Berry) and Maimendong (Ophiopogon Tuber).
Dry mouth and tongue indicate deficient Kidney Qi; remove the previous three herbs, and add Shudihuang (Prepared Rehmannia Root), Shanzhuyu (Cornus Fruit), and Shanyao (Dioscorea Rhizome). If ineffective, combine with Liuwei Wan (Six-Ingredient Pill). If still ineffective, assist with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
Spontaneous sweating, internal heat, and dry mouth indicate deficient Stomach Qi; remove the previous three herbs, and add Renshen (Ginseng Root), Danggui (Angelica Sinensis Root), and Baizhu (Atractylodes Macrocephala Rhizome). If ineffective, temporarily use Liujunzi Tang (Six-Gentlemen Decoction).
Night sweats, internal heat, and dry mouth indicate deficiency of Yin Blood; remove the previous three herbs, and add Shudihuang (Prepared Rehmannia Root), Maimendong (Ophiopogon Tuber), and Wuweizi (Schisandra Berry). If ineffective, temporarily use Danggui Liuhuang Tang (Angelica and Rheum Decoction).

Xue Ji stated: The previous condition is one of the Seven Evil Signs. This formula treats it with very rapid effect. In cases where the left hand pulse is floating, large, and hot or accompanied by thirst, exterior pathogens should be dispersed. In cases where the right hand pulse is deep, solid, and hot or accompanied by thirst, interior pathogens should be cleared. If thirst occurs after ulceration, or with dysuria resembling strangury, or with a large but forceless pulse, it indicates deficiency of Qi and Blood, requiring major tonification of Qi and Blood. Some believe that persistent fever or thirst after ulceration constitutes a败证 (failure syndrome) that is untreatable. However, if the patient has a history of Stomach Fire, or residual heat-toxin in the Stomach Meridian, and the Stomach pulse remains rapid, they should still take the aforementioned decoction. If, after taking it, the pulse becomes larger, fever increases, and thirst worsens, then it is truly a failure syndrome, caused by deficiency of True Qi and excess of Pathogenic Qi. Currently, patients with ulcerations fear Shigao (Gypsum Fibrosum) and Huangqin (Scutellaria Root) because of their cold nature and refuse to take them. If the Stomach pulse has not stabilized, no other herb can treat this. If delayed for a long time, the Qi and Blood are scorched, more pus and blood lead to greater deficiency, resulting in a failure syndrome. Physicians should prioritize the pulse over the symptoms. Generally, in sores and ulcers, seeing one or two of the Seven Evils is very frightening; deficiency syndromes presenting with evil signs are untreatable; excess syndromes without evil signs heal spontaneously.
