If the fetus has not turned yet and the mother exerts effort too early, causing the fetal feet to be pushed out, the mother should lie flat on her back calmly. The midwife should gently push the fetus back in with her hand, waiting for the fetus to naturally align itself. If delivery does not occur after a long time, the midwife should insert her hand into the birth canal, maneuver the fetus from one side to face the correct birth passage, and then administer Foshou San (Buddha's Hand Powder) to induce labor. Once the fetal head is correctly aligned with the birth canal, the mother can exert effort to deliver the baby.
Transverse presentation occurs because the fetus is positioned horizontally, and the mother’s sudden or excessive pushing forces the fetus into an abnormal position. The mother should lie on her back while the midwife gently guides the fetus into alignment. The midwife should use her middle finger to probe the fetal shoulder, ensuring the umbilical cord is not pulled taut. Once the fetal head is aligned, administering Foshou San (Buddha's Hand Powder) will facilitate delivery.
Foshou San (Buddha's Hand Powder) (see previous text).
(Attachment: Summer Delivery and Cold-Induced Delivery)
In cases of prolapsed rectum during delivery, the rectum protrudes before the fetus is born. If the intestine protrudes, it should be kept in a very clean, unbroken lacquerware vessel. Immediately, grind forty-nine seeds of Ricini Semen (Castor Bean) into a paste and apply it to the crown of the mother’s head; the rectum will retract. Wash off the Ricini Semen with warm water afterward. If the rectum has dried, moisten it slightly with water used for sharpening knives, and then have the mother drink a decoction of high-quality magnetite prepared by geomancers; the rectum will retract. Another method: dip paper in sesame oil, ignite it, extinguish the flame, and use the smoke to fumigate the mother’s nostrils; the rectum will retract. This method is highly effective.
Summer delivery refers to situations occurring in the height of summer, where intense heat steams and oppresses the mother, causing dizziness and facial redness. Place a basin of well water beside the bed to absorb the summer heat. If cool winds or rain occur, close the doors to avoid exposure.
Cold-induced delivery refers to severe cold in winter causing stagnation of Qi and blood, preventing the fetus from descending. This condition is most harmful. The mother should keep her back facing the fire and maintain warmth in the lower body so that blood circulation resumes, facilitating delivery. Generally, when abdominal pain occurs, the mother should wait patiently and sit on a birthing stool. When the timing is right, she should deliver quickly; delay may lead to complications. Urgently administer Foshou San (Buddha's Hand Powder) and other labor-inducing medicines. Assess the situation carefully and ensure rapid delivery.
Cui Sheng Ru Shen San (Divine-Efficiency Labor-Inducing Powder)
Soot from the bottom of the cooking pot (Caogenshuang, i.e., soot. This substance implies stopping bleeding as red turns to black due to fear of excessive blood loss; do not use postpartum). Angelicae Dahuricae Radix (Baizhi, do not roast over fire).
Grind both ingredients into equal parts powder. Take three qian per dose. Mix with child’s urine and rice vinegar to form a paste, then take with boiling water, or mix with wine and child’s urine and take hot.
Shen Qin Wan (Divine Rest Pill)
Resina Incensi (Ru Xiang, clear dripping frankincense) (five qian, separately ground). Aurantii Fructus (Zhi Ke, bitter orange fruit, roasted).
Grind both into powder, mix with refined honey to make pills the size of tung tree seeds. Take thirty pills with warm wine.
Cui Sheng San (Labor-Inducing Powder)
Abelmoschi Semen (Huang Shu Kui Zi, half he, ground into paste).
Filter the residue using wine and take warm.
There are many formulas for inducing labor, such as Rabbit Brain Pills, which are difficult to prepare and hard to find at present. However, I have never used them myself. I primarily rely on Foshou San (Buddha's Hand Powder), which has proven effective in many cases. Here, I briefly list the above three formulas for ease of access. Ye Shi recorded a case where the mother died but the fetus survived and could not be delivered. In such a case, place mercury the size of a bullet into the mouth via the throat, lift the body downward, wait for the duration of a meal, then lift the body upward; the fetus will be delivered. This is recorded here for reference only and should not be used.
