Newly postpartum patients presenting with rapid breathing and panting (asthma) due to the sudden depletion of Ying Blood (Nutritive Blood), leaving Wei Qi (Defensive Qi) without a foundation, is termed "Lone Yang" (Gu Yang). This is an extremely critical condition. Immediate treatment should employ Liuwei Dihuang Tang (Six-Ingredient Rehmannia Decoction) added with Renshen (Ginseng) to tonify Yin. If caused by Spleen and Lung deficiency, use Sijunzi Tang (Four Gentlemen Decoction) added with Danggui (Angelica Sinensis) and Heijiang (Charred Ginger) to tonify Yang. If spontaneous sweating and cold extremities are observed, add Fuzi (Aconite Root). In cases of false heat causing panting with profuse sweating, Shiquan Dabu Tang (Ten Significant Tonifying Decoction) added with Fuzi (Aconite Root) must be used in large doses to save the patient. The most dangerous scenario is rapid panting inducing Stasis Blood to rush upward; if it enters the Lung, epistaxis (nosebleed) occurs, indicating impending Lung Qi exhaustion; if it enters the Heart, confusion and loss of consciousness occur, indicating impending Heart Qi exhaustion. For cough and asthma due to external contraction, or for those with chronic Phlegm-Wheezing syndrome, there is no danger; treat with Sijunzi Tang (Four Gentlemen Decoction) added with Sugen (Perilla Stem), Chenpi (Tangerine Peel), Paojiang (Prepared Ginger), Danggui (Angelica Sinensis), and Xiong (Ligusticum Chuanxiong). *Liuwei Tang* (see section on Aphasia); *Sijunzi Tang* (see section on Blood Syncope, i.e., Liujunzi Tang minus Banxia and Chenpi); *Shiquan Dabu Tang* (see previous sections).
For diseases occurring postpartum, whether accompanied by profuse sweating, rapid panting, or fever, close observation is required. A yellow and bright nose tip indicates survival. A blue-green nose tip, blackness around the mouth, and coal-like black spots inside the nose are all signs of organ failure and death. If rapid panting occurs, Erwei Rensu Yin (Two-Ingredient Ginseng and Perilla Decoction) may rapidly rescue failing Qi. Zhang Jingyue stated: Inability of Qi to ascend and descend results from rootless Yuan Qi (Primordial Qi), with Lung and Kidney deficiency causing panting. When coal-like blackness appears on the nose and lips, Qi is about to collapse. Immediately administer Zhenyuan Yin (True Origin Decoction) to rescue this condition; this is the general principle. Coal-like blackness appearing in the nose signifies the exhaustion of True Yin and the impending failure of the Metal (Lung) and Water (Kidney) organs; fewer than one in ten can be saved.
**Rensu Yin (Ginseng and Perilla Decoction)**
Renshen (Ginseng Root) (1 liang)
Sumu (Sappan Wood) (3 qian, pounded into fine powder)
Decoct in water and take orally. Add Shufu (Prepared Aconite Root) (2-3 qian) if cold extremities and spontaneous sweating occur.
**Zhenyuan Yin (True Origin Decoction)**
Dushudi (Prepared Rehmannia Root) (3 liang)
Danggui (Angelica Sinensis Root) (3 qian)
Zhigancao (Honey-fried Licorice Root) (1 qian)
Add Rougui (Cinnamon Bark) (1 qian) for Yin-cold and cold feet, or add Paojiang (Prepared Ginger) and Renshen (Ginseng Root) (each 3 qian). Decoct in water and take frequently.
If parturient women mistakenly administer cold-purgative powders, resulting in a black and dry tongue, or a smooth red tongue without coating, these are critical conditions. These arise from excessive sweating or overuse of Child's Urine without protecting the Heart and Kidney. The tongue is the sprout of the Heart and the root of the Kidney. If the tongue is black but moist, recovery is possible. Treat with Liuwei Tang (Six-Ingredient Decoction) using heavy doses of Shudi (Prepared Rehmannia Root), added with Renshen (Ginseng Root) and Paojiang (Prepared Ginger). If the tongue is dry, black, withered, and accompanied by thirst, Blood has been exhausted, and the condition is incurable. At this time, only Dushen Tang (Single Ginseng Decoction) of approximately 1 liang, administered frequently, might save one in ten. Without Ginseng, there is no benefit. Since Kidney Qi is exhausted, harsh and hot herbs like Guizhi (Cinnamon Twig) and Fuzi (Aconite Root) should not be used to further deplete the body; only Renshen (Ginseng Root) can rescue Qi from nothingness. Furthermore, Yang generates Yin and produces fluids, offering hope for recovery. This condition often appears within ten days postpartum; otherwise, it is due to sweating causing Yang collapse or lower deficiency causing Yin collapse. For example, in dysentery, a withered tongue or a smooth red silk-like dry tongue with thirst indicates an untreatable condition, specifically lower deficiency and Yin collapse.
**Dushen Tang (Single Ginseng Decoction)**
Use high-quality Shuren (Mature/Prepared Ginseng Root) (1 liang), stew into one bowl. Alternatively, add Wuweizi (Schisandra Berry) (1 qian) and administer frequently. Or use Renshen (Ginseng Root) (5 qian) and Dushudi (Prepared Rehmannia Root) (2 liang), named Liangyi Gao (Two-Polymer Paste), decocted and taken orally, which is also effective.
