[Mei] Treats the condition of excessive flesh growth in back sores (exuberant granulation tissue), where the skin is insufficient to cover the wound, and exposure to wind leads to the formation of stiff, necrotic flesh. Use Shuihushi (Calcitum, 寒水石), calcined and ground into a fine powder, applied to the sore; then cover with Tonglv (Chalcitis Viridis, 铜绿) fine powder, and the stiff flesh will disappear.
Ancient prescription from *Tujing*: For treating malignant sores and carbuncles, or pain radiating to the perineum and thighs, accompanied by severe spasms pulling into the lower abdomen that are unbearable and can be fatal within one night. Use Huixiangmiao Ye (Foeniculum Vulgare Seedlings and Leaves, 茴香苗叶), crushed to extract one sheng of juice, taken orally three to four times daily. Apply the remaining dregs to the swelling; the root may also be used in winter. This is a foreign prescription, used since the Yongjia era, with miraculous effects in reviving the dying.
Jingyao Xijiao Gao (Essential Rhinoceros Horn Ointment): Treats sores in the throat and tongue. In the past, a nobleman developed this condition due to a carbuncle. Physicians believed his heart function had failed and all resigned. I administered this medicine, and he was stabilized in one day.
Zhen Hupo (Amber, 真琥珀, ground) [1 qian], Sheng Xijiao (Fresh Rhinoceros Horn, 生犀角) [1 qian], Chensha (Cinnabar, 辰砂, ground), Fushen (Poria with Hostwood, 茯神, woody part removed) [2 qian], Zhen Naozi (Borneol, 真脑子, ground, 1 zi), Renshen (Ginseng Root, 人参, rhizome removed), Suanzaoren (Ziziphus Seed, 酸枣仁, skin removed, ground) [2 qian].
Grind Renshen, Fushen, and Xijiao into fine powder separately in a mortar with human milk. Mix other ingredients evenly, knead with honey to form an ointment, and store in a porcelain bottle. When the disease flares up, take a dose the size of a bullet, dissolve in strong decoction of Maimendong (Ophiopogon Root, 麦门冬, core removed), and take orally. Take five doses consecutively in one day for effect. (This prescription is not suitable for ulcerated wounds.)
[Xue] Mr. Dongyuan said: When heat-toxin from sores and ulcers is deep-seated and fixed, manifesting as vomiting, hiccups, cardiac discomfort, fever, irritability, pulse that is deep and forceful, local swelling that is hard and numb, and constipation, the toxin is in the zang organs. It should be unblocked, hence it is said: "Unblock the interior to cut off the source." It is also said: In sores and ulcers and other diseases, if the face is red, although there is latent fire-heat, do not rashly purge the interior. Instead, yang qi is constrained, and pathogenic factors are in the meridians; release the exterior to expel them, hence it is said: "When fire is constrained, disperse it." For constipation, even if food intake is large but the abdomen is not distended, do not use purgatives. If there is abdominal fullness and distension with constipation, it is due to dryness of the large intestine; use pig bile for enema. If constipation is due to deficiency of qi and blood in the stomach and intestines, use Shiquan Dabu Tang (Decoction for Ten-fold Tonification, 十全大补汤) to nourish. If the sore belongs to the yang syndrome, or constipation results from kidney injury due to sexual excess, use the aforementioned formula plus Jiang (Ginger, 姜) and Fu (Aconite Root, 附子) to restore yang; many survive. The classics state: The kidneys open into the two yin orifices, and essence is stored in the kidneys; when fluids are moistening, bowel movements are normal. If such symptoms occur in ulcer patients due to qi and blood deficiency and intestinal dryness, major tonification is best. If one fails to examine deficiency and excess and blindly uses draining methods, errors are common. One person suffered from a back carbuncle in midsummer, dark and swollen over a chi, with multiple heads resembling millet grains, lasting four days. This indicated deficiency of true qi and excess of pathogenic factors. Garlic-separated moxibustion was applied, and Muling Yin (Decoction for Saving Lives, 活命饮) was taken for two doses, causing the pathogen to recede rapidly. Then, pure tonification of true qi was employed, and Shengmai San (Pulse-Nourishing Powder, 生脉散) was used as tea substitute, greatly reducing the sore's pathogens. I left for other business and revisited after three days. Food intake had ceased, dead flesh appeared in the center, constipation occurred, and urine was red and turbid. I said: Central necrosis indicates abundant toxins and spleen qi deficiency. Constipation indicates intestinal weakness failing to transmit. Red turbid urine indicates spleen deficiency with sinking fire. Treatment is difficult. He initially asked: Is this due to interrupting tonic medicines? I said: Yes. Urgently, Liujunzi Tang (Six Gentlemen Decoction, 六君子汤) was modified with Danggui (Angelica Sinensis, 当归), Chaihu (Bupleurum Root, 柴胡), and Shengma (Cimicifuga Rhizome, 升麻). Food intake gradually improved, and bowel movements normalized. Waiyong Wujiangao (External Black Gold Ointment, 乌金膏) was applied to the center, about three cun. Surrounding redness and swelling subsided, and central black necrosis cleared. Then, Danggui Gao (Angelica Ointment, 当归膏) was applied, and Dihuang Wan (Rehmannia Pill, 地黄丸) was alternated with the internal medicine. After one hundred doses, he recovered. Another man had a thigh abscess. Due to taking harsh purgative drugs, his yuan qi was damaged, and pus could not form. I used Tuoli (Supporting the Interior) methods to induce suppuration, followed by major tonification to promote healing. However, stool was dry and hard. Shiquan Dabu Tang was used with added Maimendong and Wuweizi (Schisandra Fruit, 五味子) to moisten. After more than a month, it remained hard. Confused by others' advice, he took Runchang Wan (Intestine-Moistening Pill, 润肠丸), leading to persistent diarrhea. I stopped this using Buzhong Yiqi Tang (Decoction for Supplementing the Middle and Augmenting Qi, 补中益气汤) to deliver Sishen Wan (Four Spirits Pill, 四神丸) several times.
Runchang Wan: Treats latent fire in the spleen and stomach, dryness of the large intestine, or wind-blood stasis. Use this pill to unblock. If impaction is in the rectum, use pig bile enema. The kidneys govern the five fluids and open into the two yin orifices. If fluids are moistening, bowels are regulated. If fluids are insufficient and spleen qi is deficient, tonification is necessary, especially avoiding the aforementioned drugs.
Huo Ren (Hemp Seed, 麻子仁), Tao Ren (Peach Kernel, 桃仁, skin and tip removed) [1 liang each], Qianghuo (Notopterygium Rhizome, 羌活), Danggui Wei (Angelica Sinensis Tail, 当归尾), Huangda (Rhubarb Root, 大黄, stir-baked), Zaocishi (Gleditsia Spine, 皂角刺), Qin Jiao (Gentiana Macrophylla Root, 秦艽) [5 qian each].
Grind each ingredient separately into powder. Form pills with refined honey or pig bile, size of Sophora seeds. Take 30-40 pills per dose with warm water. If pig bile enema is used without dryness, urgently tonify yuan qi.
