Cases where the infant is expelled through the anus after childbirth occur because the sitting or lying position was unstable, and the mother rushed to force expulsion. The violent impact of Qi causes the infant to lose its proper path. Women may give birth in either a sitting or lying position. If giving birth while sitting, one must sit upright, with assistants holding the ribs and waist firmly to prevent tilting, so that the infant follows the natural order. If giving birth while lying down, one must lie flat with the back level against the bed and the body not curved, ensuring the infant does not deviate from the correct path. If the sitting or lying position is unstable, with the body slanted or curved, and the infant is turning when the mother rushes to force expulsion, the violent impact of Qi will cause the infant to move toward the anus, resulting in transverse or breech presentations. These are all due to haste during childbirth or violating taboos, leading to an unstable posture. Carefully examining one's actions reveals that the sitting and lying positions during childbirth must be level and upright, following the directions of the four seasons and avoiding Five Elements taboos. Violations often lead to disasters.
If the mother dies while the infant remains inside, it may be due to violating taboos, premature delivery caused by fright before the time for labor has arrived, or improper support by attendants holding the waist. These factors cause difficult labor, leading to fetal Qi rushing upward to cover the Heart, resulting in syncope and death. Observing the mother in her final moments: if the face is blue, the tongue is red, and blood gushes from the mouth, the mother dies while the infant survives. Therefore, the orientation of sitting and lying in the birthing area must follow the seasonal directions and avoid Five Elements taboos; violations often invite disaster.
Difficult labor with the infant dead in the womb is mostly caused by premature disturbance or violating taboos. In difficult labor, foul fluids flow downward. If the time for delivery has not yet arrived but lochia has ceased, the fetus becomes dry and withered, resulting in death within the womb. Signs include a blue-black tongue and coldness of the fetus, indicating the infant has died. Thus, the orientation of sitting and lying in the birthing area must follow seasonal directions and avoid Five Elements taboos; violations often invite disaster.
The condition of Xue Yun (postpartum syncope) is characterized by mental restlessness and breathlessness approaching cessation. This can result from excessive blood loss or, conversely, extremely scanty bleeding, both leading to syncope. If postpartum blood loss is excessive, leading to extreme deficiency of Blood and Qi, the resulting syncope manifests merely as mental distress. If bleeding is scanty but Qi is rebellious, the Qi rises upward to cover the Heart, also causing syncope, manifested as mental distress and chest fullness/urgency. These two conditions differ. One should observe the amount of postpartum bleeding to determine whether syncope will occur. However, unrelenting mental distress can lead to death. During childbirth, attention must be paid to sitting and lying orientations; violating taboos often leads to syncope and variable bleeding amounts. Therefore, in the birthing area and regarding posture, one must follow seasonal directions and avoid Five Elements taboos; violations often invite disaster.
