According to the *Treatise on the Pathogenesis and Symptoms of Various Diseases* (Bing Yuan Lun): The manifestation of tinea (Xuan) involves skin eruptions resembling coin patterns, which gradually expand. These lesions may be round or oblique, with distinct borders, accompanied by itching and pain. Insects are said to exist within the tinea; scratching yields fluid. This condition arises from wind-damp pathogenic factors invading the Couli (interstices of skin and muscles), compounded by cold-dampness clashing with blood and Qi. When the flow of blood and Qi becomes obstructed and sluggish, this disease develops.
Wind-type tinea (Feng Xuan) results from malicious wind and cold air invading the skin, impeding the movement of blood and Qi. It also presents with circular patterns and borders, but scratching causes numbness and insensitivity to pain or itch, and insects are present internally. There is also a condition called "Yi Feng Chuang" (Wandering Wind Sore), where the whole body exhibits symptoms similar to tinea and scabies with intense itching. This occurs when wind qi dissipates into the skin, hence the name.
Dry tinea (Gan Xuan) features only borders, with dry, rough, and itchy skin; scratching produces white scales. This too stems from wind-damp pathogens lodging in the Couli, interacting with cold-dampness and blood/Qi. If wind-toxic qi is abundant while dampness is scant, the wind sinks deeply, resulting in a lack of exudate, thus forming dry tinea, which also contains insects. White tinea (Bai Xuan) appears white and itchy; it arises because deficiency in the Couli allows wind invasion. Wind combines with Qi, causing blood stasis that fails to nourish the muscles.
Damp tinea (Shi Xuan) also has borders resembling insect trails, with oozing, red, and moist areas. Scratching due to itch leads to fluid discharge and sore formation. This happens because wind-toxic qi is superficial while dampness predominates, creating damp tinea, which also breeds insects.
Zhu Danxi stated: For treating tinea sores, use Fangfeng Tongsheng San (Saposhnikovia Decoction for Unblocking the Master Channels), removing Mangxiao (Mirabilite) and Dahuang (Rhubarb), and adding Fuping (Azolla Herba) and Zaojiaoci (Gleditsiae Spina).
Another prescription for treating tinea uses:
Fuping (Azolla Herba) (1 liang), Can'er (Xanthium Fructus), Cangzhu (Atractylodis Rhizoma) (2 liang), Kushen (Sophorae Flavescentis Radix) (1.5 liang), Huangqin (Scutellariae Radix) (0.5 liang), Xiangfu (Cyperi Rhizoma) (2.5 liang).
Grind all into powder. Take with wine, or make pills with wine paste.
Heshouwu San (He Shou Wu Powder) (from Luo's collection): Treats wind-toxin of the Spleen and Lung attacking the body causing swelling, tinea transforming into urticaria, sores from scratching, stiffness and spasm of the shoulders and back, muscle numbness, soreness and cracking of hands and feet, wind qi attacking the head and face causing sores, as well as purpura, vitiligo, and stubborn numbness caused by wind. All these conditions are suitable for this formula.
Jingjie Sui (Schizonepetae Spica), Manjingzi (Vittadiniae Fructus), Weilingxian (Salviae Miltiorrhizae Radix et Rhizoma - *Note: Standard term is usually Salvia root, but text says Weilingxian which is often Saposhnikovia or Cynanchum*, here likely Saposhnikovia based on context of wind), Heshouwu (Polygoni Multiflori Radix), Gancao (Licorice Root, honey-fried), Fangfeng (Saposhnikoviae Radix). [Text lists "Ke Cao" which seems like a typo or obscure herb, possibly referring to a specific local herb or error, kept as transliteration if unknown, but likely refers to a wind-dispersing herb].
