Generally, sores appearing on the human body are caused by heat from the Zang organs rushing outward, combined with external Wind-Dampness fighting together. Among these, if the Wind-Dampness carries Heat-Toxin, the sore will be painful, itchy, and swollen, failing to heal for a long time; thus it is named "E Chuang" (Malignant Sore).
Children injured by Wind-Heat Toxin Qi, where the pathogen lodges in the skin, forming pustules that subsequently ulcerate into sores, is named "Chuang" (Sore/Pox).
When children develop Heat Sores on their bodies, they inevitably develop "Luo Li" (Scrofula/Tuberculous lymphadenitis). Its presentation appears as nodular masses located between the skin and flesh, often two or three connected together. This arises because Wind-Evil fights with Blood and Qi, causing stagnation and knotting.
"E He" (Malignant Nodule) occurs when Wind-Heat Toxin Qi fights with Blood and Qi, condensing into a nodule that grows beside the neck. If additionally constrained by Cold-Wind, it fails to dissipate or suppurate, hence named "E He".
Regardless of gender or age, individuals constitutionally intolerant to lacquer who encounter lacquer trees or newly lacquered vessels will contract Lacquer Poisoning. This causes swelling of the head, face, and body, presenting with hidden rashes, red coloration, and sores accompanied by itching and pain.
When the Six Fu Organs are disharmonious, Cold Evil lodges in the skin. Cold fights with Blood, leading to obstruction and stagnation that remains in the meridians. It knots and swells at the head, forming an "Yong" (Abscess). Its manifestation is characterized by swelling with thin, glossy epidermis. When Heat Qi attacks this condition, and Heat surpasses Cold, the flesh and blood rot and transform into pus. After the pus discharges, the sore fails to heal, hence termed "Yong Chuang" (Abscess Sore).
The presentation of "Chang Yong" (Intestinal Abscess) involves slight hardness and pain in the lower abdomen. This is generated by the fighting of Cold and Heat Qi within the intestines, causing Qi and Blood to become obstructed and knotted.
A knotted swelling measuring one to two cun in length is called "Jie" (Furuncle/Boil). Like an Yong, it presents with heat and pain; after a long time, pus discharges, and once the pus and blood are fully expressed, it heals. This is also formed by Cold-Wind Qi lodging in the skin, causing Blood and Qi to stagnate and knot. For all Yongs and Jies, if the pus and blood are not fully expressed during squeezing before the wound closes, malignant fluid remains inside. Although it may appear healed superficially, it will eventually recur and transform into "Lou" (Sinus/Fistula).
If the Five Zang Organs are unregulated, "Ju" (Deep-seated Abscess) develops. This too results from Cold Evil lodging in the skin, constraining Blood and Qi, causing them to become obstructed, stagnant, and unable to flow smoothly, leading to knotting and aggregation. Generally similar to Yong, the difference lies in its surface texture being hard like the skin of an ox's neck. Yong is superficial, whereas Ju is deep. Regarding tissue necrosis, suppuration, and discharge, Ju is more severe than Yong; it damages bones and rots sinews, potentially leading to death.
